Introduction

This unit consists of two modules. Module One explores the burden of disease. Students will explore an evidence-based, multidisciplinary, person-centred approach to care planning. Through this approach students will develop multiple care plans addressing the continuum of care from primary prevention to the acute exacerbation of chronic disease. In Module Two students will develop health promotion frameworks to address the complex interplay of factors leading to improved rural/remote health outcomes.
