Dr Andrew Williams is a Senior Lecturer, Accredited Exercise Physiologist and coordinator of the Bachelor of Exercise Physiology (Professional Honours) in the School of Health Sciences. He is a former National Director and current Research Committee Chair for Exercise & Sports Science Australia and serves on the Primary Health Tasmania Social Determinants of Health Steering Group.

Biography

Dr Andrew Williams graduated with a B Appl Sc degree from the University of South Australia in 1996, and a BSc Honours degree from Flinders University in 1998 before moving to Melbourne to complete his PhD at Victoria University. He moved to the University of Tasmania in 2003, and has since established himself as an accomplished academic with a strong background in teaching, a productive research focus and as an accomplished clinician.

Dr Williams is part of a multi-member team teaching Exercise Physiology to undergraduate and professional honours level students. He was responsible for the development and is the course coordinator of the Bachelor of Exercise Physiology (Prof Hons) course and coordinated, developed and wrote the professional accreditation application for this and the Bachelor of Exercise Science programs.

Research Themes

Dr Williams' research aligns with the University's research theme of Better Health. His main research focus is in the role of exercise in the prevention and treatment of chronic disease with particular emphasis on the effects of a range of exercise treatments on cardiovascular health and the role of technology in encouraging healthy lifestyle behaviours.

Based on his published research, he has been invited to contribute to specialist panels involved in the creation of position statements on:

  • Exercise in Chronic Heart Failure ( published 2010)
  • Exercise in Chronic Kidney Disease (published 2013)
  • The development of the Australian Pre-Exercise Screening System (APSS), a system for determining relative risks of exercise prior to participation for individuals looking to commence an exercise program

Collaboration

Dr Williams has research links with the Launceston General Hospital, the Menzies Institute for Medical Research and numerous Universities throughout Australia.

Awards

As director of the University of Tasmania Exercise Physiology Clinic, Andy led successful nominations for team awards at University (Vice Chancellors Community Engagement Award 2013) and industry (Tasmanian Allied Health Professionals Advancement Committee Connecting with the Community 2013).

Detection of early small airway abnormalities in apparently normal smokers (2021)$15,794
Description
The aim of this project is to detect early lung function changes in smokers to change disease trajectory.We will achieve this by measuring the following variables:1.Spirometry (the current reference measure of lung function based on performance of the large airways); 2. Forced Oscillation Technique(FOT; a measure of small airway function); and, 3. Blood biomarkers of respiratory inflammation and degradation of pulmonaryextracellular matrix, in three groups of individuals: 1. patients with chronic obstructive respiratory disease (COPD), 2. smokers with normallung function; and, 3. healthy individuals who have never smoked in order to find the relationship between the measures of spirometry,FOT and biomarkers across the sample population.

Cornerstone Youth Services (CYS) Nutrition and Physical Activity Capacity Building Project (2020 - 2021)$47,000
Description
This project aims to increase the capacity of CYS staff to deliver best practice interventions aimed at improving nutrition, physical activity and mental health and reducing obesity risk. This project involves:1). A needs assessment involving:*A brief pre training survey instrument and focus group/s to capture CYS staff skills, ability, knowledge and confidence about nutrition interventions for mental health, physical activity and referral activity*A focus group/s with the CYS Youth Advisory Group about the nutrition and needs of CYS clients and how to best engage young people about nutrition. *A cross sectional survey of CYS youth clients who have accessed CYS in the past 12 months. The survey will measure: diet quality, access to healthy food, understanding healthy food options, where/how healthy food information is accessed, current physical activity levels, barriers to participating in physical activity, awareness of the role of regular physical activity in the maintenance of health, and acceptable information delivery methods. 2). Development of training materials for staff and materials to use with youth clients and delivery of three staff training sessions 3). Collaborative exploration and brief service mapping of potential ongoing referral pathways and partnerships focusing on enabling healthy nutrition, physical activity and obesity prevention 4). A brief post training survey instrument and focus group/s to capture change in CYS staff skills, ability, knowledge and confidence about nutrition interventions for mental health, physical activity and referral activity after engaging in the training and accessing materials.

Zinc activation of enzymes responsible for insulin signalling and glucose homeostasis in type 2 diabetes (2019)$21,944
Description
The inability to respond sufficiently to insulin gradually erodes our health. Type 2 diabetes (T2D) results from such insulin resistance in peripheral tissues such as skeletal muscle. In fact, skeletal muscle is the major peripheral tissue that accounts for 70-80% of glucose uptake. While there are several theories on how insulin resistance may manifest, the molecular mechanisms are still largely unclear. Given the major impact this disorder has on the health of our communities, there is an urgent need for continued basic science to understand these mechanisms to appropriately design better therapeutics to treat this disorder. Recently, zinc has been highlighted as a critical metal ion implicated in maintaining glucose homeostasis and therefore is a vital target to study its effects on insulin resistance. We have recently published a role for this metal ion in regulating glucose metabolism in skeletal muscle cells through the concomitant action of several cell signalling molecules. However, it is unclear how this is achieved. Objective measurements of workplace sedentary behaviour (2018)$10,260
Description
Prior work on Exertime, a workplace e-health solution, demonstrates its ability to improve measures of health via the delivery of prompted movement breaks during working hours (e.g., Mainsbridge et al. 2015). It is unknown what causes these health improvements. Putatively, the Exertime software intervention changes the amount and pattern of sedentary behaviour and physical activity in the workplace; this causes improvements in markers of health. However, there is no evidence to support this hypothesis as no quantitative measure of behaviour has been collected in conjunction with the Exertime intervention. This quantitative sensor based data is crucial to establish the connection between behaviour change and health improvements. Crucially, for the first time, it will provide evidence of the magnitude of the changes in sedentary behaviour needed to improve health. Thus, providing an empirical basis for guidelines on reducing sedentary behaviour to improve health. To validate this new technology, physical activity will be measured for a period of seven days at each time point through the use of ActivPal accelerometers. These will be fitted and worn day and night for the entire seven day period. These trackers are reliable and valid and are currently considered the gold standard for physical activity monitoring. Our plan is to first conduct a small validity and feasibility study utilising current Exertime users at UTAS, for which we already have ethics approval.

Improving physical activity, pain and function in patients waiting for hip and knee Arthroplasty by combining targeted exercise training with behaviour change counselling. (2017)$47,241
Description
Waiting for publicly funded hip or knee arthroplasty can take over 12 months, and low physical activity levels while waiting for surgery may contribute to development of multi-morbidities and reduce health benefits post-surgery. The aim of the project is to quantify the benefits of a group exercise training program with targeted behaviour change counselling on ongoing physical activity participation and health related outcomes in patients on the hip or knee arthroplasty wait list. The objectives are to compare the effectiveness of the intervention compared to a usual care control group on daily physical activity and pain ratings and markers of co-morbidities (diabetes and cardiovascular disease).

Mobile Solution to Encourage Clients to Exercise at Home (2016 - 2017)$5,523
Description
The aim of the project is to engage clients in home exercise programs to improve their rehabilitation outcomes, through the development of a targeted smart phone application with individualised exercises, diary and interface with clinicians. The application will allow the clinician to take a video of the client performing exercises on their own phone, which they can then refer to when completing the exercises at home to ensure they are doing them correctly. A diary component of the application will allow the dose to be set and allow adherence to exercise to be monitored remotely by the clinician. The application will make it easier for the clinician to monitor and supervise exercise between visits, with flow on benefits to the client's progress.

Exercise Physiology in Practice: Development of an Evidence Based Practice Network (2016 - 2018)$116,925
Description
This study aims to: 1) investigate the effectiveness of services provided by Accredited Exercise Physiologists (AEPs) using the model of type II diabetes (T2DM) in a primary practice situation on (i) sustained participation in physical activity and (ii) surrogate markers of clinical prognosis such as cardiovascular and metabolic risk in each client; 2) estimate the association between different levels of participation and surrogate markers of clinical prognosis (dose-response); 3) estimate the relative cost-effectiveness of different frequencies of attendance; and 4) establish a methodology for regular surveillance of practice effectiveness. Clients with T2DM will be recruited for this study to one of ten university sites throughout Australia that regularly treat T2DM clients.

This project will be funded through TAZREACH and seeks to address a gap in service delivery by providing a bundle of sessions designed to increase the health literacy of the participants. The sessions focus on issues pertinent to chronic disease prevention and self-management including: nutrition, physical activity, communicating with health service providers, and goal setting. Health professionals are well-placed to assist clients with low HL through education, advocacy and support, thus improving health outcomes. These sessions will be provided by health professionals from UTAS with the support of undergraduate students wherever possible.
