Why do certain groups of people develop disease while others don't? And, if we can answer that question, should we intervene? If so, will preventive interventions have a positive impact on the population overall, or will they do more harm than good? These are the types of questions that concern epidemiologists. Epidemiology is the study of determinants of disease and health-related states in populations.

Helping people make informed decisions about health

Professor of epidemiology, Alison Venn, is studying health-related issues commonly affecting Tasmanians, and others around the world. Her findings and recommendations can assist policy makers and individuals make informed decisions about health.

'Epidemiology is a discipline that spans the social determinants of health right through to molecular science. We seek to uncover the genetic determinants of disease as well as the lifestyle, environmental and health-system factors that contribute to population risk,' said Professor Venn.

Ultimately, epidemiology is about saving lives. It is about knowing what the causes of disease are, and uncovering the factors affecting which groups are healthy and which are at risk. Recommendations for treatments and preventive interventions can then be made.

The two main focuses of Professor Venn's current research are obesity and childhood determinants of health.

'Obesity underlies the development of many health problems, including cardiovascular disease, type 2 diabetes and some cancers. Once people become obese they have difficulty losing weight and, if they do, they often regain it. More than 2 million Australian adults are currently obese, which increases their risk of developing disease.

'I am exploring how we can better support people to lose weight and sustain it. Bariatric surgery is a promising treatment, but it is expensive. In the public system, current waiting lists span many years.'

Obesity is more common in socially disadvantaged communities. Professor Venn is currently leading a program that is exploring potential changes in the public health system that would make bariatric (weight loss) surgery more accessible and affordable for those in greatest need.

'I am working with health economists, sociologists, and policy makers. We are exploring the issue from the perspective of the patient, their families, policy makers, and society.'

'One argument is that funding more surgery may reduce the long-term health costs to society.'

A second study being undertaken by Professor Venn is the Childhood Determinants of Adult Health Study, which is exploring which childhood risk factors affect heart disease and type 2 diabetes in adulthood.

'In 1985, a large sample of children had their risk factors measured at school, including weight status, physical fitness, blood pressure, cholesterol levels and lifestyle. We found those kids and invited them to participate in a follow-up study. It is the only national Australian study of its kind, and one of only a handful in the world with a large sample size and long-term follow up.'

The Childhood Determinants of Adult Health Study is exploring questions like:

  • Which childhood risk factors affect adult disease?
  • How do risk factor trajectories develop over time and how do their outcomes differ?
  • When does prevention need to start?
  • Should we intervene clinically and, if so, when?

Epidemiologists around the world are now collaborating on this study, a consortium that was initiated by Tasmanian scientists. By pooling their data and testing whether findings can be replicated in other studies, they are increasing the scope of the research.

'Together we will be able to study cardiovascular disease in more than 20,000 people and all groups are using standardised methodology and classification,' said Professor Venn.

'One important finding from this collaboration is that people who are overweight as a child have much higher risk of cardiovascular disease and diabetes as adults. However, those that are overweight as a child but avoid obesity in adulthood have no increased risk.'

The current study is in the first year of a five-year project with US$13 million funding.

Alison Venn is the  Director of the Menzies Institute for Medical Research and a Professor of Epidemiology. She has a diverse background including immunology and epidemiology. Her breadth of experience from lab to policy has seen her take on a number of leadership roles, identifying multidisciplinary approaches to solving complex problems. Professor Venn's current research interests are in the causes, prevention and management of chronic disease. She has a particular focus on the factors that lead to the development of cardiovascular disease and diabetes later in life. Professor Venn holds positions on a number of committees including Director of the Tasmanian Data Linkage Unit and the Tasmanian Cancer Registry, both based at the Menzies Institute for Medical Research.


Biography

Professor Alison Venn completed her PhD in immunology at the National Institute for Medical Research in the UK. Following postdoctoral research in malaria immunology at the Walter & Eliza Hall Institute in Melbourne, she trained as an epidemiologist and spent ten years doing research on women's reproductive health at La Trobe University. Since joining the Menzies Institute for Medical Research in 2000 she has broadened her research interests to cover a range of chronic diseases.

Expertise

Professor Venn is recognised for her leadership in chronic disease, cancer and reproductive epidemiology with major grants, publications with international impact in high profile journals, and plenary conference presentations in each of these areas. She has led major cohort studies, multidisciplinary teams, national and international collaborations, and developed successful partnerships with clinicians and policy makers to address complex problems of public health significance.

Professor Venn leads the large, nation-wide Childhood Determinants of Adult Health cohort study which focusses on cardiometabolic disease, and two NHMRC partnership projects – on workplace health promotion and bariatric surgery for management of severe obesity. She is recognised for her leadership in building capacity in population health research in Tasmania.

Research Themes

Professor Venn's current research interests are in the causes, prevention and management of chronic disease. She has a particular focus on how lifestyle (smoking, physical activity, diet, alcohol consumption) and obesity in childhood and early adulthood affect the risk of developing cardiovascular disease and diabetes later in life.

Collaboration

Professor Venn has strong networks of local, national and international collaborators reflected by co-authorship of most of her publications. For example, the International Childhood Cardiovascular Cohorts (i3C) Consortium brings together the world's major cohort studies with cardiovascular risk factors measured in childhood and follow-up over several decades. Initiated in 2002 by former Menzies Director Prof Terry Dwyer, it includes 7 cohort studies. As CDAH study lead, she is one of 7 PIs on a new NIH-funded study – the i3C Cardiovascular Outcomes Study (2015-19).

Improved cardiovascular disease health service delivery in Australian (IDEAL study) (2019 - 2022)$1,071,230
Description
Cardiovascular disease (CVD) has the highest burden of global disease and highest level of health care expenditure in Australia. Most of the management of CVD is performed in general practice, where the recommended strategy is to determine absolute CVD risk based on multiple factors (e.g. cholesterol, blood pressure, diabetes, smoking). Australian primary care doctors are not yet adopting guideline recommendations, with as few as 30% using risk calculators, and this leads to people remaining at high risk for otherwise avoidable CVD events. All partners in this grant are working together to rectify this evidence-practice gap, using a new, yet simple and widely implementable, digital health service that has been developed by researchers, industry, health service providers and policy makers.The concept involves absolute CVD risk being measured at pathology services in patients referred for blood lipid assessment. Patients have blood pressure measured and CVD risk factors collated electronically by pathology services using an automated, high throughput approach, with absolute CVD risk results conveyed to general practitioners via an established pathology reporting system. Abnormal results are highlighted and advice provided regarding appropriate, guideline-directed action. Significant development work that confirms proof-of-concept has been completed. This program will refine and test the clinical value of the new health service method upon establishment statewide across Tasmanian pathology services. In an Australian first, the system is designed for research infusion, with all information data-based (through UTAS) and serving as a platform for rapid assessment of clinical value by linkage to health outcomes.Ultimately, the new service is expected to improve CVD outcomes, reduce health care costs and improve doctor and patient experiences. Importantly, the model is transferable to other national and international settings.

Centre of Research Excellence in Health in Preconception and Pregnancy: Prevention of maternal obesity (2019 - 2024)$2,496,527
Description
Two-thirds of Australian adults are overweight or obese with reproductive-aged women leading this trend. Over half of Australian women enter pregnancyoverweight or obese and gestational weight gain (GWG) above US Institute of Medicine recommendations occurs in the majority of pregnancies in Australia.Preconception higher body mass index and/or excessive GWG increase the risk of adverse maternal and foetal outcomes and increase the risk of maternal andchildhood obesity. Targeted efforts are needed urgently to improve lifestyle behaviours prior to conception and during pregnancy to assist women to be thehealthiest weight they can be, to optimise gestational weight gain and to improve short-and long term maternal and child health outcomes.The CRE in Health in Preconception and Pregnancy (CRE HiPP) aims to :1) Generate new knowledge on: a) adapting, co-designing and testing models of preconception lifestyle care; b) implementing and evaluating the integrationof lifestyle intervention into routine antenatal care; c) co-designing and testing culturally relevant models of preconception and pregnancy lifestyle healthcare; and d) estimating the economic implications of pregnancy lifestyle interventions.2) Translate this new knowledge into policy and practice through: a) our extensive professional networks and leadership roles; b) consumer and communityinvolvement; c) knowledge synthesis, dissemination, and exchange strategies; and d) by up skilling health professionals.3) Develop a multidisciplinary research workforce in preconception and pregnancy lifestyle health promotion by building capacity in early and mid-careerresearchers, and the clinical workforce, with mentoring and training programs by world leading national and international experts.4) Leverage our extensive existing multidisciplinary collaborations and proven track record of translation into policy and practice to apply CRE knowledge onhow to prevent maternal obesity.

Description
We are facing a global inactivity crisis. Participation in recommended amounts of physical activity (PA) has remained stagnant since the 1980s,indicating limited population-level impact of strategies to promote PA. A shift in focus from leisure-time to transport-related PA holds promiseas an attractive but under-exploited opportunity to support individuals to incorporate more activity into daily life. Healthy transport options -walking, cycling and public transport - are critical social determinants of health, by reducing traffic accidents, increasing social contact,reducing air pollution, and increasing exercise. Improving our understanding of travel behaviours and identifying strategies to increase use ofhealthy transport options have been identified by our three highly engaged Tasmanian Partner Organisations as important priorities. This projectseeks to generate evidence with service providers, state government and local government authorities to support decision-making and resourceallocation in relation to increasing active and public transport use. Working closely with our Partners, we will analyse data from existing datasources to better understand Tasmanians' travel behaviours and the factors that influence these, by evaluating the effectiveness of two 'realworld'fiscal strategies aiming to increase active and public transport use, and by developing and implementing a knowledge transfer strategy toensure findings are effectively communicated. This project will generate some of the first evidence internationally about the effectiveness offiscal strategies to increase active and public transport use, and will provide crucial information to directly impact the work of service providers,decision makers, policy-makers and practitioners. By identifying effective strategies to increase PA through active transport, the implications of this project have the potential to make major direct health, social, environmental and economic impacts globally.

Understanding the life course relationship of DNA methylation with obesity traits and its association with obesity-related diseases (2018)$25,000
Description
Australia is facing a public health crisis of overweight and obesity. In 2014-15, nearly two out of three (63%) Australian adults and over one quarter (27%) of Australian children and adolescents aged 5-17 years were overweight or obese. Overweight and obese children are more likely to become overweight and obese adults than normal weight children. There are substantial health benefits if childhood overweight and obesity 'resolve' over time but very little is known about what predicts this pathway. Recent epigenome-wide association studies (EWAS) suggest a close relationship between BMI and DNA methylation, a key regulator of gene expression and molecular phenotype. This project aims to investigate the relationship between DNA methylation and BMI trajectories from childhood to midlife. Furthermore, we aim to examine the association between DNA methylation and obesity-related diseases, such as metabolic syndrome, hypercholesterolemia and depression.

Using data from a large cohort of Australian children followed for over 30 years, this study will investigate how weight status and physical fitness from childhood to adulthood determine early markers of cardiovascular disease and type 2 diabetes (cardiometabolic disease) in midlife. It will provide much needed evidence to address the high prevalence of cardiometabolic disease risk factors in young Australians through better targeted disease prevention and early clinical intervention strategies.

Pathways to better health and education outcomes for Tasmanias children (2016 - 2018)$803,171
Description
All Australian states and territories provide universal health and education services for children under the age of five. The need for these servicesis starkly illustrated by the latest national figures that show that one in five children are behind in their development when they start school at the age of five. For far too many of these children, a poor start predicts a poor future. This is especially true for children growing up in Tasmania, which has the highest poverty rate, lowest school completion rate and shortest life expectancy rate, than any other state or territory.The Tasmanian Department of Health and Human Services (DHHS), Department of Education (DoE) and Department of Premier and Cabinet (DPaC) are working hard to address these problems in innovative ways to improve the lives and prospects of young Tasmanians. This project brings these government departments together with researchers, service providers and consumers to better understand the combined benefit of early childhood services in Tasmania. The power of data linkage, quantitative and qualitative methods will be combined to provide partners with a whole-of-population view of the impact of service use on child outcomes, taking risk and protective factors into account, and including the perspectives of service providers and consumers. The project will track 12,000 children from birth, through all the health and education services available up to the start of formal school at age five. The information will be used to review and reform existing services to increase the reachand benefits of services for all children, especially those with high service needs.

Childhood CV Risk and Adult CVD Outcomes: an International Long-term Follow-up (2015 - 2019)$13,288,802

This project aims to determine the association between levels of cardiovascular risk factors (anthropometrics, lipids, blood pressure, glucose, smoking) in childhood and cardiovascular morbidity and mortality in adulthood. It will do this by pooling data on more than 40,000 participants from seven major cohort studies, 5 from the USA, 1 from Finland, and 1 from Australia (the Menzies Research Institute-led Childhood Determinants of Adult Health Study). Participants in all countries will be followed up to ascertain self-reported cardiovascular disease and type 2 diabetes, collect information from medical records, and classify disease outcomes through a central adjudication process.

About 1 in 20 Tasmanians are severely obese. Bariatric surgery can improve health for individuals with severe obesity but access to surgery is poor for most public patients. This study will investigate the pathways of patients referred for bariatric surgery in the Tasmanian public and private hospital systems to better understand the demand for surgery, the health outcomes, and costs associated with these different pathways. The findings will inform public policy and resource allocation.

Guidance of Heart Failure Management Programs by Risk Assessment (2013 - 2018)$1,422,739
Description
The aims of this project are to reduce Heart Failure (HF) admissions and costs in Tasmania by: i) using data linkage to identify locations where community service delivery should be targeted, and to evaluate current performance in HF readmission across the State; ii) developing a risk algorithm for HF readmission, including psychosocial determinants; iii) using i) and ii) devising a novel means of providing a disease management program (DMP), titrated to risk assessment, with physiologic assessment used to guide the management of the highest risk patients; iv) developing a clinical trial to define the merits of this approach; v) evaluating the cost-effectiveness of this intervention; vi) presentation of educational activities with community-based providers. A successful intervention is likely to be sustainable. As HF readmissions in Tasmania cost ~4550 bed days/y, the investment in this grant (~$1/2m/y) could be reproduced by a 50% reduction of readmissions.
