Incidents and Hazards should be lodged using the online system located  on the WHS Home Page    If you are unable to access the online Incident or Hazard system, the following forms can be completed and sent to the WHS Unit - Private Bag 46 - Hobart

• Notification of Incident/Injury Form 
• Safety Hazard Form  
• Injury/Incident/Near Miss Full Investigation Report  (PDF) (this form should be read in conjunction with the Incident Response Procedure )

The following forms should be completed after reading the minimum standards, policies, guidelines associated with these forms.  All policies, standards and guidelines can be found at policies and standards  on the WHS Home Page

• Task Risk Assessment (WORD 154.8KB) 
• Authorisation to Drive a University Vehicle
• Driver Medical Disclosure 
• Claim for Kilometreage Allowance  
• Appointment of a Designated First Aid Officer
