The University of Tasmania Faculty of Health is now the biggest provider of dementia education in the world, reaching thousands of students in over 100 countries.

Professor of Pathology, James Vickers, said that Tasmania is recognised internationally as a leader in dementia research. Professor Vickers' research has focused on ways to maintain or improve plasticity of the brain.

Solving the public health issue of the 21st century

Dementia is arguably the public health issue of the 21st Century. Ageing is the biggest risk factor in developing dementia and our population is ageing.

Dementia occurs when cells in the outer part of the brain die. This part of the brain is where memories are stored, language held and judgments made. The disease is irreversible and leads to death. No drugs currently exist to stop the progression of dementia.

The good news is that researchers at the Wicking Dementia Research and Education Centre may have uncovered a way to stop the onset of dementia. Findings from a long-term prospective study show that actively using the brain could be the answer.

'Our greatest strength is the critical mass of complimentary disciplines that interact here in the Wicking Centre. We are conducting research into age related diseases of the brain, trying to find ways to prevent the onset of brain disease,' said Professor Vickers.

'We are also looking at ways to prepare the world for this impending problem through education.'

'Today dementia is Australia's second biggest killer. In 10-15 years, it is likely to be number one,' said Professor Vickers.

'The brain is constantly changing. Plasticity is a measure of how malleable a brain is and also how resistant a person may be to neurodegenerative conditions such as dementia. The Centre is investigating whether actively stimulating the brain through complex mental stimulation, would keep the avenues of brain plasticity alive. Our findings to date are very promising.'

The study, The Tasmanian Healthy Brain Project, is the only trial in the world exploring education as an intervention.

'Subjects undertake study at university and have regular psychological assessments to test higher brain functions such as processing speed, reasoning and memory.'

'We are examining whether increasing the amount of study you do, will make you less likely to be at risk of serious ageing-related cognitive decline and dementia. In short, the saying if you don't use it, you lose it seems to be very much the case here.'

'The public health outcome is that we can inform people about the importance of their cognitive lifestyle – or, exercising their brain.'

'Our other goal is to explore whether there are other means of exercising the brain that will ward off dementia, because university study is not accessible to everyone. We are also interested in whether people need to exercise the brain early in life, throughout their life or later in life to get the best results.'

But it is not just scientific discovery that drives Professor Vickers.

'One of the most satisfying achievements that I have been involved with in the Centre is the development of a free online course in dementia education that has reached over 50,000 people around the world.'

'If people understand what is happening in the brain, it helps them understand the behavioural changes associated with dementia. Carers often don't have any experience of dementia until a family member develops symptoms. The Wicking Centre has shown that the standard of care in our homes, hospital and residential facilities would increase if health professionals had more knowledge about dementia.'

'With the high care needs of dementia sufferers, family members are going to be crucial in caring for dementia suffers as the population ages.'

Professor Vickers is the Director of the Wicking Dementia Research and Education Centre. His research interests include neurodegenerative disease (particularly Alzheimer's disease), traumatic brain injury, structural brain plasticity, dementia risk reduction and health services for dementia. Professor Vickers has had a keen interest in the role of education for dementia, and this has contributed to the development of the Wicking Centre’s Massive Open Online Courses in dementia, the Bachelor of Dementia Care program, and the NHMRC-funded Tasmanian Healthy Brain Project. Professor Vickers is a Past-President of the Australasian Neuroscience Society, and currently serves on the boards of the Dementia Australia Research Foundation, Royal Hobart Hospital Research Foundation and Glenview Community Services.

Professor Vickers completed a BSc(Hons) at the University of Tasmania, a PhD at Flinders University, South Australia and was awarded a DSc from the University of Tasmania in 2005.

Research Themes

Professor Vickers' research aligns with one of the University's key research themes of Better Health. He has a prominent international research identity in neurodegenerative conditions, particularly Alzheimer's disease, as well as neural injury. In this regard, Professor Vickers has a unique research background, having an extensive track record in laboratory-based neuroscience research, from human brain research through to in vivo and in vitro models, as well as interventional cohort studies, cognition, neurogenetics and health services research. Professor Vickers has published on Alzheimer's disease, amyotrophic lateral sclerosis, traumatic brain injury, glaucoma, excitatory receptors and excitotoxicty, neuronal cytokeleton in health and disease, and structural neuronal injury, plasticity and regeneration.

Development of a non-invasive screening test to detect risk of Alzheimer's disease pathology (2022 - 2024)$446,957
Description
Alzheimer's disease (AD) starts damaging the brain >10 years before memory problems begin. To reduce dementia incidence, we must detect AD earlier so we can target prevention through early clinical trial recruitment and risk reduction (that could prevent 40% cases). With 10,000 available UTAS research participants and 200 patients from RHH and the community, we aim to develop a test to detect early signs of AD by analysing hand- AND speech- movements using a smartphone and machine learning algorithms.

The future in our hands: Screening for preclinical Alzheimer's disease by analysing hand movements (2021 - 2025)$899,782
Description
Global dementia prevalence is rising. Alzheimer's disease (AD), the most common cause, has devastating effects on people's quality of life. AD has a preclinical (pre-AD) period of 10-20 years when brain pathology silently progresses before any cognitive symptoms appear. Current tests for pre-AD are invasive, costly and unsuitable for screening at population level. If we can't identify people with pre-AD, we can't offer them early interventions. This hinders our strategies to reduce AD prevalence. We will develop a new scalable test (HandTest) by combining two innovative ideas: hand-movement tests to detect pre-AD >10 years before cognitive symptoms begin; and computer vision so people can self-test online at home. This unique approach builds on the recent discoveries that the posterior cingulate cortex (the first brain region to show abnormalities in pre-AD) is involved in hand-movement control, and that hand-movement patterns change in pre-AD. We will use exquisitely precise computer vision methods to automatically analyse movement data from thousands of participants, who complete the online test by simply opening and closing their hands repeatedly for 5 minutes in front of a standard computer, tablet or smartphone camera. The CI/AI team of experts in hand-movement analysis, pre-AD, computer vision, AD biomarkers and biostatistics has access to 3 well-phenotyped cohorts, >10,000 existing participants and a cutting-edge assay for a blood AD biomarker, ptau181. We will develop a HandTest algorithm to classify hand-movement data for pre-AD risk. In a longitudinal cohort we will determine HandTest's precision to prospectively predict 5-year risks of cognitive decline and AD. This is the critical first step for up-scaling HandTest nationwide via memory clinics of the Australian Dementia Network. With worldwide access to computers, tablets and smartphones increasing, HandTest is scalable globally, potentially transforming dementia prevention and management.

Older Tasmanians undergoing emergency surgery are at high risk of complications and of subsequently needing rehabilitation or nursing home placement following discharge. We will investigate the need for a novel model of shared Geriatric/General Surgery care for older RHH patients, and measure its effect on return to pre-admission residence and in-hospital complications.

The team has allocated time (milestone indicators) and resources (budget) for extended community engagement at all stages of the project making the proposal highly feasible. Additionally, we have already established relationships with Aboriginal and Torres Strait Islander communities. Tasmanian Aboriginal Centre (TAC) and CI Schramm have assisted with the design of the project and TAC have agreed to host at least one Aboriginal health worker for the duration of this project. In addition, TAC has confirmed the transferability of the skills (e.g. communication, analysis, education design) gained from working on this project are highly desirable for future research and education programs.The incidence of TBI in the Aboriginal and Torres Strait Islander communities is almost twice the national average this means urgent attention is needed to increase awareness and educate. Tasmanian Aboriginal Centre (TAC) has indicated to the research team that brain injury awareness and education are a priority for their communities. Our research proposal aims to not only establish awareness of TBI but also the lived experience of those who have sustained a TBI. Accounts can be captured in their language making the applicability to others in the community stronger. These personal accounts may also identify areas of need for specific educational programs, which are culturally appropriate and can be developed in consultation with the local communities.

Preventative Health Research in Rural and Regional Communities (Tasmania) (2019 - 2021)$2,400,000
Description
An ageing Australia will increasingly impact social, health and economic activity, as exemplified in complex disorders such as dementia. Obesity is also increasing at a rapid rate and is a major antecedent risk factor for a range of chronic illnesses. Both conditions will be a particular challenge for rural and regional communities as they have the highest rates of related risk factors and chronic illness, and also relatively reduced access to specialist medical services and preventative health programs. This proposal focusses on dementia and obesity in north-west Tasmania, developing innovative initiatives to manage and reduce risk of these conditions, which will reduce medical procedures overall including presentations to hospital. The program involves two major projects: the Island Study Linking Ageing and Neurodegenerative Disease (ISLAND) and the Critical Age Periods for Impacting Obesogenic Lifestyles (CAPITOL) study. The ISLAND study will involve a pragmatic clustered randomised controlled on interventions for major potentially modifiable risk factors for dementia, including obesity, physical activity, diabetes, smoking, hypertension, depression and cognitive stimulation/education, most of which are antecedents to chronic illness. The CAPITOL study will facilitate a transdisciplinary professional network to build a community-specific, sustainable approach to supporting children and families in relation to physical health and wellbeing. This project will identify enabling contextual factors which influence engagement in healthy physical play practices; foster buy-in from families (at multiple entry points); improve school readiness (through enhancing outcomes for children (0-8 years) in the AEDC domain of physical health and wellbeing), and generate sustainable communities of practice. North-west Tasmania is an ideal 'test-bed' to develop preventative health strategies that are relevant and scalable to other Australian rural and regional communities.

The Tasmanian Healthy Brain Project: a longitudinal intervention study to reduce the risk of ageing-related cognitive decline and dementia (2016 - 2020)$878,790
Description
It has been estimated that between 30 and 50% of dementia cases may be preventable by addressing modifiable risk factors such as vascular factors and complex mental stimulation. Increasing levels of education at a younger age have been associated with decreased risk of developing dementia in older ages, leading to the suggestion that 'building' cognitive reserve may provide greater resilience to dementia. The Tasmanian Healthy Brain Project (THBP) is a large scale prospective trial that examines whether the 'real-world' intervention of university-level study, as a form of complex mental stimulation, in older adults may provide benefits to cognitive resilience, so as to reduce ageing-related decline in cognitive performance, and reduce the risk of dementia. The initial phase of the THBP (2010-2015) involved identifying genetic interactions that may modify risk of ageing-related cognitive decline and mediate the benefits of engagement in complex mental stimulation. In addition, the THBP has developed constructs to measure prior and current levels of cognitive reserve and performance. The proposed project will realise the long term outcomes of the THBP to determine how a university course-based intervention may affect cognitive capacity through ageing and if it may effectively protect individuals from significant decline and dementia. Furthermore, we will determine how common variations in genes linked to brain plasticity, such as brain-derived neurotrophic factor (BDNF) gene, may modify how an individual gains potential benefit from the intervention. In this regard, a genetic profile comprised from BDNF and apolipoprotein E polymorphisms, may also predict those at greatest risk of dementia. Finally, we will investigate whether BDNF protein levels in blood may serve as a biomarker for the effects of the intervention and/or as a potential indicator of relative risk or protection from significant cognitive decline.

Single-cell DNA Methylome Sequencing to detect epigenetic heterogeneity in aged pyramidal neurons (2016)$14,407
Description
Chemical and physical modification of DNA enables phenotypic variation within individuals built from a single genome. Thestudy of epigenetics has the potential to span the spectrum of biology, but has been somewhat limited to some fields (e.g. cancerand aging) or by virtue of the methods currently available to map epigenomes. DNA methylation is a major component of theepigenome and underpins normal development and survival. It is measured by whole genome bisulfite sequencing (WGBS), whichprovides information on the methylation status of DNA loci from the entire pools of cells sampled as a population; thus,knowledge regarding the cell-to-cell variation in epigenetic signatures is severely lacking. Single-cell WGBS experiments are atthe cutting edge of epigenetics research (Smallwood et al., 2014, Nature Methods, Farlik et al., 2015, Cell Reports) and will allowus to ascertain the heterogeneity in DNA methylation patterns from cell to cell. Our study will address a substantial gap in knowledge in the field by generating the first single-cell WGBS data from individual aged neurons. Pyramidal neurons are postmitotic long-lived cells with the ability to accrue epigenetic differences over time, thus, providing an ideal model to assess the heterogeneity in the DNA methylomes of individual cells in healthy aging.

Chemotherapy-induced neuropathy (CIN) is a dose-limiting side effect of chemotherapy treatment, causing irreversible disability in 40% of cancer survivors. CIN results from the loss or damage of neurons during chemotherapy. We have compelling evidence that the protein metallothionein promotes regeneration of peripheral nerves, and examine its therapeutic use in CIN.This project will create new knowledge in the area of chemotherapy induced neuropathy, and establish the suitability of trophic agents as disease-modifying agents for chemotherapy-induced neuropathy.

<p>AD is highly correlated with ageing. Alterations in neuron function are important in age-associated cognitive decline. In AD memory deficits are due to dysfunction, degeneration and death of a vulnerable subpopulation of neurons. Existing knowledge is extremely limited and does not identify why a subset of neurons are selectively vulnerable in AD.</p><p>Hypothesis: Vulnerable neurons in AD exhibit a distinct signature of alterations in healthy aging and early AD.</p><p>A multi-dimensional approach (patch-clamp electrophysiology, morphological analysis, cognitive assessment) will be used to:</p>1: Determine how neuronal activity, morphology and cognition are altered in neuron subpopulations in healthy ageing.<br>2: Examine how neuronal activity and morphology are altered in vulnerable neurons in early AD.<br>3: Identify and characterize alterations in currents underlying age- and AD-associated changes in neuronal activity.<br><p>This proposal will significantly advance our knowledge by producing data essential for understanding how neurons function across healthy ageing and how this is altered in the subpopulation of neurons vulnerable to AD, providing vital knowledge that will be valuable across key areas of neuroscience research. Understanding of the mechanisms underlying the selective vulnerability of this subset of neurons in AD will inform the design of therapeutics aimed at protecting these cells.</p>

The role of FTLD proteins in neurite health, function and dysfunction (2014 - 2016)$90,000
Description
In the last seven years a number of proteins have been identified that are pathologically or genetically associated with Frontotemporal Lobe Degeneration including TDP-43, FUS, C9ORF72 and progranulin. As ubiquitously expressed proteins it has been difficult to assess the role these proteins play in disease, particularly associated with ageing. This project proposes that the disease-associated roles are likely to stem from functions that are specific to neurons and/or glial cells. Accumulating evidence suggests that many of these proteins are involved in neurite outgrowth and cytoskeletal maintenance. This project will use a threefold approach to investigate the role of FTLD proteins in disease. First, it will utilize novel primary cell culture techniques that allow compartmentalization of the neuron to probe axonal and somatodenditic mechanisms. Second, it will utilize an in-vivo intraocular injection model to rapidly examine the effect of mutant and non-mutant proteins on downstream connectivity. Finally, it will investigate neurite pathology in human tissue. This project will provide mechanistic insight into the role of FTLD proteins in disease.

Transmission electron microscopy is a fundamental tool for the study of biological systems at the ultrastructural level. Tasmania has only one, aged and barely functioning electron microscopy facility. The proposed instrument will be established in new purpose-built laboratory space in the UTAS Medical Sciences Precinct, and will be supported by the Central Science Laboratory. Description
Effective and embedded clinical redesign is essential to the ongoing sustainability, safety and quality of health care delivery in Tasmania. In a state smaller in population and area than some single Local Hospital Networks in other states, there are significant structural, logistic, allocative and technical challenges to the provision of care in Tasmania. A central, dedicated evidence-based facility, driving redesign projects within a research framework in partnership with the purchaser, providers, clinicians and the community, will position the Tasmanian health system to meet current and future demands. This project seeks to establish a Clinical Redesign Laboratory (CRL) within the new Centre for Health Service Research (CHSR) at the University of Tasmania to provide a statewide approach to supporting healthcare research and redesign that will assist health services to make evidence-based service improvements in a systematic and sustainable way.

Evaluation of a Multidimensional Cognitive Enhancement Training Program for Healthy Older Adults (2009 - 2012)$139,141
Description
Normal ageing is associated with progressive decline in cognitive function. Even modest declines in memory function can negatively impact on the quality of life and independence of older adults. Although it is commonly believed that keeping mentally active can slow or even prevent brain ageing, whether cognitive enhancement programs can moderate cognitive decline is unclear. This project will evaluate the mental-exercise hypothesis via a longitudinal study of the effects of a 10-week multidimensional cognitive enhancement program designed to promote transfer to everyday tasks. How cognitive training affects the structure and function of the ageing brain and the effect of genetic factors on response to the intervention will also be examined.
