The role of community pharmacy in the post hospital management of patients initiated on warfarin

This project aimed to improve the outcomes for patients discharged from hospital on warfarin by developing and implementing a sustainable program of home visits by trained accredited pharmacists.

This project was funded by the Australian Government Department of Health and Ageing as part of the Fourth Community Pharmacy Agreement through the Fourth Community Pharmacy Agreement Grants Program managed by the Pharmacy Guild of Australia.
