Associate Professor Ingrid van der Mei, a leading epidemiologist at the University of Tasmania's Menzies Institute for Medical Research, is trying to solve the puzzle of Multiple Sclerosis (MS), a debilitating neurological disease that often presents in young and middle-aged adults. MS is caused by the demyelination of nerve cells, and depending on the location of these, people develop a large range of symptoms including problems with fatigue, mobility and muscle function, pain, cognition, speech, bladder, and vision.

Ingrid's research focuses on understanding why people get MS and what influences the progression. She targets factors that have the potential to be modified (e.g. lifestyle, exposure to viruses) but also brings in genetics by examining how genes interact with those modifiable lifestyle factors. The ultimate aim is to prevent the disease, to slow the progression and improve the quality of life of each person with MS.

One of the team's biggest findings to date has been proving the link between MS and sun exposure. When Ingrid first started her MS research in 1998, it was known that there was a latitudinal gradient with the rate of MS being seven times higher in Tasmania than in Northern Queensland. That led to the idea that sunlight exposure might be involved. That hypothesis turned out to be true.

Ingrid led a case-control study where the questionnaire answers of people who already had MS were compared to a group of people without MS. The research showed that people with MS reported lower levels of sun exposure, particularly during adolescence, and this was backed up by objective measures of sun exposure. This major work was published in 2003 in the British Medical Journal.

The research continued with an incident case-control study, including people who were right at the start of their disease process. This research showed that both low sun exposure and low vitamin D levels were important risk factors of MS.

In another line of research, the team followed people with MS over time, carefully assessing their lifestyle and vitamin D levels. Intriguingly, low vitamin D levels were not only associated with the onset of MS but also influenced the number of MS relapses. This major discovery was published in 2010 in the Annals of Neurology. At the same time there was a paediatric MS study that backed up these findings, clearing the way for randomised control trials.

The work, conducted over 15 years, clearly shows how the 'Chain of Research', as Ingrid calls it, works. She started to build the evidence with a simple correlation between ambient UV and MS prevalence, then extended to better and more expensive study designs. Along the way the work dramatically changed vitamin D prescription practices. However, the international community is still waiting on the randomised controlled trials that are being conducted, because they provide the ultimate evidence on whether vitamin D supplementation really improves the course of the disease.

Ingrid is running the Australian MS Longitudinal Study. More than 3,000 people around Australia participate in a number of surveys each year. The data is owned by MS Research Australia and used for research, service delivery and advocacy, and is available for other Australian MS researchers. It is a dynamic project with many aims being achieved.

Ingrid is also taking advantage of the new era of technology to improve health.

She is working on setting up an electronic portal where people with MS can be empowered by having important aspects of their health and lifestyle visualised. They will also have the opportunity to share their 'personal health record' with clinicians in order to optimise their management. It basically results in a new health care model.

Another exiting project is the development of an online self-management program for people with MS – MS WorkSmart. It will assist people with MS to maintain their employment and reduce difficulties associated with their employment. Nearly half of the total MS cost is due to sickness absence and early retirement.

'I want to keep being challenged in my work, through new lines of research and the digital era,' she said. 'I like what I do. To see the slow change in knowledge and to be able to do something for the good of all those people with MS is really satisfying.'

Assoc Prof van der Mei is a senior epidemiologist, originally from the Netherlands where she studied Human Movement Science and Environmental Health Science.

She has expertise in conducting, managing and analysing large epidemiological studies including case-control studies and longitudinal studies with repeated measures.

Her research focuses on identifying factors that cause Multiple Sclerosis (MS) and factors that influence the progression of the disease. She contributed substantially to identifying that low sun exposure and low vitamin D levels as a risk factor for MS (British Medical Journal 2003, Neurology 2011) and that higher vitamin D levels are associated with a lower risk of having relapses among people with MS (Annals of Neurology 2010).

Research Themes

Ingrid's research aligns to the University's research theme of Better Health and the Menzies Institute for Medical Research theme of Public Health and Primary Health Care. It focuses on identifying risk factors of MS and identifying factors that influence the progression of the disease. In addition, she focuses her attention on improving the lives of people with MS, through the development of interventions.

Ingrid has a particular interest in gene-environment interaction, so most studies have data available on genetic factors as well as lifestyle/environmental factors.

Collaboration

Ingrid is currently involved in an international collaboration with Bournemouth University, United Kingdom, and an international collaboration with UCSF Medical Center (San Francisco) & University of California (Berkeley), USA in relation to the pooling of data. She is part of a large national collaborative team for the Ausimmune/AusLong study (Murdoch Childrens Research Institute, Australian National University, University of Melbourne, Queensland Institute of Medical Research, Barwon Health, John Hunter Hospital). National collaborations for the Australian MS Longitudinal Study include e.g. Monash), and she also has a national collaboration with Austin Health, Melbourne, on a project on Primary Biliary Cirrhosis.

Current projects

Ingrid currently manages the Australian MS Longitudinal Study, a research platform owned by MS Research Australia that aims to improve the lives of people with MS.
She also runs the Primary Progressive MS study, an epidemiological study for people with Primary Progressive MS (PPMS). There are currently no treatments available for people with PPMS and this study aims to examine whether known MS risk factors also hold for people with PPMS and which factors might be associated with the progression of the disease.
She is a chief investigator of the Ausimmune Study and the AusLong study. These projects investigate which factors are associated with a first episode of CNS demyelination (many of whom will develop MS) and factors associated with the conversion to MS and progression of the disease.
Projects that are older but still being used for data analysis and writing papers are the Tasmanian MS Longitudinal Study and the Tasmanian MS case-control study.

Publications

Ingrid has published >90 publications. Her research has made in internationally outstanding contribution in relation to the association between sun exposure/vitamin D and MS. She published a BMJ paper in 2003, being one of the first to show that higher levels of personal sun exposure were associated with a lower risk of MS. She also was instrumental in identifying that higher vitamin D levels were associated with a lower risk of relapses. This association is now being tested in international randomised controlled trials. She has identified a number of other novel risk factors of MS, including low infant sibling exposure (a measure of infection load) and low parity number. She has made a substantial contribution on gene-environment interactions, identifying a number of interactions, which all contribute to unravelling the puzzle of MS.

Evaluating the factors influencing National Disability Insurance Scheme participation and its impact among Australians with MS (2023 - 2025)$244,123

The role of pain in MS (2023 - 2025)$225,000

Description: Pain is prevalent in Multiple Sclerosis (MS), yet there is uncertainty of the nature of MS-related pain, how pain fluctuates over time in the short and long term, and how it relates to other symptoms of MS. While pain has a substantial impact on people with MS (PwMS), only few studies have separated the different types of pain that PwMS experience. Pain is commonly treated in MS, but low levels of relief have been reported. This program of work will focus on understanding pain in MS by examining specific types of pain, in terms of prevalence, severity, predictability and its chronic nature. Pain management experiences will be examined by identifying which pharmacological and non-pharmacological treatments are most frequently used, and which have the highest perceived effectiveness. This work will focus on how pain relates to other symptoms of MS and how it fluctuates in the short-term (during the day) and long term (multiple years). The project is designed to improve the understanding on individual types of pain of PwMS, to develop advice on pain management for PwMS and health practitioners, and to design treatment intervention studies for specific types of pain.

Improving outcomes for people with MS through digital technologies (2022 - 2026)$650,000

Description: The health-related quality of life in people with MS is substantially lower than the Australian population. My vision is to harness digital health technologies with the aim of improving both clinical care and self care. The Menzies Multiple Sclerosis Flagship Program (2020 - 2024)$10,000,000

Description: The Menzies Institute for Medical Research Multiple Sclerosis Flagship Program (MS Flagship) leverages the unique critical mass of MS research in Tasmania connected with Australian MS researchers, collaborators, MS societies and people with MS to address the gaps in the assessment, diagnosis and treatment of MS. Our work builds on our well-developed collaborations and laboratory, epidemiological, health economics and clinical research platforms. Our aim is to improve the clinical management of MS by slowing, halting and even reversing the rate of MS disease progression and to improve the quality of life and reduce the impact that MS has on individuals, their families and society. InforMS - an electronic patient-driven health care model with digital biomarker monitoring that improves the clinical care of people with MS (2020 - 2025)$1,167,815

Description: Multiple Sclerosis (MS) is a high cost and high burden neurological disease.Problem: there are many disease-modifying therapies available, so clinicians can theoretically switch treatments rapidly based on disease activityand side-effects. Yet this proves difficult when data on these is infrequently collected (currently 1-2 times a year) and is insensitive tofluctuations between visits.Solution: modern computing technologies (mobile apps, wearables) are changing the way health is monitored throughout medicine. We areenabled to collect more data, more accurately, and more frequently, and to share it more easily. If we could effectively harness such newmonitoring technologies for MS, clinicians could be supported with objective, prospectively collected, frequently measured, change-sensitivedata.Proposed research: we will create an electronic patient-centred health care system, InforMS, that becomes a one-stop-shop managementsystem for people with MS. It visualises patient-reported data, allows daily tracking and uploading of data from smartphones and wearablesensors, and allows sharing with clinicians via print, email, third-party access and via MSBase: the most widely used clinician registryworldwide. For clinical care, it would (1) enable timelier changes to treatments, and (2) provide greater confidence that treatments work. Forpatient self care, it would (3) empower people with MS to take a central role in managing their disease, and (4) activate them to collect noveltime-sensitive data. For research, it would (5) deliver powerful annual trend data to enhance MS research. This is a paradigm shift in MS careand research.We already have a prototype of InforMS, developed together with end-users. In this proposal, we will add several key functionalities, roll out thefull InforMS system within a registry of ~3,000 people with MS, and follow participants over time to test and optimise the system in practice.

Understanding the role of diet in Multiple Sclerosis (2019 - 2022)$91,000

Description: Many people with Multiple Sclerosis (MS) modify their dietary intake or use MS-specific diets, but there is a lowevidence base that different aspects of dietary intake has an effect on disease activity, progression and MS-specific symptoms. I willreduce this knowledge gap by examining the role of diet in MS using the AusLong study - an NHMRC funded and internationally uniquecohort of people who were originally diagnosed with a first clinical diagnosis of demyelination and have been followed annually for 10years. I will examine whether dietary patterns and indices of diet quality and inflammation are associated with relapses, disabilityprogression, Magnetic Resonance Imaging outcomes, fatigue, anxiety, and depression. This knowledge is essential in order to providedietary advice to MS patients to enhance quality of life and to design diets that could be tested in a randomised controlled trial setting.

A large societal cost saving can be made by assisting people with MS to maintain their employment and reduce difficulties associated with their employment. Further, this will have financial benefits for people with MS, as well as improve their overall quality of life. Despite a clear need, there are currently no self-management interventions specifically for people with MS in the workforce. In this project we propose to complete the development and undertake a feasibility study of the MS WorkSmart Program. MS WorkSmart is an online cognitive behaviour therapy self-management program with telephone support accessible on different devices. MS WorkSmart guides a person with MS through six modules over 13-15 weeks, using quizzes, videos and homework activities to assist the learning of strategies that can be applied to their working and daily lives for optimal MS management. The program is being developed together with people with MS and other stakeholders using focus groups and iterative feedback. It is underpinned by cognitive behavioural theory, is embedded in the literature and specifically designed for people with MS who are in the workforce. People will be assisted with telephone-delivered psychological support.

Identifying epigenetic factors involved in MS onset : utilising population-based studies with genetic and environmental measures (2018 - 2020)$851,451

Description: Multiple Sclerosis (MS) is an inflammatory and degenerative disease of the central nervous system (CNS)whose onset is associated with both environmental and genetic factors. These factors likely act, at least in part, through epigenetic alteration, a modifiable mechanism of gene regulation. DNA methylation is a well characterized epigenetic mechanism that can be evaluated at the genome-wide level with the aid of high throughput scanning technologies. Studies of MS patients at disease onset that include epigenetic factors in conjunction with genetic and environmental factors will provide new insights into disease etiology and thus guide better preventive and treatment strategies. This proposal is to undertake a whole epigenomewide study in a multi-site case control study (with replication in other international studies using similarmethods).

Description: Based on the published report 'Economic Impact of Multiple Sclerosis in 2010 - Australian MS Longitudinal Study (AMSLS)' Roche is interested in cost and utility data collected from patients with primary progressive multiple sclerosis (PPMS) only.Despite the type of MS not specifically being captured in the 2007 Cost Diary, 39 people with PPMS could be identified by matching participants with a more recent data collection.Roche is seeking Australian, PPMS-specific cost and utility data to inform a PBAC submission lodged in March 2017. Existing and upcoming treatments for MS are expensive, with current prices of up to $25,000 per patient per year and rising. Using the best available data, this study will identify the treatments that are greatest value for money for the most common form of MS (relapsing-remitting MS), with a goal to reduce wastage of limited healthcare resources.

Case-control study in Primary Progressive MS: Are the risk factors the same as for people with RRMS onset and can we identify new ones? (2015 - 2016)$142,000

Description: Examining risk factors of disease by comparing cases with a disease to healthy controls has been critical in understanding the aetiology of diseases. In MS, for example, important progress has been made through the epidemiological findings that cases were more likely to have had infectious mononucleosis, have high anti-EBNA IgG titres, and have low sun exposure levels. These findings resulted in a large body of laboratory work focusing on the mechanisms behind these observations. We have previously conducted epidemiological case-control studies on MS, but the number of people with PPMS was always too low to conduct a sub-analysis to examine whether the risk factors that we identified were also important in those with PPMS. We now propose to conduct a case-control of PPMS in a very cost-effective way, by using the data already collected on a large sample of healthy controls (n=558) from the Ausimmune study. We aim to collect the same data from a sample of cases with PPMS in Australia and compare that to the controls of the Ausimmune study. Moreover, the longer-term vision is that this data will form the basis for a prospective longitudinal study, where the cases with PPMS will be followed over time, in order to examine factors that are associated with the progression of MS. This will then be important for identifying possible treatments in MS for people with PPMS. Hypotheses*Established risk factors in MS, including low sun exposure, infectious mononucleosis, smoking, high anti-EBV-EBNA IgG and HLA-DR15 genotype are associated with an increased risk of PPMS onset.*Low parity, lack of younger sibling exposure and exposure to livestock are associated with an increased risk of PPMS.*People with PPMS have a higher EBV DNA load in whole blood and altered EBV serology compared to people with a RRMS onset.

Australian MS Longitudinal study (2014 - 2024)$1,372,869

Description: The main objectives of this project are: a) to continue the established multi-disciplinary research database for Australia with MS that will provide answers to important questions about the disease that are not able to be addressed by projects funded in the normal, relatively short cycle of research grants; b) to encourage nationwide (and international) multi-disciplinary collaboration on MS research projects, utilising valid and reliable measures that are internationally accepted and compatible wherever possible, and to lead by example in the unification of Australia's efforts to solve the various problems associated with MS; c) to reduce the cost of multi-disciplinary research on MS by provision of a large, well-characterised cohort of volunteer subjects for future studies.

The role of environmental and genetic factors in progression of multiple sclerosis (2013 - 2016)$945,084

Description: Much is known regarding the factors that increase the risk of developing multiple sclerosis. However there is little data regarding the importance of these and other factors on the rate of progression of MS. Once the diagnosis of MS is made one of the critical issues for patients and treating physicians is; what will happen now? As there are currently no established modifiable risk factors for MS progression, this project aims to delineate potentially modifiable risk factors for MS progression.

From risk factor analysis to translation: Multiple Sclerosis and vitamin D deficiency (2010 - 2014)$570,552

Description: This research focuses on Multiple Sclerosis (MS) and population vitamin D deficiency. The MS research aims to increase the understanding of the causes of MS, identify modifiable factors of MS progression and translate findings by conducting a vitamin D supplementation trial. High quality epidemiological studies on vitamin D deficiency in healthy populations aim to develop new public health recommendations on sun exposure and vitamin D that balance risk of skin cancer and vitamin D deficiency. Both research themes have the potential to have a substantial impact on health. The proposed body of work is internationally competitive, translates results into practice and expands research capacity in MS and vitamin D research.

Ingrid has successfully supervised a large number of PhD, Masters and Honours students.

She has numerous opportunities available for students with a background in epidemiology, nutrition, neurology and genetics.

The Australian MS Longitudinal Study aims to improve the quality of life of people. A number of surveys are done on ~3000 people with MS and new hypotheses can be generated within this study that can be tested
The Primary Progressive MS study is a case-control study with a large amount of environmental and genetic data
A number of studies are available where people with MS are tracked over time (AusLong study, PPMS study (longitudinal component), AMSLS study)
Most of the studies have Food Frequency Questionnaire data. Students with a background in nutrition can examine whether particular foods can influence the onset and/or progression of MS
Genetic data is available on most studies, so there are opportunities to examine gene-environment interactions in MS
