WDREC Strategic Intervention Tool
This project examining the communication processes between the acute care sector and residential aged care facilities (RACF) in Tasmania focuses on facilitation of the transition of care for people with dementia. The project’s first phase was examining the processes undertaken with relation to the admission of a new resident to a RACF from the acute care sector, involving an initial document analysis followed by interviews to determine any points that can be improved upon.
Dependent upon the results the project may progress to redevelopment of referral documents and trialling of the revised process. If this occurs PWD will be tracked through the process of settling into their new RACF for the first month.
