Online Incident Notification System
Please use your University username account and password to login. If you have difficulties logging in, then email email@example.com or phone +61 3 6226 6298 for help.
When notifying via this online form, the system automatically generates a record and emails it to Officers, Business Managers, local Health Safety Representative (HSR) and the Work Health Safety (WHS) Staff. It is important that anyone raising an online incident is mindful that circulation of confidential inappropriate or sensitive information may offend. In accordance with the University Issue Resolution for Work Health and Safety Procedure if you select Work Health and Safety as the Organisational Unit, the information is only sent to WHS until the matter has been discussed with you.
