Tony Barnett

Dr Barnett has trained as a nurse at Whyalla (South Australia), has worked clinically in a number of hospitals in Victoria and South Australia and held academic and Head of School positions at both Deakin and Monash Universities. He is currently engaged in a number of rural health projects in Tasmania on topics such as: interprofessional practice, simulation, oral health, rural health workforce and clinical education. Dr Barnett has supervised to completion, over 25 graduate student research projects conducted in Australia, Malaysia and Hong Kong. He is a chief investigator with the (national) Centre of Research Excellence in Primary Oral Health care and a board member of the Australian Rural Health Education Network (ARHEN).

Administrative expertise
Dr Barnett is the Director, Centre for Rural Health (CRH). Established in 1997 the CRH is a multidisciplinary, research focused academic unit with a State-wide brief to improve rural health by supporting student clinical placements and engaging with the rural health workforce and health services. The CRH is one of 11 university-based rural health units funded by the Commonwealth Department of Health to deliver across 8 Key Performance Areas.
Prior to coming to the University of Tasmania in mid- 2010, Dr Barnett was at Monash University – the majority of this time was spent as Head of the School of Nursing and Midiwifery, a multi-campus academic unit with a student enrolment of more than 1,000 students . During this time, he led the strategic development, accreditation, implementation and evaluation of a range of innovative new courses at undergraduate and post-graduate level, both on and offshore and initiated a range of strategies to support and drive research activity.
From 1985-1991, Dr Barnett was the foundation Dean of the Faculty of Nursing and Health Sciences at Warrnambool Institute of Advanced Education (WIAE)/Deakin University. His brief was to introduce courses relevant to the Federal Government's decision to transfer nurse education to the tertiary sector and then transition of CAE programs to the University sector as part of the Dawkins reforms.

Teaching expertise
Dr Barnett has taught subjects on: research methods, evidence-based practice, collaborative health care, rural health, nursing, professional issues and health professional transitions. In the past five years, he has secured a major HWA (Health Workforce Australia) simulation education grant and, with colleagues, been awarded two OLT (Office for Learning and Teaching) grants in the areas of student transition and technology enhanced learning, and three TCEN (Tasmanian Clinical Education Network) grants.

Expertise
As a researcher, I try to adopt approaches, methods and techniques best suited to the research question or problem to be solved. Dr Barnett has worked with a number of research designs (eg. descriptive, RCT, pre-test:post-test, grounded theory); has used a variety of data collection methods: (eg. survey, focus groups, interviews, audit) and enjoys both descriptive and comparative approaches to data analysis techniques (both numeric and narrative).

Collaboration
Dr Barnett collaborate with Oral Health Services Tasmania, the University of Adelaide and the University of Western Australia on oral health research projects. He also work with health service colleagues and academics from other schools and disciplines across a range of health service and education projects.

- The Royal Australian and New Zealand College of Ophthalmologists (RANZCO) delivers a Vocational Training Program for future ophthalmologists. Training is primarily based on work-based learning. Both formative and summative assessments are used to assess trainees' performance. The RANZCO Advanced Clinical Examinations (RACE) is the final hurdle for trainees before trainees begin a self directed learning program in their final year. RACE has clinical and written sections. Trainees need to pass both sections to commence their final year of training. Pass rates for both clinical and written components are declining. Pass rates directly influence the number of new ophthalmologists available to join the workforce and has significant implications for ophthalmic service delivery, particularly in regions and settings that are currently underserviced such as rural and remote areas. This study aims to understand why such a high proportion of RANZCO trainees fail RACE and to identify strategies to better support and prepare trainees for their exams.

- Vietnam is facing overcrowding in central hospitals due to the disproportional distribution of health workforces in terms of both quantity and quality. In many rural and remote areas, the healthcare staff lack to access real-time, effective assistance, and realistic training provided by experts located centrally. It may lead to less effectiveness of health care services and additional costs for rural/ remote residents. UTAS Augmented Reality Remote Assistance system (UTAS-ARRA) is a solution developed by the Centre for Rural Health and the Human Interface Technology Laboratory, University of Tasmania. In this project, we will employ the UTAS-ARRA to improve the effectiveness of the remote situational tele-mentorship between central experts and rural/ remote less-experienced healthcare staff in Vietnam. The system aims to improve the response of rural/remote staff to manage patients under the assistance and guidance provided by central experts in real-time.

- Managing tooth decay of primary teeth in young children using silver diamine fluoride to reduce cost, fear and pain. The aim of the project is to determine the best, and least invasive, practice to manage tooth decay of baby teeth in young (0-8 years) children using silver diamine fluoride (SDF). SDF is an antimicrobial liquid that can be brushed on to cavities quickly and easily, to painlessly and noninvasively stop the spread of cavities and even prevent tooth decay. We will compare the: *tooth decay arrest and prevention abilities;*relative cost effectiveness;*clinical costs to the public health sector;*acceptability to the child and their parent/carer, and*the ease of treatment for the dental clinician of:(1) 5% Sodium fluoride varnish (NaF) - current practice is application every 3 months; or(2) 38% silver diamine fluoride (SDF) and potassium iodide (PI) and a sealant over the cavity - this practice is called Zero Invasion Dentistry (ZID) and is 1 treatment and will last for 12 months.

- The Department of Health (DoH) has contracted GP Assist to establish and trial a dedicated palliative care afterhours phone support service for palliative care clients and caregivers across North and north west Tasmania. The aim of this project is to provide a locally tailored service for Tasmanian palliative care clients with the aim of preventing unnecessary presentations to ED, avoidable hospitalisations and utilisation of ambulance services. Currently there exists no comprehensive or clear data or information regarding demand or the most appropriate service model to provide afterhours supports for palliative care clients and their carers. The proposed 10 month trial provides an opportunity to test and evaluate a service model, including gathering data about service need to inform future service planning. This formative evaluation will identify quality improvement suggestions within the context of the key performance indicators (Effectiveness, Access, Appropriateness, Quality, Efficiency) for the Palliative Care After hours Phone Support Service

- TCF funding was requested to support a number of initiatives and activities of the Tasmanian Bereavement Care Network (BCN) over a three year period (2018-2020) including:1. Further development of the BCN website, including a web-based directory of bereavement care services 2. Development of resources (pamphlets, posters) to promote the BCN to the Tasmanian community 3. Provision of bereavement care skills and professional development opportunities to address identified skills gaps in bereavement care, for example skills to support bereaved children and families; and supporting people experiencing complex grief. 4. Facilitate the uptake and implementation of bereavement care practice principles and values across member organisations; 5. Develop and implement an appropriate business model for the sustainability of the BCN into the future; 6. Undertake an evaluation of the network; 7. - Tasmania has the highest age-standardised incidence rate of bowel cancer in Australia with 74 cases seen per 100,000 persons. Tasmania is also on par with the Northern Territory as having the highest age-standardised mortality rate for bowel cancer at 19 deaths per 100,000 persons. These figures highlight bowel cancer as s significant public health issue in Tasmania.When bowel cancer is detected and treated early, the 5-year survival rate is as high as 93%. In Tasmania, less than half (44%) of the 65,379 eligible Tasmanians invited to take part in the 2014-2015 National Bowel Cancer Screening Program (NBCSP) chose to participate.Research Shows that general practitioner (GP) endorsement is a key predictor for bowel cancer screening uptake.The primary aim of this study is to determine whether a clinical prompt, based on NBCSP and Royal Australian College of General Practitioners guidelines, delivered to GPs at point-of-care, can increase bowel cancer screening participation in first-time receivers of the NBCSP kit.

- Headspace is a youth health program for people aged 12-25 years focusing on early intervention and prevention and mild to moderate mental health issues. The Link Youth Health Service Inc. (The Link) is the organisation responsible for the delivery of the program in Southern Tasmania and Cornerstone in the North and North-West. This project will review Headspace clinical services, focusing on client pathways, to develop a best practice framework for the delivery of a range of high quality wrap around services that include a focus on: mental health, drugs and alcohol, general health, vocational support and referral. Aims The review aims to:1.Identify those features of the current model/s of care provided to clients that work well and not-so-well and describe an aspirational model that could be delivered by the program.2.Engage with a sample of clients to describe their experiences of the program, from initial contact through to exit or referral3.Review current partnerships and identify opportunities for connections and linkages with other programs and services.4.Identify opportunities for and the capabilities required to provide new services and re-configure existing services to better meet the needs of clients 5.Propose indicators and measures that would better capture program outcomes from a client-centred perspective.

This study pertains to the implementation of SACS-R across the Child Health and Parenting Service (CHaPS) across Tasmania to monitor all children's social attention and communication development and prospectively identify infants and toddlers 'at risk' of ASD during their routine assessments at 12, 18 (piloted in the South) and 24-months of age. Children identified as at risk will be referred to a community developmental assessment team at St Giles for specialist multi-disciplinary assessment and subsequent diagnostic assessment at two years of age.

- Access to sound expertise and guidance to perform clinical procedures is often lacking, especially in more rural and isolated areas. In the absence of direct supervision, practitioners and students can feel underprepared and lack the skills and confidence to perform many procedures. This project aims to improve the situation by making remote expertise and guidance more accessible. The project will trial the application of cutting-edge technology, a wearable tele-assistance system that will allow novices to undertake procedures with real-time audio and visual guidance provided by a supervisor at a distant location. This technology enables the instructor's helping hands to be visually projected onto site (a patient's wound for example) and allow the novice to shadow the instructor's movements throughout the procedure (if necessary) or allow the instructor to step-in should an incorrect action be initiated. Key outcomes will be: 1) a working remote training and guidance system; and 2) a report on the feasibility of the technology, its benefit and impact.

- The aims of the project are, at the behest of the Tasmanian Aboriginal Centre (TAC) to: collaboratively develop, trial and evaluate Virtual orientation Tours of three Aboriginal Health Services in Tasmania. Methodology: An action research approach will be adopted to guide the collaborative development, trialing and evaluation of the Virtual Tours of three Aboriginal Health Services. The outcomes will be three virtual orientation tours hosted online via the Tasmanian Clinical Education Network (TCEN) and TAC websites with links to other areas of the TAC website that the TAC considers appropriate for increasing students' cultural awareness and competence. These virtual orientations will prepare students for professional experience placements in Aboriginal Health services in a way that is timely and cost-effective for students and placement providers, alleviates reality shock, increases students' understanding of the local culture, nature of the services and learning opportunities available and what will be expected of them. In addition to preparing students for placement in Aboriginal Health, these Virtual orientation programs will be accessible to all health professional students via the TCEN website and potentially other students and prospective employees via the TAC website and thus help to address the identified need to expose more health professionals and students to Aboriginal Health issues and Aboriginal Health Services (Australian Government Mason Review, 2012; Closing the Gap; 2008-14; TCEN 2014-15).
