Biography
Professor Robinson is Co-Founder of the Wicking Dementia Research and Education Centre which was established in 2008. Andrew is an innovator in dementia care practice, aged care workforce development, health professional and on-line dementia education, and translational research in aged care. He was a founding director of Dementia Training Australia, has received over $25 million in research grants and has more than 100 peer reviewed publications. Andrew was Co-Director of the Wicking Centre, and Professor of Aged Care Nursing until September 2017 when he retired from these positions. He is now an Emeritus Professor at the Wicking Centre.

- Oral care is critical to keeping the mouth clean and the body and brain as healthy as possible. This is especially the case for adults with dementia who rely on others for care. With poor oral health, harmful bacteria throughout the mouth cause tooth decay, gum inflammation, gum disease, and tooth loss. Frequently adults with dementia take multiple medications and the side-effects of these medications include reducing saliva and drying the mouth which in turn increase the harmful oral bacteria. When these harmful bacteria are breathed (or aspirated) into the lungs, the lungs become infected and aspiration pneumonia results. Adults with dementia, particularly those who live in residential communities, are vulnerable to aspiration pneumonia as they are often frail, not well nourished, have complicating medical conditions such as diabetes, heart disease, lung disease, and swallowing problems, as well as needing help from others for their care. We know, from published studies, that vulnerable adults who have their own teeth need to brush their teeth for 2 minutes after each meal. Adults who wear dentures need to clean their dentures each day and take them out when they are planning to sleep. Whether they have their own teeth or wear dentures, all adults with dementia also need to rinse their mouth after each meal. Unfortunately, oral care for adults with dementia in residential communities remains inconsistent and fragmentary. Our studies have documented the barriers, real and perceived, that prevent systematic oral care and we have developed a protocol to overcome these barriers. This protocol has five components: (1) establishment of an innovative Community of Practice (CoP) including health care professionals in the community-at-large, policy-makers, and consumers, together with visionary residential care leaders and nurse-carer champion partners to support staff and drive practice change; (2) education of nurse-carer champions by CoP members; (3) ongoing education and training of all point-of-care staff by nurse-carer champions as they work with RACF-based dental/oral health therapy students, and are supported by CoP members and online resources; (4) implementation of daily evidence-based oral care by staff; and (5) regular audits of daily oral care by nurse-carer champions and the students to ensure its sustained implementation.To measure the effectiveness of our protocol, we will follow 60 residents in each of two residential care communities in Tasmania. We will document the number of cases of aspiration pneumonia these residents experience before and after a 3-month period of daily evidence-based oral care (component 4 of the protocol). We also will document the number of times these residents have been transferred to hospital or admitted to hospital before and after the 3-month period of daily oral care. To document improvements in the residents oral, nutritional and general health and their quality-of-life (QOL), we will use the following measures that are appropriate for adults with dementia: (i) Oral Health Assessment Tool (OHAT), (ii) Mini-Nutritional Assessment (MNA), (iii) 3-day food record, (iv) Yale Swallow Protocol that identifies any signs of swallowing difficulty, (v) Sit-to-Stand Manoeuvre that identifies any difficulties with strength and movement, and (vi) the EuroQol-5 dimension tool that documents residents perceptions of their ability to move, care for themselves, and participate in daily activities along with any pain, discomfort or anxiety they are experiencing. We will administer these measures to the residents before and after the 3-month period of evidence-based daily oral care. We will analyse the data and write reports about the findings. We will publish these reports in academic journals and magazines that are widely-read by the public. - Description
- Managing chronic respiratory conditions in residential aged care is challenging, particularly for residents with dementia. In pilot studies, we have shown that respiratory conditions are often underdiagnosed and undertreated. Carers are well placed to observe and respond to changes in respiratory health but their knowledge of respiratory care is limited and the recognition, treatment and management of respiratory conditions is not well understood. In collaboration with Aged Care staff and respiratory experts, we will develop an educational module: Optimising Respiratory Care in Dementia (ORCID), to address this deficit in knowledge. Once developed, we will examine the impact this has on the dementia and respiratory knowledge of a group of carers and nurses in a pilot trial. A better approach to respiratory care in dementia will ultimately improve the quality of life of residents and allow the development of a systematic approach to observation, review and referral for residents with respiratory needs.

- It has been estimated that between 30 and 50% of dementia cases may be preventable by addressing modifiable risk factors such as vascular factors and complex mental stimulation. Increasing levels of education at a younger age have been associated with decreased risk of developing dementia in older ages, leading to the suggestion that building cognitive reserve may provide greater resilience to dementia. The Tasmanian Healthy Brain Project (THBP) is a large scale prospective trial that examines whether the real-world intervention of university-level study, as a form of complex mental stimulation, in older adults may provide benefits to cognitive resilience, so as to reduce ageing-related decline in cognitive performance, and reduce the risk of dementia. The initial phase of the THBP (2010-2015) involved identifying genetic interactions that may modify risk of ageing-related cognitive decline and mediate the benefits of engagement in complex mental stimulation. In addition, the THBP has developed constructs to measure prior and current levels of cognitive reserve and performance. The proposed project will realise the long term outcomes of the THBP to determine how a university course-based intervention may affect cognitive capacity through ageing and if it may effectively protect individuals from significant decline and dementia. Furthermore, we will determine how common variations in genes linked to brain plasticity, such as brain-derived neurotrophic factor (BDNF) gene, may modify how an individual gains potential benefit from the intervention. In this regard, a genetic profile comprised from BDNF and apolipoprotein E polymorphisms, may also predict those at greatest risk of dementia. Finally, we will investigate whether BDNF protein levels in blood may serve as a biomarker for the effects of the intervention and/or as a potential indicator of relative risk or protection from significant cognitive decline.

- The Wicking Dementia Research and Education Centre Teaching Aged Care Facilities (TACF) Program is built on a formalised partnership between universities and aged care providers, auspiced at the level of Faculty Dean/Head of School and ACF Board Chairman/CEO. The partnership is formed with the express purpose of establishing a Teaching Aged Care Facility. In the initial phase of the program the conditions that facilitate establishing a TACF are created by: providing health care students with a positive placement experience so they develop a more positive attitude to working in ACFs; building organisational and leadership capability within ACFs to support students professional education and drive a high performance culture more capable of realizing evidence based practice; and creating a collaborative organisational environment conducive to establishing a TACF. Objectives of this project are to: (a) extend the UTAS Wicking TACF model in a tripartite collaboration with the Australian Catholic University (Melbourne) and Curtin University in Western Australia, and three large corporate aged care providers in regional and urban settings (Southern Cross Care Tasmania in Hobart, Catholic Homes in Melbourne and Uniting Church Homes (UCH) Rowethorpe in Perth); and a small stand-alone rural facility, Mount St Vincent Nursing Home and Therapy Centre, in Ulverstone, Tasmania; (b) establish three nodes including: a regional/rural Tasmanian node involving a partnership between UTAS and Southern Cross Care Tasmania and Mount St Vincent Nursing Home and Therapy Centre; a Victorian urban node involving a partnership between the Australian Catholic University (Melbourne) and Catholic Homes; and a Western Australian urban node involving a partnership between Curtin University and UCHRowethorpe; (c) develop node-based collaborative arrangements between the respective universities and ACFs to implement the Wicking TACF model with the intention of establishing a Teaching Aged Care Facility as a UniversityACF partnership; (d) implement the Wicking TACF prototype program in each node as a collaboration between the respective universities and aged care partners; (e) participate in evaluation and promulgation activities.

Research Supervision
Prof Robinson has a track record in building research capacity in Tasmania. He established the BN (Hons) program in the SNM in 1995, with over 70 graduates to 2007. He took a key leadership role in developing the SNM Graduate Research program between 2002 and 2007 building the cohort from 2 to around 20 students. Between 2009 and 2012 Robinson supervised seven PhD students to completion.
