Perfecting the diagnosis and treatment of heart damage

The heart is an incredible and complex muscle. It is the source of life for every one of us. The heart delivers oxygen and nutrient rich blood to every cell in our body. If it stops working, we die.

But how many of us know what condition our heart is in? And if we knew that our heart was showing signs of damage, what could we do about it?

Professor Marwick is shining light on these two fundamental questions.

'My research seeks to do two things. First, determine the best ways to identify the earliest signs of heart damage; and second, determine the best ways to treat that damage so that it does not progress to heart failure' said Professor Marwick.

'As a Cardiologist, I see the results of undiagnosed heart failure. Once people present with the condition it is relentless. It can result in years of disability with a seriously compromised quality of life.'

'It is the single most expensive medical diagnosis in our hospitals because it requires repeated admissions for treatment. One in three people over the age of 60 will develop heart failure - and our population is ageing.'

It is Professor Marwick's experience treating patients with heart failure that drives his determination to improve preventative approaches.

'With early detection it can be delayed or prevented. Heart failure today often represents a failure in diagnosis and treatment yesterday.'

Professor Marwick's research has uncovered a number of ways that heart damage can be detected.

'We have discovered that rigidity in the heart is an early indicator of damage. Through cardiac imaging, we can take pictures of the heart as it beats and monitor how the muscle deforms and bounces back - like a stress ball. If the heart is not deforming properly, it shows that the muscle is becoming damaged.'

'Cardiac imaging can identify the beginnings of heart damage – in people over the age of 65, with heart failure risk factors, these are present in around 50% of people. We think there are around 22,500 people in Tasmania now that are likely to have the beginnings of a problem.'

'We are working with the community now on a simple screening test to identify the precursors to heart damage. Seeing how far a person can walk in six minutes might be effective in identifying early warning signs. It would tell us who requires closer inspection through cardiac imaging, and it is something that anyone could do.'

'If cardiac imaging shows there is damage to the heart, people leave with a report that tells them exactly what they need to do to delay or stop further damage.'

'Currently, heart damage is treated with a variety of drugs that cover all potential causes of damage.'

The research being conducted by Professor Marwick is also concerned with understanding how the heart works so that more specific treatments can be developed.

'The current blanket approach with treatment can make people feel unwell. It is a hard sell to ask people who otherwise feel healthy to take a preventative approach,' said Professor Marwick.

One breakthrough has been the discovery of a drug called ivabradine that specifically addresses the heart's ability to fill with blood.'It was thought in the past that the heart filled passively, and that the effort was in the pumping alone. We now know that the process of filling is an active one. The heart's muscle fibres are structured in a spiral. When the heart pumps, it wrings blood out like water from a towel. The action of untwisting is then very important in allowing the heart to refill. We think that ivabradine may aid this process.'

Biography

Tom Marwick completed training in medicine and cardiology in Australia, before undertaking an Imaging Fellowship at Cleveland Clinic, a PhD at the University of Louvain, Belgium and a Masters in Public Health at Harvard. He is the former Director of the Menzies Research Institute in Hobart and is currently the Director of Baker Heart and Diabetes Institute in Melbourne. He has divided his career mostly between Australia (former Professor of Medicine and Head of Cardiovascular Imaging Research Centre, UQ) and the USA (former Head of Cardiovascular Imaging at Cleveland Clinic). His main contribution has been in clinical research and research training, and has supervised about 30 research higher degree students – mainly clinical – including 22 completed PhDs. He was one of the initiators of stress echocardiography, and has made contributions to the prognostic evidence underlying echocardiography. His main current research interests relate to the detection of early cardiovascular disease and cost-effective application of cardiac imaging techniques for treatment selection and monitoring. He has published about 600 papers, reviews, chapters and editorials, and is an Associate Editor at JACC and Deputy Editor at JACC-Cardiovascular Imaging. Tom has been the recipient of more than fifty significant research grants and several awards, including the Simon Dack Award from the American College of Cardiology (2009), the RT Hall Prize (2006) and Kempson Maddox Lecture (2011) of the Cardiac Society of Australia and New Zealand.

Research Themes

Tom's research aligns with the University's research theme of Better Health. His research is centred on cardiology and clinical research. These foci were cemented when Tom, as a newly qualified doctor, gained early success in getting a paper on adrenalin in ventricular fibrillation accepted by The Lancet. Early in his career Tom had a particular interest in multi-modality cardiac imaging.

When Tom returned to the Cleveland Clinic in 1993, his interests turned towards predicting outcomes in patients with coronary and valvular heart disease. At this time he turned his multi-modality interests towards the prediction of myocardial viability – specifically identifying patients with heart failure who would benefit from revascularisation due to the presence of live muscle.

In 1998 when the chair of medicine came up at the University of Queensland, he took the opportunity to transfer his young family back to Australia. His interaction with a dynamic Diabetes group led to an interest in the cardiomyopathy accompanying type 2 diabetes. 'Heart failure is the forgotten complication of diabetes, but when you take the trouble to look, around 50% of type 2 patients have disturbances of cardiac function,' says Marwick.

Tom has also developed a high level of expertise in quantitative analysis after completing Masters in Public Health at the Harvard School of Public Health.

SMART-Risk: A risk-guided strategy to reduce readmission for Acute Decompensated Heart Failure using mHealth (2024 - 2025)$19,270
Description
The aim of this study is to evaluate the impact of Smart-HF (a mHealth platform) on reducing readmission to hospital and other secondary outcomes. The Smart-HF platform is an app for the patient that delivers educational information, goal setting, homework tasks, self-monitoring, and questionnaires to patients with heart failure and allows patient progress and outcomes to be measured via a clinician portal. Participants are patients admitted to the general or cardiology wards with Acute Decompensated Heart Failure (ADHF). Enrolled patients classified with a high risk of hospital readmission and/or death - i.e., ≥median of predicted risk, 33% based on the TAS-Help study algorithm developed by CIs Marwick and Huynh - will be randomised to either usual care or Smart-HF plus usual care, with stratification by study site and type of HF (reduced or preserved ejection fraction). The intensified home surveillance will show a reduction of hospital readmission and/or death (i.e., >50% risk) from use of Smart-HF. The primary endpoint is hospital readmission at 90 days post hospital discharge. Secondary endpoints - 30-day readmission, 30- and 90-day composite outcome.

Reducing Cognitive impairment by management of Heart Failure as a Modifiable Risk Factor: Cog-HF (2024 - 2026)$340,483
Description
Heart failure and dementia are common and serious problems in the community. Dementia and heart failure frequently co-exist due to shared disease processes and pathological mechanisms. Cognitive impairment an important sign of dementia is highly prevalent (>50%) in heart failure. The presence of one may lead to and/or worsen the other. This bidirectional relationship between dementia and heart failure acts as a vicious cycle that increases the burden of both diseases. We have successfully developed a disease management program for heart failure that are shown to reduce repeated hospital admission and mortality. These benefits appeared to be greater among patients with cognitive impairment who have higher risks of adverse outcomes. In this study, we aim to determine if these benefits among patients with cognitive impairment are due to an improvement in cognitive function.RCT to standard care versus an intensive disease management program that focuses on control of the bodys fluid volume, expedited treatment, self-care education and a novel exercise program delivered remotely via a smartphone or tablet application

Assessment of MEchaNical Dyssynchrony vs standard selection on clinical response in candidates for Cardiac Resynchronization Therapy (AMEND-CRT) (2023 - 2027)$106,536
Description
CRT (getting a pacemaker) is an effective treatment for many patients with heart failure and an abnormal echocardiogram (ECG), some people (a third or more) do not respond to this treatment (so the risk and discomfort of having a pacemaker inserted is without benefit), and a few people may actually get worse. The heart muscle normally contracts in a co-ordinated fashion so that all regions shorten at the same time. In some types of heart failure, this does not occur and different parts contract at different times, which is inefficient and can be corrected by a pacemaker. Some cardiac imaging methods can provide information about these mechanical disturbances (dyssynchrony) in the heart that determine whether patients will and wont benefit from a pacemaker. In this research project we are trying to establish whether information from simple tests of the heart could be used to better select patients for CRT. If the tests do not identify the mechanical process that CRT corrects, we would then know that pacing would be ineffective, and a pacemaker implantation could be avoided. The aim of the study is to see whether people without abnormal mechanical function derive any benefit from CRT. We will assess this by follow-up ultrasound measurements, questionnaires and walking tests at 12 months. AMEND-CRT is a RCT with all participants receiving a CRT device (special pacemaker), as planned by their doctor. The insertion of the device will be done as it normally is during standard care. Of those people where the imaging test shows that benefit is unlikely, half will have the device turned on, and half will have the device left off for the duration of the study.

Description
Coronary artery disease (CAD) is a potentially serious illness. At the moment, there is no established strategy to screen cancer survivors for CAD. Traditional risk factors only allow us to identify some people at risk, and therefore we are missing some of those who need early treatment to prevent a heart attack/stroke and potentially unnecessarily treating others. The goal of the REDEEM-CAD study is to identify the feasibility and value of systematic risk evaluation and coronary calcium scoring as a component of survivorship care. Some new cardiac imaging methods have allowed the early detection of coronary artery disease before the patient develops symptoms or has a heart attack. In this project, we will undertake a CT cardiac scan to identify if there is coronary calcification. Prior to the cardiac CT a baseline health review will be undertaken. If there is calcification that meets protocol inclusion criteria, participants will be recommended appropriate therapy to stop progression of coronary disease usually involving statins, blood pressure control and lifestyle guidance regarding diet and exercise. This treatment will be under the guidance of the participants GP. Yearly visits at 12 and 24 months, final visit at 36months.

Risk-guided strategy for reducing readmission for Acute Decompensated Heart Failure (RiskHELP) (2019)$107,400
Description
A prospective RCT with 320 HF randomised patients admitted with Acute Decompensated Heart Failure (ADHF) confirmed by diagnosis from RHH. Eligible patients (based on study inclusion exclusion criteria), identified by applying the TAS-Help (Study based at Menzies) algorithm as being at high risk for hospital readmission and/or death, will be randomised into two groups of 160 patients each arm. The control arm will receive usual care while the intervention arm will receive DMP-Plus. Patients with ADHF will be screened for a period of 24 months or until targeted sample size is met.

Effectiveness of imaging pathways for CVD epidemics of the elderly (2018 - 2020)$703,875
Description
Cardiovascular imaging, especially echocardiography, is a cornerstone of modern cardiology; guidelines stipulate the use of echocardiography to inform therapeutic intervention in heart failure and valvular heart disease. However, there is also a risk of overuse, and in contrast to North America, where the growth of imaging has been controlled over the last decade, use in Australia continues to grow at ~10% per year. The dependence on imaging is greater in the cardiovascular disease (CVD) epidemics of the elderly (atrial fibrillation [AF], aortic stenosis [AS] and heart failure [HF]) than in other diagnoses. Not only are large numbers of tests being done currently, but the growth of these conditions will magnify the problem over the coming years. Failure to manage the hospitalisation, device and procedural costs for treatment of these new CV epidemics can be linked to shortcomings in the practice of CV imaging. The need for ongoing surveillance that is heavily dependent on cardiac imaging will introduce new complexities around imaging quality. Despite the expenditure of ~$1 billion/year on CV imaging in Australia, there is little evidence of value. The Partnership will link researchers with providers and imaging industry to improve all steps of the imaging "value chain" - ranging from the selection of suitable patients to the quality of interpretation. These studies will improve appropriate selection for testing, quality control and integration of decision-making with imaging.

Cardiometabolic risk trajectories from childhood to midlife: finding pathways to better health (2017 - 2020)$1,531,986

Using data from a large cohort of Australian children followed for over 30 years, this study will investigate how weight status and physical fitness from childhood to adulthood determine early markers of cardiovascular disease and type 2 diabetes (cardiometabolic disease) in midlife. Detection and management of subclinical pulmonary vascular disease (2015 - 2017)$110,000
Description
Pulmonary hypertension is a debilitation condition, with a 3 year survival rate of only 50%. Tasmania currently has a higher prevalance and incidence of pulmonary hypertension related to connective tissue disease than other regions in Australian. There is a large disease registry that for over 10 years has been aggressively screening and treating patients at only mild onset of this condition. This involves treating patients with vasodilator therapies. This is believed to improve exercise capacity, mobility, vascular spasm, right heart function and overall quality of life. It is not yet clear if the strategy of early treatment improves hard clinical outcomes and whether this is cost effective. This is important as the cost of maintaining these patients on treatments over a long term period need to be justified. We aim to look at a wide range of aspects of this disease process. Initial, at the microvascular level to determine how this affects functional decline in patients, to the function of the right ventricle and how new research methods are able to detect changes earlier. We will also asses whether timing of intervention impacts upon health status, and what the cost efficiency of early intervention regarding disease outcomes and quality of life.

Use of Advanced Cardiac Imaging in Radiation-Related Cardiotoxicity. (2015 - 2017)$90,000
Description
Successful advances in oncology over past decades have led to 5-year cancer survival rate improving from 50% in 1975-77 to 68% in 1999-2005. This success has led to a large pool of cancer survivors who are at substantial risk of cardiovascular morbiity and mortality. The specific complication of chemotherapy-induced cardiotoxicity (CTX), including radiotherapy cardiotoxicity, may occur in up to 20% patients within 5 years, and has a 50% mortality rate by 2 years. Cardioprotective (CP) medications have been demonstrated to reduce the risk of CTX, but uniform use would impose a financial burden on the health care system and expose many patients to unnecessary side-effects. One means of targeting CP to patients most likely to benefit is the use of novel echocardiographic techniques to detect preclinical cardiac damage. 2D-strain echocardiography measures changes in regional myocardial deformation, and identifies an earlier stage of cardiac dysfunction than the current standard of using left-ventricular-ejection-fraction (LVEF)-guided therapy. By identifying patients at an earlier stage,it is likely possible to target patients during a window when they are more likely to be responsive to CP, and avoids uneccessarily exposing patients who would never develop CTX to side-effects and costs of CP.

Cardiometabolic benefits of exercise training in type 2 diabetes independent of weight loss (2014)$50,735

This project will recruit an overweight type 2 diabetes cohort, medically assessed for an exercise program. It aims to recruit some with family history of type 2 diabetes and some without. The cohort with exercise test before and after 6 weeks of the exercise program, with type 2 medications discontinued before test. The aim is to show that heart health will not be as good with family history of type 2 and that exercise will improve heart health but will be of more benefit to those with a family history than those without.

Guidance of Heart Failure Management Programs by Risk Assessment (2013 - 2018)$1,422,739

The aims of this project are to reduce Heart Failure (HF) admissions and costs in Tasmania by: i) using data linkage to identify locations where community service delivery should be targeted, and to evaluate current performance in HF readmission across the State; ii) developing a risk algorithm for HF readmission, including psychosocial determinants; iii) using i) and ii) devising a novel means of providing a disease management program (DMP), titrated to risk assessment, with physiologic assessment used to guide the management of the highest risk patients; iv) developing a clinical trial to define the merits of this approach; v) evaluating the cost-effectiveness of this intervention; vi) presentation of educational activities with community-based providers. A successful intervention is likely to be sustainable. As HF readmissions in Tasmania cost ~4550 bed days/y, the investment in this grant (~$1/2m/y) could be reproduced by a 50% reduction of readmissions.
