Description
This project driven by clinicians concerns regarding delays experienced by lung cancer patients. Tasmanian lung cancer patients experience some of the worst outcomes nationally. Timely diagnosis and treatment can improve survival and quality of life. To address this, we will investigate patient and service delivery factors that influence outcomes with the aim of co-designing and piloting strategies to improve the outcomes for Tasmanians diagnosed with lung cancer

Hospital Acquired Complications: prevalence and economic implications (2022)$61,214
Description
The first principle of health care is to do no harm. Yet approximately 1 in 10 patients continue to experience harmful safety lapses during their care (Slawomirski & Klazinga 2022). In the hospital setting, these adverse events -- also called hospital-acquired complications (HACs) -- include nosocomial infections, inpatient falls, venous thromboembolism and pressure injuries. Not every HAC is preventable, but their incidence can be reduced through clinical risk management strategies, protocols and interventions. These can be highly cost-effective, especially if implemented within a complementary policy and funding framework (Slawomirski & Klazinga 2022). The Australian Commission on Safety and Quality in Health Care has listed 16 priority HACs that are common and amenable to reduction. The use of the condition onset flag (COF) in Australian admitted patient data makes it possible to determine if a diagnosis was pre-existing or of it was acquired during the episode of care. While some HACs result in premature death, most increase patient morbidity prolonging length of stay and requiring additional medical and surgical interventions. The cost of this additional care in Australia and other developed countries is estimated at 10% to 15% of hospital expenditure (Ehsani et al 2006; Slawomirski et al 2017). In 2018, the Independent Hospital Pricing Authority (IHPA) introduced a national pricing adjustment scheme where the incremental cost of the ACSQHCs sixteen priority HACs is not funded. However, the additional health needs of harmed patients rarely end at hospital discharge. Many HACs will necessitate more follow-up treatment in outpatient, primary and community care, emergency presentations and even hospital readmissions. Relatively little empirical work has been conducted on these downstream effects and their resulting costs of hospital harm. Also, while frailty, co-morbidities, polypharmacy and length of stay are known to increase likelihood of harm, there may be other patient-related or contextual factors that can be used to manage risk. In addition, the association between patient harm and the emerging field of low-value care has not been studied extensively.

Assessing the clinical value of exercise stress echocardiography: an EXERcise stress Test collaboratION (EXERTION) study (2022)$24,951
Description
The primary objective of this study is to determine the clinical value of ESE in the prediction of CAD compared to an exercise stress ECG test. Specifically, we aim to determine the association of ESE test result with 1) probability of subsequent coronary angiogram procedure, and 2) the risk of CAD (as confirmed by coronary angiography) in comparison to that of an age- and sex-matched group of participants who completed a simple exercise stress ECG test.
