Kazuaki (Kaz) Negishi is a Cardiologist, Professor of Medicine at the University of Sydney, and adjunct Professor at Menzies Institute for Medical Research, University of Tasmania. As a physician-scientist, he works in clinical research on Cardiovascular (CV) Medicine, via application of CV imaging modalities, such as echocardiography, myocardial strain imaging etc. His current interests are, the effect of air pollution on CV system, identification and risk stratification of subclinical CV dysfunction (cardiotoxicity of cancer chemo therapy, hypertension and diabetes), hemodynamics and heart failure. He also has been working on the effect of microgravity on the Cardiovascular system, collaborating with NASA.

Biography

Kaz has been working on cardiovascular research in Japan, USA, and Australia. Upon the completion of his clinical training in Internal medicine, he pursued cardiology career, including echocardiography and interventional cardiology fellowship and PhD in Japan.  Then, he moved to Cleveland Clinic, OH, USA to pursue his research interests and worked as a Post-doctoral Research Fellow for three years. He had worked at the Menzies Institute of Medical Research, University of Tasmania, Australia between 2013 and 2019. Early 2019, he moved to the University of Sydney.

Administrative expertise

Kaz started an echocardiography laboratory in Gunma University Hospital, where he served as the director to manage 2500-3000 transthoracic and 200-300 transesophageal echocardiograms per year and to train junior cardiology fellows and colleagues for five years. Kaz is leading the Echocardiography laboratory at Royal Hobart Hospital.

Research Themes

Kaz's research aligns to the University's research theme of Better Health. His research interests include understanding cardiovascular hemodynamics, identification of subclinical diseases (Stage B), elucidating the effect of air-pollution on CV system, further risk stratification, and the management of risk factors. Kaz's research into myocardial function has spread to working with the NASA in the USA to identify the behaviour of cardiovascular function in Space. Kaz is also researching the heart in haemodialysis patients, who mostly die from cardiovascular diseases. He keeps very good relationships with Japanese cardiologists and is supervising research projects and PhD students in echocardiography lab in Gunma University, Japan.

Publications

Kaz has >80 peer-reviewed publications predominantly in international journals, including JACC, JACC CV Img, Circulation CV Img, Heart, AJC, AHJ and JASE. Two of his papers on cardio-toxicity (Negishi JASE2013) (Negishi Eur H J CV Img 2014) have been cited in the US and EU expert consensus statement as crucial evidence bases for applying new echo technique (myocardial strain) and cut-off value for patients' management algorithms. His work on vendor difference in myocardial strain assessment helped identify this problem (Negishi UMB2013), and this led to the initiation of a Standardisation Task Force to solve this issue (Yang JASE2015). He is an Editorial Board member of several medical journals including, JACC: Cardiovascular Imaging and Journal of American Society of Echocardiography.

Effectiveness of imaging pathways for CVD epidemics of the elderly (2018 - 2020)$703,875
Description
Cardiovascular imaging, especially echocardiography, is a cornerstone of modern cardiology; guidelines stipulate the use of echocardiography to inform therapeutic intervention in heart failure and valvular heart disease. However, there is also a risk of overuse, and in contrast to North America, where the growth of imaging has been controlled over the last decade, use in Australia continues to grow at ~10% per year. The dependence on imaging is greater in the cardiovascular disease (CVD) epidemics of the elderly (atrial fibrillation [AF], aortic stenosis [AS] and heart failure [HF]) than in other diagnoses. Not only are large numbers of tests being done currently, but the growth of these conditions will magnify the problem over the coming years. Failure to manage the hospitalisation, device and procedural costs for treatment of these new CV epidemics can be linked to shortcomings in the practice of CV imaging. The need for ongoing surveillance that is heavily dependent on cardiac imaging will introduce new complexities around imaging quality. Despite the expenditure of ~$1 billion/year on CV imaging in Australia, there is little evidence of value. The Partnership will link researchers with providers and imaging industry to improve all steps of the imaging "value chain" - ranging from the selection of suitable patients to the quality of interpretation. These studies will improve appropriate selection for testing, quality control and integration of decision-making with imaging.

Clinical application of cardiovascular imaging for detection of subclinical cardiovascular dysfunction and establishing treatment (2018 - 2021)$376,000
Description
This project aims to identify the early signs of heart diseases and to better understand precise mechanisms using new imaging techniques. Early detection will enable suitable management of these diseases by providing treatment to protect the heart. The avoidance or delay of CV disease will achieve better health outcomes in Australia. For this purpose, four clinical studies on CV medicine will be performed: Study 1 is an international multicentre project to examine if a new imaging technique (myocardial strain) for finding early disease provides a better guide for protecting the heart from cardiotoxic chemotherapy agents. In Study 2, a series of cross-sectional studies will be performed using data from an ongoing trial with the aim of determining the relationship between central blood pressure (BP) and heart damage. Study 3 is a 10-year observational study of the foetus (mothers) and infants who were exposed to smoke from the Hazelwood coal mine fire in Victoria in 2014. Early changes in the blood vessels from smoke exposure will be determined. Study 4 will explore if individual measure of air pollution would be a better marker of heart and vessel damages compared to traditional city-level measure. This program will make a significant contribution to Australia's medical research capacity and knowledge, with potential to develop evidence based CVD health policies, and with results being translated into practice.

Effect of ambient air quality on cardiovascular disease and telomere length (2017)$25,000

Although air-pollution is widely known to cause lung damage, it's effect on heart health is greater than its effects on lung diseases of cancers. In this proposal, we will investigate the effect of individual level exposure to outdoor air pollution on heart disease as well as a marker of ageing.

Successful advances in oncology over past decades have led to 5-year cancer survival rate improving from 50% in 1975-77 to 68% in 1999-2005. This success has led to a large pool of cancer survivors who are at substantial risk of cardiovascular morbiity and mortality. The specific complication of chemotherapy-induced cardiotoxicity (CTX), including radiotherapy cardiotoxicity, may occur in up to 20% patients within 5 years, and has a 50% mortality rate by 2 years. Cardioprotective (CP) medications have been demonstrated to reduce the risk of CTX, but uniform use would impose a financial burden on the health care system and expose many patients to unnecessary side-effects. One means of targeting CP to patients most likely to benefit is the use of novel echocardiographic techniques to detect preclinical cardiac damage. 2D-strain echocardiography measures changes in regional myocardial deformation, and identifies an earlier stage of cardiac dysfunction than the current standard of using left-ventricular-ejection-fraction (LVEF)-guided therapy. By identifying patients at an earlier stage,it is likely possible to target patients during a window when they are more likely to be responsive to CP, and avoids uneccessarily exposing patients who would never develop CTX to side-effects and costs of CP.

Tasmanian Study of Echocardiographic Detection of Left Ventricular Dysfunction (2014)$53,679
Description
HF is increasing in Australia but treatment in the early stages is able to prevent or delay its progression. The problem is how to recognise the early phase. This project will: implement a screening tool for heart failure; deliver an effective heart failure screening outreach service; and implement an effective communication process to Primary Care and referral to Cardiology. A database will be established to support follow up and personalised care of at risk subjects. This study expects to prevent 24 people developing heart failure.
