Improving anticoagulant management in the hospital setting: the establishment and evaluation of an Anticoagulant Pharmacist position

This project involved implementation of a separate warfarin therapy chart at the Royal Hobart Hospital to improve the quality of use of warfarin in hospital setting. Educating hospital staff and new warfarin patients, and improving post-discharge communication with GPs were also part of this project.

The project was funded by the Society of Hospital Pharmacists of Australia’s Abbott-Australasia Research Grant.
