Once your research project has approval from the HREC, any changes you wish to make must be submitted for approval by the HREC. This might include, for example, a new method of recruitment or a change to the wording of the information sheet.
To submit an amendment, please complete an amendment form describing the proposed changes to the project. You will also be asked to identify any ethical issues arising as a result of this amendment, and to let the committee know if the information sheet and consent form (if applicable) need to be changed. Please also submit a copy of any project documentation that has changed as a result of this amendment. Changes made to these documents should be indicated in bold, highlighting, underlining or tracking for easy identification by the committee.
