Professor Mark Nelson’s research focuses on the diseases of ageing and what can be done to prevent them.

Throughout his career, he has been involved in large-scale community-based clinical trials to investigate the effectiveness of strategies aimed at lowering the risk of cardiovascular and other diseases that particularly affect the elderly.

Professor Nelson is a principal investigator on the ASPREE study, which investigated the effectiveness of aspirin as a preventative therapy. ASPREE is the largest clinical trial ever conducted in Australia. It began 17 years ago, involved nearly 20,000 participants and has received about $100 million in funding.

There had been evidence to indicate that aspirin could lower the risk of cardiovascular diseases – heart attack and stroke – in the elderly and that it could also play a role in preventing cancer and dementia. ‘Some saw it as a wonder drug,’ Professor Nelson says.

However, the results of ASPREE – with three articles published in the prestigious New England Journal of Medicine in 2018 alone – have been clear: aspirin shows no benefit as a preventative therapy in the healthy elderly. In fact, it may be counter-productive and carries the risk of harmful side effects.

'The importance of high-quality research like this is that you get a definitive answer,’ Professor Nelson says. Previously, there had been a view that all elderly people should be taking aspirin. This research shows that aspirin should only be used for people who have already had a heart attack or a stroke as a way of preventing secondary events, not as a preventative strategy.

Conclusively demonstrating that taking aspirin is not a beneficial preventative strategy is a highly significant result. With elderly people often already taking a range of medications, each additional drug they take increases the risk of harmful interactions between them. ‘This becomes a leading cause of the elderly ending up in hospital rather than a way of keeping them out,’ Professor Nelson says.

Following on from ASPREE, Professor Nelson is also a principal investigator for STAREE. This large-scale clinical trial will use the same methods to evaluate the benefits of taking statins – cholesterol-lowering agents – as a preventative strategy.

'What is innovative about these trials is that we recruit healthy elderly people as the participants. Earlier trials had mostly recruited either middle-aged participants or elderly people who were already in secondary preventive care. But those groups are not the targets of these kinds of preventative strategies.'

For Professor Nelson, clinical trials allow medical researchers to question established practices and get definite answers as to whether they are truly beneficial, ineffective or even harmful.

'It is striking how often your assumptions are proven wrong,’ he says. ‘As a clinician, you can influence the health of maybe two or three thousand people at most, but as a researcher, you can improve the lives of millions.'

Mark Nelson is Professor and Chair, Discipline of General Practice, and Senior Professorial Fellow, Menzies Institute for Medical Research, and an Adjunct Professor, Department of Epidemiology and Preventive Medicine, Monash University. He is Medical Director of the Blood Pressure Clinic, MIMR, and is also in general practice in Hobart. His research interests are around large-scale clinical trials of cardiovascular disease prevention in primary care.

Biography

Mark did his PhD on a prospective study nested within the Second Australian National Blood Pressure study at Monash University. As a post-doc at Monash University he initiated the ASPREE study.

Expertise

Marks research interests are around large-scale clinical trials in primary care.  He has 250 peer reviewed scientific publications, has been awarded more than $80 million in competitive grants and is a principal investigator on the NIH sponsored ASPREE / ASPREE-XT study (N = 19,000) investigating if aspirin extends healthy active life, and the NHMRC sponsored STAREE (recruitment to date >3000) similarly investigating if statins extend healthy active life.

Research Themes

Mark’s research aligns to the University’s research theme of Better Health. To this end he has been a principal investigator or worked in the largest clinical trials every conducted in Australia, ANBP2 (6,083 participants), ASPREE (19,114 participants) and STAREE (ongoing more than 3000 participants). These are supported by the NIH and NHMRC and are all aspects of prevention of diseases common in the aged, cardiovascular disease, dementia and cancer.

Collaboration

Mark collaborates with the Bermann Center (ASPREE). The US has 2500 of the participants in ASPREE predominantly US minorities as was the stipulation of the NIH. Extensive local collaboration too through this project with Monash University, the University of Melbourne, University of Adelaide, CSIRO, NCI etc.

Oxford group Primary Care group. We are currently collaborating in a UK-Australian survey of current knowledge, attitude and stated practice of the use of blood pressure measurements made out of the clinic. This work will help us in the uptake of such measurements into routine clinical practice.Prof Ryuki Kassei (Fukushima University, Japan). Mark is mentoring the nascent family medicine registrar research group of Japan.

Love Your Brain: A stroke prevention digital platform (2023)$944,782
Description
Stroke is common with an estimated 1 in 4 people having a stroke in their lifetime. Prevention of stroke is a priority for the Australian public due to high levels of stroke risk factors, inadequate existing primary prevention strategies, and identification of prevention of stroke as a priority by stakeholders. There is also growing demand for information about how to prevent stroke in the community given the large number of people living with stroke or who are at high risk of stroke. The proposed digital platform aims to reduce the burden of stroke across Australia by harnessing the power of digital health to help people identify and manage their risk factors for stroke. The proposed platform will leverage the existing Stroke Foundation StrokeSafe program. The proposed platform has two components informed by evidence - the online, structured health education program and the iVERVE text messaging system including goal-directed short messages on risk factor management. Systematic review found that digital health interventions appear to have small but significant effects of cardiovascular risk factors. There is a need for further studies with rigorous designs, objectively measured outcomes and examination of cost-effectiveness. The outcomes of research project will include: - co-designed digital platform which will include an online course and the health messages that will build on StrokeSafe and other programs (i.e. iVERVE) to inform the design. - assessment of outcomes of participants in a 3-arm trial (control; online course and health messages interventions) to improve general practitioner attendance for risk factor management and knowledge of risk factors and appropriate behaviour in the community.

Annual seasonal influenza vaccination is recommended in a number of countries around the world to prevent influenza in children and adults. Older adults, particularly those with medical comorbidities, are at increased risk of influenza morbidity and mortality and are recommended to receive high-dose or adjuvanted influenza vaccine formulations. RSV and influenza are both typically seasonal diseases, with peaks during winter in temperate climates. Therefore, it is a possibility that RSVpreF may be given at the same time as seasonal influenza vaccine.The purpose of this study is to assess the safety and immunogenicity of RSVpreF when coadministered with SIIV compared to sequential administration of the vaccines when given 1 month apart (SIIV followed by RSVpreF). Additionally, the study will contribute data supporting the development of RSVpreF as a prophylactic vaccine against RSV disease in infants through maternal immunization and in older adults through active vaccination.

SAFER (AUS) Trial: Screening for Atrial Fibrillation with ECG to Reduce stroke - a randomised controlled trial (2021 - 2024)$1,925,223
Description
SAFER-AUS is the Australian (and only international) arm of the very large UK SAFER study: a cluster-randomised trial with a stroke endpoint. SAFER-AUS will recruit 18 general practices (5% of UK total), randomised 1:2 screening vs control. Participants aged ≥70 years in screening practices will be given a novel thumb-ECG device to record heart rhythm 4 times daily for 3 weeks. ECGs will be interpreted by an algorithm and confirmed by a cardiologist. Patients with AF will see their GP for review and management.

Description
Most risk factors for stroke are known and >80% of strokes can be prevented by simple lifestyle change and adherence to prevention medication. This is sub-optimally achieved, with modest detection of those at risk. While new breakthroughs can help, the greatest gains can be made by implementing recommended care.Each component of our application targets current gaps. THEME 1 will identify the best groups to target for prevention, and the optimal timing for prevention strategies, capitalising on the resources available to us. THEME 2 will test novel interventions aimed at motivating lifestyle changes and adherence to medications using technology and health service innovations. THEME 3 underpins the program by establishing national infrastructure to monitor the impact of new interventions. We can do this as members of the team are strategically placed to influence policy, guidelines, and practice for stroke.Unquestionably each component of the program would result in productive outcomes individually, but the real value lies in the collaborative gain that can be achieved through this interdisciplinary proposal. Our synergies include our complementary expertise in critical components of our program, and our state, national and international networks. Our overarching strategy is to be responsive to the outcomes of early work in the program and focus on the areas that show the most promise. This will ensure that we achieve outcomes with the greatest public health benefit.Funding of this international leading program will deliver major advances in preventing stroke and other vascular disease in Australia and overseas. Our approach, which covers diverse populations, is crucial to make major advances in preventing stroke.

Improved cardiovascular disease health service delivery in Australian (IDEAL study) (2019 - 2022)$1,071,230
Description
Cardiovascular disease (CVD) has the highest burden of global disease and highest level of health care expenditure in Australia. Most of the management of CVD is performed in general practice, where the recommended strategy is to determine absolute CVD risk based on multiple factors (e.g. cholesterol, blood pressure, diabetes, smoking). Australian primary care doctors are not yet adopting guideline recommendations, with as few as 30% using risk calculators, and this leads to people remaining at high risk for otherwise avoidable CVD events. All partners in this grant are working together to rectify this evidence-practice gap, using a new, yet simple and widely implementable, digital health service that has been developed by researchers, industry, health service providers and policy makers.The concept involves absolute CVD risk being measured at pathology services in patients referred for blood lipid assessment. Patients have blood pressure measured and CVD risk factors collated electronically by pathology services using an automated, high throughput approach, with absolute CVD risk results conveyed to general practitioners via an established pathology reporting system. Abnormal results are highlighted and advice provided regarding appropriate, guideline-directed action. Significant development work that confirms proof-of-concept has been completed. This program will refine and test the clinical value of the new health service method upon establishment statewide across Tasmanian pathology services. In an Australian first, the system is designed for research infusion, with all information data-based (through UTAS) and serving as a platform for rapid assessment of clinical value by linkage to health outcomes.Ultimately, the new service is expected to improve CVD outcomes, reduce health care costs and improve doctor and patient experiences. Importantly, the model is transferable to other national and international settings.

Statins in Reducing Events in the Elderly trial (2019 - 2023)$5,815,765
Description
This application seeks support for the Statins in Reducing Events in the Elderly (STAREE) randomised clinical trial based in Australian general practice.STAREE is a large scale primary prevention trial examining whether statin therapy prolongs independent living and reduces cardiovascular events in olderadults (≥70 years) without established vascular disease or diabetes. Despite older age being the strongest determinant of cardiovascular risk, few previousstatin trials have included older adults, and few have investigated whether any statin benefits are offset by adverse effects such as memory loss, myopathy anddevelopment of diabetes, all of which are more prevalent in the elderly. In the context of international debate and interest in STAREE, the trial has nowexpanded its primary outcome to include the internationally accepted composite of disability free survival (death, dementia or disability) and to moreextensively examine the safety of statins for this age group. The trial is well established across 6 Australian states (metropolitan and regional areas) withrecruitment to date of over 2100 general practitioner co-investigators and 3500 participants. Participant recruitment will be completed in 2019 with follow upcontinuing until 2023. This proposal seeks funding to complete the participant follow up and the collection of clinical data essential to confirm and adjudicatethe study endpoints.

A Pilot Dementia Clinical Quality Registry to Improve Dementia Clinical Care (2017 - 2019)$1,571,500
Description
Clinical Quality Registries are powerful and cost-effective tools that promote and monitor the implementation of clinical guidelines into practice. Clinical Quality Registries can improve adherence to guidelines, clinical performance and patient outcomes, as well as facilitate epidemiological research and updates to guidelines. Internationally, many registries for dementia are in operation, some of which have a primary focus on clinical care. The case for an Australian dementia registry, with a clinical care component, is well established, particularly with the imperative to implement the new Clinical Practice Guidelines and Principles of Care for People with Dementia in Australia. However, certain challenges exist for the design and expansion of a registry, including the identification of an epidemiologically sound minimum data set, entry points, and development of a sustainable cost structure for a national roll-out. This proposal seeks funds to pilot a Clinical Quality Dementia Registry to establish procedures, including testing a minimum data set and recruitment strategy, by leveraging a well characterised cohort of older adults with dementia from the ASPirin in Reducing Events in the Elderly (ASPREE) study. ASPREE is a primary prevention study evaluating the role of low dose aspirin in healthy adults aged 70 and over at enrolment. ASPREE has recruited >16,500 Australians, and engaged with >2,000 General Practitioners, across both regional and metropolitan settings in South Eastern Australia. Approximately 800 cases of dementia in this cohort will have occurred by the end of 2017. The pilot will also capitalise on the Monash University School of Public Health and Preventive Medicine's unique national expertise and leadership in registry development. Outcomes from this pilot registry will inform and assist in the efficient development of an eventual Australian National Dementia Registry.

Description
The 2013 Global Burden of Disease study ranks major depression as a principal contributor todisease burden. In almost every other area of health, preventative strategies have made a majordifference to disease incidence, whether it be vaccination for infectious disease, smoking controland lung cancer, clean water and diarrhoeal diseases, or risk factor modification and cardiovasculardisease. In stark contrast to the somatic medical field, psychiatry is almost entirely lacking inevidence based prevention strategies. Preventive interventions in psychiatry mainly consist of litecognitive behaviour type therapy given to individuals with early or emerging symptoms1. Notably,there are no evidence based preventative pharmacotherapies for psychiatric disorders.Current evidence suggests a possible aetiological role for inflammation in the genesis andpathophysiology of depression. Increased mean plasma levels of the inflammatory bloodbiomarkers, tumour necrosis factor alpha (TNF-a), interleukin-6 (IL-6) and C reactive protein(CRP) are found in patients with clinical depression. Inflammation increases with ageing, and wehave shown that elevated levels of these biomarkers are associated with the onset of de-novodepression in the elderly2. The possible link between inflammation and depression raises thequestion of whether the association is 'causal', and if so, whether pharmacological suppression ofinflammation is effective in preventing depression.ASPREE (ASPirin in Reducing Events in the Elderly) is a NIH and NHMRC supported 5 yearrandomised controlled trial (RCT) of aspirin (100mg daily) or placebo in 19,000 healthy, olderadults, and offers a unique opportunity3 to study the impact of low-dose aspirin on the incidence ofthis condition and to determine whether the relationship between inflammation and depression islikely to be causal. Critically, the existing infrastructure of ASPREE can be leveraged at a relativelyminor cost to determine the utility of aspirin for the prevention of depression in the elderly - ASPREE-D. ASPREE-D is both one of the first trials to explore preventative pharmacotherapy forpsychiatric disorders, and as probably the largest RCT ever in psychiatry, with a sample sizeamply powered to detect the predicted incidence. In addition to examining the efficacy of aspirin inreducing depression in the elderly, ASPREE-D will utilise biomarkers collected in ASPREE toconfirm or refute whether inflammatory biomarkers are risk biomarkers for depression genesis. The primary aim of ASPREE-D is to determine if use of low-dose aspirin reduces the risk of depression in healthy individuals over 65 years of age.

Opportunity to initiate primary prevention strategy and evaluate if such a strategy is successful in positively affecting cardiovascular risk. Aim to set up a RACPAC clinic at RHH to investigate new onset chest pain in a timely, consistent, efficient, cost effective and audited manner. Random trial of those referred to RACPAC but who do not have a primary cardiac diagnosis to evaluate the effectiveness of using absolute cardiovascular risk (ACR) scores to guide intensive risk factor modification. RACPACs have been shown to assess patients in a rapid and efficient manner while reducing cost and admission rates.

Bariatric surgery in Tasmania: investigating health service use, costs, patient outcomes and policy options (2014 - 2019)$839,027

About 1 in 20 Tasmanians are severely obese. Bariatric surgery can improve health for individuals with severe obesity but access to surgery is poor for most public patients. This study will investigate the pathways of patients referred for bariatric surgery in the Tasmanian public and private hospital systems to better understand the demand for surgery, the health outcomes, and costs associated with these different pathways. The findings will inform public policy and resource allocation.

Guidance of Heart Failure Management Programs by Risk Assessment (2013 - 2018)$1,422,739
Description
The aims of this project are to reduce Heart Failure (HF) admissions and costs in Tasmania by: i) using data linkage to identify locations where community service delivery should be targeted, and to evaluate current performance in HF readmission across the State; ii) developing a risk algorithm for HF readmission, including psychosocial determinants; iii) using i) and ii) devising a novel means of providing a disease management program (DMP), titrated to risk assessment, with physiologic assessment used to guide the management of the highest risk patients; iv) developing a clinical trial to define the merits of this approach; v) evaluating the cost-effectiveness of this intervention; vi) presentation of educational activities with community-based providers. A successful intervention is likely to be sustainable. As HF readmissions in Tasmania cost ~4550 bed days/y, the investment in this grant (~$1/2m/y) could be reproduced by a 50% reduction of readmissions.
