Professor Leigh Blizzard is the senior biostatistician at the Menzies Institute for Medical Research, where he is a Senior Member. He has expertise and rich experience in the practice of biostatistics, and skills in designing and developing analytical strategies for health research projects. The small size of the research group in the early days of the Institute provided an unparalleled opportunity for him to take hands-on responsibility for the design, conduct, analysis and reporting of internationally-recognised research in sudden infant death syndrome (published in JAMA), early life determinants of disease (BMJ, The Lancet, Circulation), multiple sclerosis (BMJ) and musculoskeletal conditions (Arthritis and Rheumatism).

Biography

Since completing his PhD in 2000, Leigh has built statistical capacity at the Institute to 7 positions and, in statistical research, has contributed to developments in log-link modelling of binary, multinomial and ordinal outcomes to provide estimates of risk ratios in follow-up and cross-sectional studies. Recently he has established a statistical research program in assessment of goodness-of-fit of log-link models in collaboration with Professor David Hosmer, a noted international authority. The significance of his research in statistical methods has been recognized with a NHMRC Project Grant, a Population Health Career Development Award and a Career Development Fellowship.

Expertise

Leigh's expertise is based on practical experience in the conduct of high-quality epidemiological studies. He has been personally responsible for ecological and cross-sectional studies, case-control studies, cohort study follow-ups, data linkage studies, and family and genetic studies of brain and lung cancer, non-Hodgkin's lymphoma, melanoma and non-melanoma skin cancer, and stroke. Additionally, he has supervised or guided studies of adolescent self-harm, alcohol consumption, bicycle injuries, cardio-metabolic risk factors, diet, disorders of cognition and gait, eating behaviours, economic cost (direct and indirect), geographic distribution of health, headaches of cervical origin, hypertension, iodine deficiency, immunization, falls by thoroughbred jockeys, falls by older adults, musculoskeletal conditions, physical activity, quality of life, renal disease, respiratory conditions (including a RCT published in the BMJ), tobacco smoking, urinary incontinence, vascular health, vitamin D and youth suicide.

The wide range of Leigh's contributions in guiding epidemiological research (see publications), encompassing all research designs and many different forms of analysis, attests to his leadership and research impact. Following research he supervised into falls by thoroughbred racing jockeys, the Australian Racing Board has implemented several changes. Leadership by a statistician is judged also by their influence in helping others to apply existing statistical methodology, and in developing or adapting statistical methodology where existing approaches are limited or circumscribed. Leigh has developed statistical methodology for the estimation of relative risk in cohort studies and for the assessment of the goodness-of-fit of the models used to estimate relative risk. Vindicating that work, which is of international significance, the editors of the American Journal of Epidemiology now recommend use of log binomial regression in the analysis of binary follow-up data.

Health by Stealth: Increasing Physical Activity through Active Travel in Tasmania (2018 - 2020)$282,362
Description
We are facing a global inactivity crisis. Participation in recommended amounts of physical activity (PA) has remained stagnant since the 1980s,indicating limited population-level impact of strategies to promote PA. A shift in focus from leisure-time to transport-related PA holds promiseas an attractive but under-exploited opportunity to support individuals to incorporate more activity into daily life. Healthy transport options walking, cycling and public transport are critical social determinants of health, by reducing traffic accidents, increasing social contact,reducing air pollution, and increasing exercise. Improving our understanding of travel behaviours and identifying strategies to increase use ofhealthy transport options have been identified by our three highly engaged Tasmanian Partner Organisations as important priorities. This projectseeks to generate evidence with service providers, state government and local government authorities to support decision-making and resourceallocation in relation to increasing active and public transport use. Working closely with our Partners, we will analyse data from existing datasources to better understand Tasmanians travel behaviours and the factors that influence these, by evaluating the effectiveness of two realworldfiscal strategies aiming to increase active and public transport use, and by developing and implementing a knowledge transfer strategy toensure findings are effectively communicated. This project will generate some of the first evidence internationally about the effectiveness offiscal strategies to increase active and public transport use, and will provide crucial information to directly impact the work of service providers,decision makers, policy-makers and practitioners. By identifying effective strategies to increase PA through active transport, the implications of this project have the potential to make major direct health, social, environmental and economic impacts globally.

REDucing Delays In aneurysmal Subarachnoid Haemorrhage: the REDDISH study (2018 - 2020)$436,023
Description
Aneurysmal subarachnoid haemorrhage (aSAH) is a rare form of stroke that kills up to 40% of sufferers within 1 month. Outcome can be improved through quickly receiving medical care but many people do not and this likely contributes to the poor outcomes. The REDucing Delays In Subarachnoid Haemorrhage (REDDISH) study will look at cases of aSAH across Tasmanian and Victoria so that we can understand the factors that contribute to delays in treatment and ultimately improve outcomes.

Cardiovascular imaging, especially echocardiography, is a cornerstone of modern cardiology; guidelines stipulate the use of echocardiography to inform therapeutic intervention in heart failure and valvular heart disease. However, there is also a risk of overuse, and in contrast to North America, where the growth of imaging has been controlled over the last decade, use in Australia continues to grow at ~10% per year. The dependence on imaging is greater in the cardiovascular disease (CVD) epidemics of the elderly (atrial fibrillation [AF], aortic stenosis [AS] and heart failure [HF]) than in other diagnoses. Not only are large numbers of tests being done currently, but the growth of these conditions will magnify the problem over the coming years. Failure to manage the hospitalisation, device and procedural costs for treatment of these new CV epidemics can be linked to shortcomings in the practice of CV imaging. The need for ongoing surveillance that is heavily dependent on cardiac imaging will introduce new complexities around imaging quality. Despite the expenditure of ~$1 billion/year on CV imaging in Australia, there is little evidence of value. The Partnership will link researchers with providers and imaging industry to improve all steps of the imaging "value chain" - ranging from the selection of suitable patients to the quality of interpretation. These studies will improve appropriate selection for testing, quality control and integration of decision-making with imaging.

Preclinical Development of Neuropeptide Y for the Treatment of Motor Neuron Disease (2017 - 2020)$936,575
Description
Motor Neuron Disease (MND) is a rapidly progressing neurodegenerative disease. There are no cures and no effective treatments for MND, with people diagnosed having a median survival of only three years from symptom onset. Not knowing the cause of MND or how it progresses through the CNS have been immense barriers to the development of effective therapeutics. We have reached a critical time in MND research. Our research team has been dedicated to researching the mechanism of MND for the last ten years. We are now poised to transition my teams research efforts from determining the cause of this disease to using this knowledge to develop treatments. A critical pathway that our work and others have shown is key to MND is altered network excitability and subsequent excitotoxicity in the cortex. We have recent published and preliminary data that identifies that a critical potential point of intervention in this pathway is via the subclass of inhibitory cells that express neuropeptide Y (NPY). The overarching Aim for this Cure for MND Translational Research Grant is to now target this pathway to develop a treatment. By performing this world-first preclinical trial we will reveal further aspects of disease pathogenesis. Positive outcomes from these trials will identify a candidate compounds for progression to a human trial for the treatment of MND.

Cardiometabolic risk trajectories from childhood to midlife: finding pathways to better health (2017 - 2020)$1,531,986

Using data from a large cohort of Australian children followed for over 30 years, this study will investigate how weight status and physical fitness from childhood to adulthood determine early markers of cardiovascular disease and type 2 diabetes (cardiometabolic disease) in midlife. It will provide much needed evidence to address the high prevalence of cardiometabolic disease risk factors in young Australians through better targeted disease prevention and early clinical intervention strategies.

Case-control study in Primary Progressive MS: Are the risk factors the same as for people with RRMS onset and can we identify new ones? (2015 - 2016)$142,000
Description
Examining risk factors of disease by comparing cases with a disease to healthy controls has been critical in understanding the aetiology of diseases. In MS, for example, important progress has been made through the epidemiological findings that cases were more likely to have had infectious mononucleosis, have high anti-EBNA IgG titres, and have low sun exposure levels. These findings resulted in a large body of laboratory work focusing on the mechanisms behind these observations. We have previously conducted epidemiological case-control studies on MS, but the number of people with PPMS was always too low to conduct a sub-analysis to examine whether the risk factors that we identified were also important in those with PPMS. We now propose to conduct a case-control of PPMS in a very cost-effective way, by using the data already collected on a large sample of healthy controls (n=558) from the Ausimmune study. We aim to collect the same data from a sample of cases with PPMS in Australia and compare that to the controls of the Ausimmune study. Moreover, the longer-term vision is that this data will form the basis for a prospective longitudinal study, where the cases with PPMS will be followed over time, in order to examine factors that are associated with the progression of MS. This will then be important for identifying possible treatments in MS for people with PPMS. HypothesesEstablished risk factors in MS, including low sun exposure, infectious mononucleosis, smoking, high anti-EBV-EBNA IgG and HLA-DR15 genotype are associated with an increased risk of PPMS onset.Low parity, lack of younger sibling exposure and exposure to livestock are associated with an increased risk of PPMS.People with PPMS have a higher EBV DNA load in whole blood and altered EBV serology compared to people with a RRMS onset.

Bariatric surgery in Tasmania: investigating health service use, costs, patient outcomes and policy options (2014 - 2019)$839,027

About 1 in 20 Tasmanians are severely obese. Bariatric surgery can improve health for individuals with severe obesity but access to surgery is poor for most public patients. This study will investigate the pathways of patients referred for bariatric surgery in the Tasmanian public and private hospital systems to better understand the demand for surgery, the health outcomes, and costs associated with these different pathways. The findings will inform public policy and resource allocation.

Guidance of Heart Failure Management Programs by Risk Assessment (2013 - 2018)$1,422,739
Description
The aims of this project are to reduce Heart Failure (HF) admissions and costs in Tasmania by: i) using data linkage to identify locations where community service delivery should be targeted, and to evaluate current performance in HF readmission across the State; ii) developing a risk algorithm for HF readmission, including psychosocial determinants; iii) using i) and ii) devising a novel means of providing a disease management program (DMP), titrated to risk assessment, with physiologic assessment used to guide the management of the highest risk patients; iv) developing a clinical trial to define the merits of this approach; v) evaluating the cost-effectiveness of this intervention; vi) presentation of educational activities with community-based providers. A successful intervention is likely to be sustainable. As HF readmissions in Tasmania cost ~4550 bed days/y, the investment in this grant (~$1/2m/y) could be reproduced by a 50% reduction of readmissions.

Much is known regarding the factors that increase the risk of developing multiple sclerosis. However there is little data regarding the importance of these and other factors on the rate of progression of MS. Once the diagnosis of MS is made one of the critical issues for patients and treating physicians is; what will happen now? As there are currently no established modifiable risk factors for MS progression, this project aims to delineate potentially modifiable risk factors for MS progression.

The quantum and range of co-contributors to his publications demonstrate the leadership role that Leigh takes in research training and mentoring of young researchers that has made a substantial contribution to the 6-fold growth since 2000 of research conducted at the Institute. As a CI on the NHMRC Capacity Building in Public Health Grant, he provided mentoring to all named postdoctoral fellow. By the end of the grant, 11 postdoctoral fellows were authors on 96 journal articles, five had been awarded postdoctoral fellowships, and all were investigators on new grants. Leigh has provided strategic analytical guidance to 6 other postdoctoral researchers who have reached level E positions and statistical support and early-career mentoring for 6 other postdoctoral researchers. He has supervised five completed PhD candidates (with 27 published papers from their theses) and is supervising three PhD candidates-in-progress (four published papers to date). He provides co-supervision for 9 other PhD candidates, and statistical support and mentoring for many others at the Institute.
