Motor Neurone Disease: How one woman’s research is empowering patients to make the most confronting decisions of their lives.

As if grappling with a terminal diagnosis isn’t enough, Motor Neurone Disease (MND) patients, and their families, are forced to confront the terrifying reality of what will soon happen to their bodies. And, the knowledge of how they are going to die. It’s hard to think of a crueller disease. Safe and high-quality care the best way we can support patients and families through this time.

MND causes motor neurones to deteriorate to the point they can no longer carry signals from the brain to the muscles. As the disease progresses, a patient’s body function will become increasingly difficult. Eventually, the body will be unable to perform the activities necessary to sustain life.

Dr Anne Hogden’s overarching research aim is to understand how health-care managers can best promote safe and high-quality care. For the past ten years, she’s made it her life’s work to ensure that wherever possible, MND patients are supported to be cared for on their terms. And that quality of life is promoted at every opportunity.

Assisting MND patients to make the hardest and most confronting decisions of their lives has been her major research focus.

Dr Hogden’s earlier research into developing a model of decision making in MND multi-disciplinary care, has evolved into developing decision-making tools for people with MND.

“MND patients are faced with really difficult decisions,” says Dr Hogden. “They are forced to weigh up, ‘do I have an artificial feeding tube which could prolong my life, or do I decide not to, die of dehydration?”’

“Not everyone wants to make those decisions and some prefer to wait and see. It’s confronting. It’s scary. We see decision regret as many equipment or procedural related decisions need to be made within  a certain time window.”

Dr Hogden has launched her website, https://mnddecisiontools.com/, and has developed three of the seven tools she intends to provide for decision support to patients and their families. Each decision-making tool lists a range of information designed to help a MND patient navigate the difficult times ahead.

For example, the first tool, Should I have gastrostomy for my nutrition and hydration in MND? explains what a gastrostomy is, how the procedure is done, how it works and when the patient should have it. Other questions follow such as will it extend their life? what will happen if they choose not to do it, and can they change their mind after it’s done?

The second tool, Should I have predictive genetic testing for MND? is, according to Dr Hogden, one of the most controversial decisions. “A family can be split down the middle over this decision,” she says. “Opinions are divided. Some people feel they need to know for their children’s sake. Even though only about five percent of the MND population have had other family members with MND, a genetic test has ramifications that can affect the lives of many. It’s a very personal decision.”

The third tool is called, Should I have assisted ventilation to help breathing difficulty in MND? “One of the reasons we put this tool together, is that people’s attitudes are changing regarding assisted ventilation,” says Dr Hogden.

“In previous years, virtually no one wanted a tracheostomy. The procedure will extend life but patients will lose their voice. In recent times, some younger people with MND have chosen assisted ventilation by tracheostomy so they can have more time with their children, or continue working.”

The remaining four tools Dr Hogden is working on include choices for communication equipment, location of end of life care, voluntary assisted dying, and should I complete an advanced care plan? The latter is a document the patient creates with doctors and family members that states their wishes for the rest of their life.

“A lot of MND patients lose their speech,” says Dr Hogden. “Our biggest hurdle is that a lot of people don’t want to look too far ahead. It can be way too confronting to make an advanced care plan. Hopefully, our decision-making tools help them to get over that hurdle. We help them by explaining all the advantages and disadvantages of the things they will need to consider.”

“It’s such a cruel disease and patients have the most heartbreaking decisions to make. My research is designed to make care the best it can be for MND patients, and to promote their quality of life as much as possible.”

“MND creates one of the most challenging and confronting situations there is. But, this is very important to me and a great thing to be a part of. It’s unbelievably tragic, but a wonderful thing to see patients who have made clear choices about their own lives and been able to live them out. To see them gain some control over the situation means a lot.”

Doctor Anne Hogden is a Senior Lecturer with the Australian Institute of Health Service Management, College of Business and Economics. Her teaching work is in convening and delivering healthcare safety and quality and research units for the Master of Health Service Research, and supervision of Master and PhD students. As part of an industry collaboration, Anne is an embedded academic with industry partner Northern Sydney Local Health District. This collaboration works to promote industry-based research, organisational development and professional development in healthcare management.

Biography

Anne is an Honorary Senior Research Fellow with the Australian Institute of Health Innovation, Macquarie University. Her research career followed on from her clinical practice as a Speech Pathologist in adult neurorehabilitation.  Her PhD research devised a model of patient-centred decision-making to guide care in motor neurone disease (MND). Undertaken at the University of NSW and the Australian National University, this model development paved the way for her ongoing research into MND service delivery. Subsequently, Anne has developed web-based decision tools to support people living with MND to make difficult decisions for care and quality of life.

Research Themes

Anne’s research interests are in healthcare communication, patient-centred care and decision-making and interprofessional team processes, with an emphasis on people with life-limiting conditions. She has led two Category 1 grants to investigate patient-centred decision-making, and cognitive and behavioural change in MND multidisciplinary care. Anne has built a strong track record of academic outputs, grant activity, higher degree student supervision and academic service. She has given numerous international and national presentations, and there has been uptake of her research internationally, resulting in several international collaborations for MND decision-making projects in the UK, Europe and USA.

Collaboration

Anne is currently involved in the following international and Australian collaborations.

  1. The UK Marie Curie Foundation grant (MCRGS-20171219-8005) was awarded to the DiAMoND collaboration (University of Southampton). Anne works with a team of researchers from the UK to develop a decision support tool for people with MND in the UK, who are considering gastrostomy.
  2. Anne is supervising PhD students who are investigating issues of health service management, including patient-centred care, health professional engagement in safety culture, and leadership for safety and quality.
