There were only four students in the Master of Pharmaceutical Science course when Rahul Patel moved from India to study at UTAS in 2004. Five years later, when Dr Patel joined the University as an academic, this number had risen to eight. Today, the UTAS Master of Pharmaceutical Science attracts 45 to 50 students every year.

‘And one of the things I like most about this course is that it’s so popular among international students,’ says Dr Patel, Senior Lecturer in Pharmacological Sciences in the Division of Pharmacy, School of Medicine.

A prescription for passion

Dr Patel’s enthusiasm is shared by his students. In his first three years of lecturing, Dr Patel was awarded two teaching merit certificates by the university and he has since been twice voted by students as Pharmacy King. To use a medical metaphor, his passion is infectious.

Dr Patel knew from a young age that he wanted to study pharmacy. He remembers wondering as a child how an oral headache pill, for instance, could target and soothe the brain. His fascination with what medications do and how they work eventually led him to a Bachelor Degree in Pharmacy, employment as an industrial pharmacist, and postgraduate study at UTAS for both a Masters Degree and a PhD.

‘I decided at that time that all I wanted to do with my life was research because it is so different,’ says Dr Patel. ‘You embark on a project and you don’t know what the results will be.’

He completed his PhD within two and a half years and, while awaiting his results, worked as an intern pharmacist to secure Australian registration. Around this time an academic job was advertised for UTAS.

‘Research I knew and loved, but I didn’t have much experience with teaching. When I had my first lecture and I was standing in front of 60 third-year students, I know how nervous I was.

‘But one thing for sure, I knew I wanted to be myself.’

From that very first day, Dr Patel’s teaching style has focused on student participation. He starts his lectures with a chosen slide, gives the students some targeted information (which will have answers hidden within it), poses a question and then allows the students to find the answer. Sometimes they do and sometimes they don’t. Either way, the students are forced to engage, and the questions are formulated so that the essential knowledge is passed on through the process. This pattern of questions and answers is usually repeated five to six times each lecture.

Bench to bedside research

Today, Dr Patel combines his love of teaching and research in his role as course coordinator for the UTAS Master of Pharmaceutical Science. Half of this course comprises individual research, allowing Dr Patel’s students to explore ways that patients can be treated with medications outside of the hospital setting. There are numerous benefits in doing this, including the cost of inpatient care, the risk of infection, and the general happiness of patients who’d simply rather go home.

One of the reasons why patients remain in hospital for months after they could otherwise leave is the need to administer intravenous drugs to fully eradicate bacteria. To overcome this, the Master of Pharmaceutical Science students investigated whether the particular drug that is used for the treatment of bone infections was stable enough to be administered by a portable elastomeric device. This device, developed by a pharmaceutical company, is basically a small balloon that, once attached to an intravenous line, can be carried by a patient to pump medication over a 24-hour period. The Masters students’ research confirmed that the drug was in fact stable enough to be administered in this way and, within a month of being published, the technique was being used by patients.

Current students are now exploring the suitability of a multitude of other drugs to be administered in this way with the goal of developing a comprehensive and peer-reviewed resource for physicians worldwide.

‘When I decided to study for my Masters abroad, I chose Australia because many of my friends and cousins were pursuing their study here,’ Dr Patel said. ‘The reason I specifically chose UTAS was because, at that time, it was the only Australian university that offered a two-year Master of Pharmaceutical Science – all of the others only offered one-year or 18-month courses.

‘So it was crystal clear to me that I should study in Tasmania. Then it was straight away that I fell in love with this place – the greenery and the hills, the kindness of the people and the low pollution. And the first time I saw the River Derwent, it redefined my idea of a river. It seemed more like an ocean.

‘One thing I always tell my students is that I too was one of them but that if you work hard, this is the land of opportunity. That is what has happened to me.’

Dr Patel is a lecturer in Pharmacological Sciences, Division of Pharmacy, School of Medicine. Since his appointment as a lecturer in December 2009, Dr Patel has made a significant contribution in learning and teaching at both under- and post-graduate level. Apart from receiving two teaching merit certificates and exceptional feedback from the under-graduate and post-graduate students, Dr Patel has thoughtfully re-structured the Master of Pharmaceutical Science (MPharmSc) course to ensure a broader career and scholarly prospective for the graduates of this course. Dr Patel's research has resulted in a significant practice change in professional/clinical practice and also assisted a number of healthcare professionals in the area of pharmaceutical formulations. Dr Patel has actively participated in marketing of the MPharmSc course at National and International level resulting in almost a 100% increase in the student numbers. Dr Patel has also actively looked for quality graduate research students within the last one and half years of his appointment as a Graduate Research Coordinator as seen by the marked increase in the number of good quality Higher Degree Research applicants.

Biography

Dr Patel completed his Bachelor of Pharmacy degree from the Rajiv Gandhi University of Health Sciences, India. Before migrating to Australia, he worked as an Analytical Chemist at Elite Pharmaceuticals, India. Dr Patel obtained his Master of Pharmaceutical Science and PhD degrees from the University of Tasmania. After completion of his PhD in 2008, he worked as a registered pharmacist in Queensland, Australia. Dr Patel joined the Division of Pharmacy, School of Medicine as a lecturer in Pharmacological Sciences in December 2009.

Teaching expertise

Teaching philosophy: Dr Patel's teaching philosophy involves problem-based learning. Generally, each lecture starts with individual examples or problems, through the consideration of which students develop general principles and basic concepts. Dr Patel often incorporate humour in every lecture to foster student-lecturer relationship and to motivate students to learn. He believes in 'active learning' or 'class room participation'; hence, he tries to get everyone involved by posing various questions and also initiate feedback from students during lectures. It creates an environment in which all participants have the opportunity to learn and students explore issues and ideas in depth, from a variety of viewpoints.

Changes to Master of Pharmaceutical Science (MPharmSc) course structure: Dr Patel has been coordinating the MPharmSc course since July 2012. In his capacity as a course coordinator, Dr Patel has made significant changes to the course structure, including changing the course duration from one to two years. The changes were greatly accepted by the students, evident from students' feedback as well as significant increase in the number of MPharmSc students.

Scholarly Teaching: Dr Patel has been involved in a number of learning and teaching related scholarship activities. These include: 1) submission of an 'Expression of Interest' for teaching development grants (University of Tasmania; category (A); 2) completion of a project that investigated the barriers among pharmacy academics with regards to the use of Facebook as a teaching tool; and 3) completion of a project that evaluated the attitudes of post graduate students towards evidence based medicine.

Dr Patel coordinates both the Master of Pharmaceutical Science and Graduate Diploma in Pharmaceutical Science course. He teaches into several units within the Bachelor of Pharmacy, Master of Pharmaceutical Science and Diploma in Pharmaceutical Science course programs.

Research Themes

Dr Patel's research aligns to the University's research theme of Better Health. His research interests include:

  • Development of novel chromatographic techniques for the structure-activity characterisation of a group of glycosaminoglycans such heparins and fucoidans. Glycosaminoglycans are composed of heterogeneous mixture of highly negatively charged structurally complicated molecules. Using sophisticated analytical, in-vitro, ex-vivo and in-vivo techniques, Dr Patel and his colleagues identified novel glycosaminoglycan molecules having anti-asthmatic and anti-ulcerative colitis potential. Dr Patel and his group has developed a number of novel and robust analytical techniques that could be used to separate and identify structurally unknown molecules of glycosaminoglycans. For example, Dr Patel and his group developed a simple and sensitive method, for the analysis of batch-to-batch variation in pharmaceutical heparins. In December 2007, an increase in the number of adverse events including deaths were reported resulting from the use of contaminated batches of heparin. To ensure safety, the US Food and Drug Administration developed a reference test method, based on the method developed by Dr Patel and his group. All Pharmaceutical companies were instructed to use this reference method to detect impurities in all heparin batches to manufacture and enter the United States.
  • Investigating the physico-chemical stability of extemporaneously prepared pharmaceutical formulations. Dr Patel has investigated the stability of a wide range of extemporaneous preparations and his research has contributed to the field of science and health. For example, the stability of one type of carbohydrate (iron polymaltose) formulation commonly used for severe iron-deficiency anaemia was unknown beyond 24 hours and therefore such preparations were traditionally prepared by Australian hospital pharmacists on a daily basis, resulting in subsequent workload and costs. Dr Patel's recent research indicated that the stability of such preparations can be safely extended from 24 hours to 28 days, and hence it is possible to prepare them in advance rather than on a daily basis. This finding has significantly reduced the workload of Australian hospital pharmacists as this formulation, based on his research finding, is now prepared commercially by Baxter Healthcare with an expiry date of 28 days. Also, Dr Patel has recently indicated that various types of antibiotics, when stored appropriately, are stable in different types of dialysis solutions. Such antibiotics premixed solutions can be prepared in advance by nurses and given to the patients. Therefore, patients can be treated at home rather than in the hospital. These findings have changed the current clinical practice as evident from the fact that based on his published research; the Royal Hobart Hospital has updated their guidelines for the preparation and storage of antibiotics in dialysis solutions.
  • Minimising the particle and microbial associated harm among injecting drug users (IDU). Oral prescription opioids (morphine, oxycodone, buprenorphine) are not always administered the way they are intended. The way IDU prepare injecting solutions from prescription opioids make them susceptible to particle- and microbial- related harm (thrombosis, amputation, pulmonary embolism, skin infection, endocarditis etc.). Dr Patel and his colleagues are working on developing various techniques that can minimise the risk of particle and microbial related harm among IDU.

Collaboration

On a number of research projects, Dr Patel has collaborated with clinicians from the Royal Hobart Hospital, researchers from Australian Centre for Research on Separation Science (FSET), Centre of Research Excellence for Chronic Respiratory Disease, School of Human Life Sciences and division of Psychology, School of Medicine. Dr Patel has collaborated with a local Biotechnology company (Marinova Pty. Ltd) on a number of research projects. These collaborations have resulted in multiple joint publications.

Tasmania's long history of iodine deficiency has improved, but some groups and individuals remain at-risk of adverse consequences, including brain-damage. Research is impeded by the high cost of current iodine status assays (due to having to send them interstate). Preventing pyridine exposure after intraperitoneal administration of ceftazidime in patients with peritoneal dialysis-associated peritonitis. (2019)$9,458
Description
Ceftazidime is the first line treatment for Pseudomonas peritonitis. Thus, the production of pyridine, a toxic degradation product of ceftazidime, raises some concern. This study will examine the degradation of ceftazidime in peritoneal dialysis solutions to propose some recommendations in order to minimise the exposure of patients to pyridine.

In vitro stability of ceftolozane/tazobactam in different types of peritoneal dialysis solutions (2018 - 2020)$26,851
Description
To date, there is no data available for the intraperitoneal administration of ceftolozane/tazobactam (C/T), the preferred and recommended route of administration of antibiotics in patients with peritoneal dialysis associated peritonitis. For an antibiotic to be administered intra-peritoneally, it needs to be stable in peritoneal dialysis solutions. Intraperitoneal administration of antibiotics to treat systemic infections [other than PDAP] in patients where administration of systemic antibiotics is impractical, is a routine clinical practice though the evidences are limited. The results of the proposed study may provide the stability information for clinicians to make such a choice where systemic administration of C/T is impractical by administering it intraperitoneally in patients that need it.

Investigating the anti-inflammatory effect of a non-anticoagulant molecule of heparin following experimental traumatic brain injury (2017)$10,000
Description
Heparin has shown promising results in attenuating neuroinflammation following experimental traumatic brain injury (TBI); however, its anti-coagulative properties make it an unsuitable treatment due to the risk of bleeding. We have recently shown that the heparin derivative Dp4 retains the anti-inflammatory actions of heparin, but not the anti-coagulative properties. In this project, we will test whether Dp4 suppresses neuroinflammation following TBI in mice, potentially identifying it as a novel and valuable therapeutic agent for brain-injured patients.

Variability in plasma concentration of glucosamine in osteoarthritis patients taking various glucosamine formulations. (2017)$9,837

Novel molecules of heparin for the management of asthma (2014)$4,998
Description
Aim: In an international first, this project aims to achieve the evaluation and identification of heparin molecules that inhibit the effect of several important inflammatory mediators involved in the pathogenesis of asthma, without possessing significant anticoagulant effect, with a view towards examining the efficacy of identified molecules in patients with asthma. Methodology: Heparin molecules will be separated and isolated using an ion-chromatography technique. Participants will attend the clinic where 50mL of blood will be taken by a qualified nurse. Mononuclear cells will be isolated from the collected blood. Then the cells will be activated to produce inflammatory mediators in the presence or absence of heparin fractions. Outcome: This research will evaluate and identify the novel molecules responsible for the anti-asthmatic activity of the parent heparin. This study would provide better insight into the mechanism(s) of heparin for its anti-asthmatic activity. It could open new avenues for the development of a heparin formulation with high anti-asthmatic properties but devoid of any anticoagulant activity. This could potentially resolve the safety and non-compliance issues objected upon in previous clinical trials.

Brown algal extracts, in particular those containing fucoidan have been shown to have potential applications in a number of health indications such as prediabetes, immunomodulation, cardiovascular disease, inflammation and gut health. With advances in technology, fucoidan extracts from brown seaweed can now be produced successfully on a commercial scale. These types of extracts are beginning to become more prevalent as ingredients in western dietary supplement markets. However, for the extracts to successfully enter this market, efficacy, safety and provenance must be proven to help gain acceptance. With this in mind, the objectives of this project include the following: to establish the efficacy of high purity fucoidan extract from Fucusvesiculosus in the treatment of ulcerative colitis in an animal model; and to compare the efficacy of the routes of administration of Fucusvesiculosus fucoidan in treating ulcerative colitis - with particular reference to oral and intra-peritoneal dosages.

Description

With this in mind, the objectives of this PhD project include the following: to establish a generic analytical method that can be used to fingerprint and quantify fucoidan concentrations in solution, serum and urine; to determine the ability of a commercially available fucoidan/polyphloroglucinol extract (Maritech Synergy) to inhibit the uptake of glucose via an acute human study; to assess the effects of Maritech Synergy on health indications including glucose control, insulin sensitivity and pre-diabetes in a human setting; and to further the understanding of fucoidan and its metabolism.
