Overcrowded hospitals and packed emergency rooms are making headlines around the country. It’s a big problem that, despite our best efforts, is continuing to grow. With an ageing population, if we don’t do something about it, the strain on our hospitals is going to get worse. Preventing hospital admissions has never been so important.

Society is changing, and so is the way medication is used. People are living longer, so the number of medications that people are taking regularly has increased. In the older age groups, chronic diseases are more prevalent. These patients are at a higher risk of experiencing medication-related problems, which include adverse drug reactions (ADRs) or side effects that occur more frequently when patients take multiple medications. While many ADRs are minor in nature, they can also be very serious and lead to hospitalisation. Unplanned admissions in the elderly are becoming a huge burden on our health system. But, encouragingly, most of these admissions are preventable.

How do we identify people at risk of ADRs, so we can intervene before an episode occurs?

Professor Luke Bereznicki is a pharmacist whose research is focused on preventing ADR-related hospital admissions in the elderly. In fact, he and his team are testing a potential solution.

‘Our research shows that in people aged 65 and over, around 15-20 per cent of hospital admissions are associated with ADRs (side effects) of their medications,’ says Professor Bereznicki. ‘About 80-90 per cent of these admissions were preventable.’

Combinations of cardiovascular medications are most commonly implicated. The challenge is that these medications are often prescribed appropriately to manage a range of conditions, but sometimes the medications have additive effects that lead to problems.

‘For example, a patient could be on multiple medications, become dizzy, and then fall and hurt themselves. When we look at those admissions, there are elements there we could prevent through better medication management. In some instances, medications might not be necessary, or we could have monitored the patient more closely.

‘Our data shows us that the number of ADR admissions reported within the public health system are just the tip of the iceberg,’ says Professor Bereznicki. ‘The coding system in hospitals means that, often, ADR-related admissions are not recorded as such. The type of drug-related problems that are not being identified during the hospital admission are typically the ones that relate to combinations of medication. And, they’re much more common. In our recent study, only 1 in 5 ADR-related admissions that we identified were recorded by the hospital as ADRs.’

Professor Bereznicki and his team looked at a year of admissions to the Royal Hobart Hospital, focusing purely on overnight stays in medication wards. This research has helped them identify risk factors and use this information to develop a prediction score. This scoring system is called the Preventing Adverse Drug Reactions in Elderly Community Dwelling Patients (PADR-EC) Score.

‘Though we started in hospital with the admission, we focused on what happened to the patient before the admission – the factors that put them in hospital in the first place,’ explains Professor Bereznicki. ‘This hasn’t been done before, which is what makes our risk score novel.

‘Ultimately, we’re trying to create a risk profile for the patient and suggesting interventional strategies during the admission and following discharge. We will be developing a medication management plan for patients, which will be sent directly to their GP and pharmacist. This plan will focus on reducing the risk of serious ADRs.’

Elderly patients are often seeing a GP who can prescribe, a pharmacist who can prescribe over-the-counter medicines, and a specialist who prescribes different drugs again. It’s easy for patients to become confused about what they should be taking.

‘Someone has to put it all together and make sure we are reducing the risk of medication-related problems, while maximising the benefits of the medications required. To me, that’s our professional calling. Dispensing medication is an opportunity to review the effectiveness and safety of a patient’s medications.

‘Targeting high risk patients and improving their outcomes through a range of interventions, that’s what I would call “pure” clinical pharmacy. Improving medication safety is at the core of pharmacy practice and is the focus of our pharmacy degree at UTAS.’

Professor Luke Bereznicki is a pharmacist with 12 years experience working in the health care sector, and has been an academic at the University of Tasmania since 2007. His career is dedicated to improving pharmacy practice in Australia through learning and teaching, research and advocacy.

Biography

Luke graduated from the University of Tasmania's Discipline of Pharmacy in 2002 with first class honours. In 2003, he was an inaugural recipient of a National Institute of Clinical Studies (now part of the NHMRC) PhD Scholarship. His research program was focused on the implementation of strategies to improve the quality use of anticoagulants in the hospital and community settings. He was appointed at the University of Tasmania's Pharmacy in 2007 and is currently Professor in Pharmacy Practice. He is recognised as a leader in Australian pharmacy practice research and education, and has received a number of recent local and national awards recognising his work. He is dedicated to improving pharmacy practice in Australia through education, research and advocacy and was the 2008 Australian Young Pharmacist of the Year. In addition to his teaching and research activities, Luke is actively engaged in professional pharmacy practice; he works regularly in community pharmacy, writes regularly for the Australian Pharmacist and performs medication reviews and consultant pharmacist activities.

Luke's research has featured the implementation and evaluation of pharmacist involvement in a variety of quality improvement activities in both hospital and community settings. He is now recognised as one of the leading Australian authorities in this area and frequently speaks at conferences and provides education and training for health professionals and consumers. His research has been recognised nationally and internationally, and has resulted in a number of awards, including the following:

Luke has published over 180 original and professional peer-reviewed papers and attracted over $13 million in competitive funding. Luke has strong links with the pharmacy profession and is a longstanding member of the Tasmanian Branch Committee of the Pharmaceutical Society of Australian (Tasmanian Branch) and former Board Director of the organisation.

Teaching

Teaching expertise

Luke teaches a diverse range of topics related to pharmacy practice including ethics, law, quality use of medicines, communication, non-prescription medicines and the identification and resolution of drug-related problems. He currently coordinates the honours program within the Division of Pharmacy, and teaches into pharmacy practice and research methodology units.

Luke has demonstrated a strong commitment to research training and leadership through Research Higher Degree (RHD) supervision, mentoring of academics and the development of research curricula and processes. He has supervised RHD students through to completion. He also supervises a number of honours students each year.

Teaching responsibility

Luke commenced teaching at Pharmacy in 2007, and is now Professor in Pharmacy Practice, Head (Pharmacy) and Deputy Head of the School of Medicine. He teaches a diverse range of topics related to pharmacy practice including ethics, law, quality use of medicines, communication, non-prescription medicines and the identification and resolution of drug-related problems. He is also the Honours Co-ordinator.

He also leads a team of researchers and students focussed on the prevention of adverse drug events leading to hospitalisation.

Luke has been a Chief Investigator on several major national projects that have investigated and implemented new professional activities for Australian pharmacists. He was recently a lead investigator on a number of national projects (> $1.5M) funded by the Department of Health and Ageing which aimed to evaluate the role of pharmacists in providing education, support and ongoing monitoring for people taking anticoagulants in the community. He developed and maintains anticoagulation.com.au, a site that provides free educational resources about anticoagulant medications to consumers and health professionals.

Luke has an outstanding national reputation for his contributions within multidisciplinary research and quality improvement teams. The collective impact of his research and leadership in his field is evidenced by invitations to present at national and international conferences.

A major focus of his work has been on strategies to improve the quality use of anticoagulant medications in the elderly via a range of innovative strategies. These projects have focussed on both the community dwelling and institutionalised elderly. He has published widely in local and international journals on this topic. His innovative research in anticoagulation management is internationally recognised. His status within the field of anticoagulant medication research was acknowledged by appointment to the Commonwealth Government reference group on the 2012 Review of Anticoagulant Therapies in Atrial Fibrillation, which preceded the PBS-listing of novel anticoagulant medications for the prevention of stroke in AF in late-2013.

Luke currently leads the Tasmanian Atrial Fibrillation Study, which is investigating the use anticoagulant medications to prevent stroke in Tasmanians with Atrial Fibrillation from 2011 onwards. While research involving antithrombotic medications has been his major focus, Luke has also been involved in a number of other projects focused on improving the quality use of medicines in community and hospital settings. Other current areas of interest include the management of respiratory diseases, prevention of adverse drug reactions, pain management and medication adherence in a range of chronic conditions.

Preventing Adverse Drug Reactions in the Elderly (2019 - 2022)$274,000
Description
Based on our prospective adverse drug reaction (ADR) registry:1*15% of medical admissions to Tasmanian public hospitals in elderly patients were ADR-related,*89% were preventable,*17% patients had a repeat ADR within 12 months, and*One third of patients with repeat ADRs experienced the same ADR caused by the same medications.From this clinical registry, we developed and validated an ADR risk prediction score (PADR-EC).2We hypothesise that ADR risk stratification during hospitalisation and communication of this risk, together with recommendations that target the specific risk factors identified, to GPs at discharge will reduce the risk of ADR-related hospitalisation.Design: A randomised controlled trial (RCT) at the Royal Hobart Hospital, the major public hospital in Tasmania.Population: Over 12-months, 1000 patients ≥65 years with unplanned admissions to medical wards will be recruited and randomly allocated to intervention or control groups.Intervention: A researcher will perform an ADR risk assessment using the PADR-EC Score,2 which is the sum of points assigned to 5 independent predictors of ADR-related hospitalisation. Based on the PADR-EC Score and the specific risk factors identified, recommendations to reduce the risk of ADRs will be determined.Recommendations will relate to:*post-discharge medication management services, *monitoring of specific parameters/medications, and *de-prescribing strategies.The PADR-EC Score and recommendations will be communicated to the patient and clinical team during hospitalisation, and the patient's GP, alongside the usual information, in the electronic discharge summary. Control: Usual hospital care, with GPs receiving the standard discharge summary.Follow up: All participants will be followed by the research team for 12-months to identify ADRs (including their management and outcome) and health services received.Outcomes: ADR-related hospitalisation (primary); ADRs requiring hospital treatment, change in therapy or specific treatment (secondary). The study will be powered to detect a 5% difference in the primary outcome

ReMInDAR Trial Reducing Medicine Induced Deterioration and Adverse Reactions Trial (2018 - 2020)$2,847,927
Description
This randomised controlled trial will assess a pharmacist led intervention to reduce medicine induced deterioration and adverse reactions. Medicine induced deterioration encompasses the spectrum of side effects that are frequently not recognised as medication related but are misattributed as geriatric syndromes, frailty or changes due to aging. The aim of the service is to enable early identification of signs and symptoms of medicine- induced deterioration so that worsening frailty and subsequent adverse events, such as injurious falls, fractures and delirium are prevented. In implementing the service, the pharmacist will use a suite of validated tools to assist in the detection of signs and symptoms of medicine-induced deterioration. The validated tools will encompass assessments to monitor changes in cognition, change in 24-hour movement behavior, including sleep, as well as grip strength. In addition, pharmacists will assess the potential for adverse medicine events via review of the resident care assessment record and through patient or carer reported changes in health since the medicine regimen was changed. The pharmacist service will be compared with usual care. The trial, involving 500 patients will take place in aged-care facilities in South Australia and Tasmania. Evaluation will encompass health program, clinical and economic evaluation. The expected clinical outcome is a reduction in medication-induced deterioration, as measured by change in frailty score. Adverse events will be measured as a secondary outcome. The cost- effectiveness evaluation will assess cost per adverse event avoided.

'What's your beat?': Reducing strokes through AF awareness and screening (2018)$69,182
Description
Stroke continues to be a major public health issue. It is the third most common cause of death. The financial cost in Australia is estimated to be $5 billion each year. Tasmania and South Australia are the worst affected States. Atrial fibrillation (AF), the most common sustained cardiac arrhythmia, is a major modifiable risk factor for stroke. AF is associated with a five times increased risk of stroke and is responsible for up to 25% of strokes in elderly adults. Recently, international guidelines and expert consensus statements have recommended more widespread screening for AF in those aged 65 years or older. The case is compelling - AF is common and a leading cause of stroke. Unfortunately, AF often has no symptoms and is commonly undiagnosed or untreated by the time stroke occurs. AF-related strokes are associated with significant morbidity, mortality, and healthcare costs, yet they are highly preventable with the use of warfarin or newer anticoagulant drugs. Given the availability of effective therapy, along with accurate and inexpensive screening technology, population-based AF screening has the potential to become an important public health program. In older age groups, identification and management of AF is the most significant way to prevent stroke.Internationally, there have been recent recommendations for more widespread screening for AF in those aged 65 years or older, as a cost-effective strategy for stroke prevention. This project will raise public awareness of AF and improve its detection, and subsequently lessen the burden of stroke in Tasmania. We intend to screen approximately 3,000 Tasmanians aged 65 years or older, and without previously diagnosed AF. Screening will take place throughout most of 2018 at a range of community venues across Tasmania.

Getting Asthma Under Control (2017 - 2020)$2,074,100
Description
We will conduct a cluster randomised trial, with asthma control being the main inclusion criterion and clinical outcome measured. The three study arms are: Group A (Pharmacy Asthma Service): eligible patients will receive four private consultations with the pharmacist, at baseline, one month, six months (via telephone) and 12 months. The consultations will focus on addressing three key factors associated with uncontrolled asthma: suboptimal adherence characterised by underuse of preventer medication and/or overuse of reliever medication; suboptimal inhaler technique; and/or uncontrolled allergic rhinitis. At each visit, asthma control and a number of secondary outcomes will be assessed. Group B (Low intervention and referral): Patients identified with uncontrolled asthma by the pharmacist will be referred to their general practitioner. Clinical outcomes will be assessed at one month and 12 months. Group C (Usual Care): The third group will be recruited from a comparable sample of patients with asthma, matched by age, gender and asthma control scores from general practice medical records. Asthma control and secondary outcomes for this group will be assessed by the project group at baseline and 12 months.

Games for Health: A technology platform for health and wellbeing (G4HEALTH) (2015 - 2016)$250,000

G4HEALTH will develop a digital game platform that will enable University of Tasmania researchers, in collaboration with industry partners, to provide a benefit for the community, and place the University at the forefront of the billion dollar games for health industry. Platform and game development will involve a cross-disciplinary community of researchers with expertise from engineering, science, education, and health through to arts. It will consist of off-the-shelf sensors and devices capable of measuring physical activity, providing feedback, delivering narrative-driven games to encourage activity, and transmitting the resulting data. Technologies identified and games and story developed will be used in two proposed intervention studies to increase engagement in physical activity by older adults, people living with chronic disease and adults in a workplace setting in the North of Tasmania. Educating Tasmanians about anticoagulant medications (2013)$10,435
Description
Atrial fibrillation (AF) is a common heart arrhythmia, which affects approximately 2% of all Australians, and almost 10% of people over the age of 75 years. AF is associated with a 5-fold increase in the risk of stroke. Anticoagulant therapies like warfarin, dabigatran, rivaroxaban and apixaban are highly effective in reducing the risk of stroke associated with AF, but also place patients at risk of bleeding adverse events. It is therefore critical that anticoagulant therapies are used appropriately in people with AF, to ensure that the right patients receive them and that the risk of bleeding is kept as low as possible. This project will enable Tasmanians taking anticoagulant therapy access to high quality, independent educational resources focussed on improving the use of this commonly used but high-risk group of medications. The project is focussed on developing educational resources for people taking the commonly used anticoagulant drug warfarin, and three new anticoagulant medications (dabigatran, rivaroxaban and apixaban). These medicines are used to prevent stroke in people with atrial fibrillation, as well as prevent and treat deep vein thrombosis and pulmonary embolism. Given the demographics of Tasmania's community, the use of these medicines and need for relevant educational support of consumers is highest in this State. The project will develop educational tools for consumers and health professionals, and promote these resources to general practices and community pharmacies.

Management of post-operative pain and the progression to persistent post-operative pain (2013)$8,634
Description
This project is a prospective, cross sectional study designed to ascertain patient or management factors that increase the risk of developing PPP, with the objective of designing a predictive model to identify high risk PPP candidates. PPP will be defined as pain at the surgical site 3 months after surgery, with severe pain being defined as 5 or more on a 10-point scale. The project involves recruitment of patients, surveys, medical chart data collection, inclusion criteria, exclusion criteria, sample size, data analysis, and assessment of variables.

Fucoidan and marine polyphloroglucinols (2012 - 2014)$85,000
Description
Brown algal extracts, in particular those containing fucoidan and marine polyphloroglucinols, have been shown to have potential applications in a number of health indications such as pre-diabetes, immunomodulation, cardiovascular disease and inflammation. With advances in technology, fucoidan and marine polyphloroglucinol extracts from brown seaweed can now be produced successfully on a commercial scale. These types of extracts are beginning to become more prevalent as ingredients in western dietary supplement markets. However, for the extracts to successfully enter this market, efficacy, safety and provenance must be proven. With this in mind, the objectives of this PhD project include the following: to establish a generic analytical method that can be used to fingerprint and quantify fucoidan concentrations in solution, serum and urine; to determine the ability of a commercially available fucoidan/polyphloroglucinol extract (Maritech Synergy) to inhibit the uptake of glucose via an acute human study; to assess the effects of Maritech Synergy on health indications including glucose control, insulin sensitivity and pre-diabetes in a human setting; and to further the understanding of fucoidan and its metabolism.
