People with chronic conditions and complex care needs who are being managed under an Enhanced Primary Care plan by their General Practitioner are eligible for a Medicare rebate for EP services (under Medicare Plus).  The Chronic Disease Management (CDM) items include, but are not limited to the management of:

• heart disease,
• diabetes,
• obesity,
• asthma,
• cancer,
• neuromuscular and musculoskeletal disorders,
• arthritis
• mental health conditions. 

Seven out of 10 consultations by General Practitioners within Australia are chronic-disease related.
