In the early 1900s, when Abraham Flexner wrote the landmark report that revolutionised medical training across the world, doctors were prescribing morphine for teething babies, hydroelectric baths for migraines, and starvation diets for patients with aneurysms. Thankfully, medicine has evolved dramatically over the past century. Today’s doctors, however, largely continue to be educated in the same model that Flexner developed in 1910: a system based on scientific foundation followed by clinical immersion.

Preparing doctors to work with people and not just diseases

Dr Kathryn Ogden, an academic general practitioner at the UTAS School of Medicine, is among those who believe we need to ensure medical education is fit for purpose. As a PhD candidate researching medical education, Dr Ogden is exploring the ways that students can develop the various attributes and qualities they need to thrive as clinicians in today’s complex and evolving healthcare system.

‘The Flexner Report has been very effective for the past 100 years but now we need to think about the next 100 years,’ says Dr Ogden. ‘And while medical education has evolved to a certain extent over the past two decades, we still focus our students on accumulating clinical knowledge and skill. This is important, but medicine is about so much more than that.’

Today’s doctors need to have skills such as collaboration, patient centredness, quality improvement, self-awareness and the ability to work within systems. They need to communicate, manage difficult situations and understand what other services are available for patients. Equally significant have been the changes in healthcare conditions in that there is much more complexity and multi-morbidity; people rarely have one disease these days and there are usually multiple interactions between those diseases and conditions.

‘My research interest is in how we provide students with opportunities to develop that set of skills that will allow them to function effectively within the systems that they work,’ says Dr Ogden. ‘We want to prepare doctors who work with people and not just diseases.’

A patient-centred approach

Part of the inspiration for Dr Ogden’s research has come from her involvement with the Patient Partner Program in the Launceston Clinical School. This award-winning program teams medical students with members of the community who are living with chronic illnesses. The unique knowledge and experience offered by the community members provide a learning partnership that fosters a patient-centred approach in young doctors.

Dr Ogden is drawing upon the lessons from programs such as these to try to clearly enunciate what it is that educators should be working towards – not only the programs that are required to produce doctors who are ‘work ready’ but a definition of what that term actually means.

‘As a doctor, there are things you come to learn as you get older and wiser and you wish you could get there a little bit sooner,’ Dr Ogden says. ‘These are uncertainties, like the fact that you cannot cure everyone. But you can still help them.

‘Medical students can be very focused on curative medicine and that’s the way we train them – someone comes into the hospital with a problem, they receive treatment, and they go home. My passion is to educate students to go into the workforce without the burden of these narrow expectations. Medicine is much more satisfying when you accept the complexities and the ambiguity. Current students find uncertainty, which are more prominent in some branches of medicine, difficult and less appealing, so we’ve got a bit of work to do to turn that around.’

As well as working towards her PhD, Dr Ogden mentors clinicians from the Launceston General Hospital in her role as Research Program Development Fellow with the Clifford Craig Foundation. She also continues to work as a GP.

Dr Kathryn Ogden is an academic general practitioner at the School of Medicine, College of Health and Medicine. She is interested in medical education, professionalism and patient centred care. Dr Ogden is a current PhD candidate, her research focusing on the attributes and non-clinical competencies required for effective doctors in 21st Century complex adaptive health care systems.

Biography

Kathryn graduated from Monash University and has worked as a GP in Melbourne and Launceston. Her academic career began after completing a Masters of Public Health, when she worked at the National Stroke Foundation and at Monash University before mobbing to Tasmania. She started work with the University of Tasmania in 2007. She currently limits her clinical work to Women's Health to allow her ongoing work as a researcher and lecturer at the University of Tasmania. She is currently undertaking at PhD in medical education.

Research Themes

Kathryn’s research aligns to the University’s research theme of Better Health. Her research interests are in health services, specifically the practice of patient-centred medicine, and in health professionals’ education. The health professionals’ education research which Kathryn is currently undertaking relates to the competencies other than core clinical skills which are found in ‘good’ doctors who are able to effectively function in modern health care systems. She is interested in how complexity theory can be used as a conceptual framework to inform how we educate doctors, and to understand what pedagogies will be required to ensure doctors are equipped with the necessary attributes and competencies to achieve capability. Kathryn is also involved in a range of clinical research occurring at the Launceston General Hospital through her role with the Clifford Craig Foundation as Research Program Development Fellow.

Collaboration

Dr Ogden has a strong collaboration with the Eastern Health Clinical School, Monash University, through joint research on patient centred care and education. She is collaborating with an international group researching medical professionalism, led by Lynne Monrouxe (Chang Gung Medical Education Research Centre) and Charlotte Rees (Monash University). She also collaborates with Family Planning Tasmania in research in sexual and reproductive health. Finally Dr Ogden has a series of collaborations with clinicians at the Launceston General Hospital, where she supports clinician led research.

The aim of this project is to explore barriers and enablers to the recruitment and retention of General Practitioners (GPs) in Tasmania by evaluating the current training pathway. Measuring Organisational Requirements for Effective Patient Centred Care (MORE PCC (Phase 1) (2017)$10,000
Description
Ogden & Barr have established what constitutes Patient-Centred Care Requirements across the spectrum of health care delivery, education and training and this will enable the researchers to engage with the Holman Clinic stakeholders (managers, clinicians, patients, allied health professionals) to determine how existing service delivery is perceived to be conducted according to the rating of Conceptual Map statements for patient-centred care. The project will also contribute to the patient engagement activities of the Holman Clinic via the new patient portal development by allowing feedback on PCCR statements according to their experiences as well as providing narrative (open text) feedback on each statement via the portal which will then be analysed to for patient-reported outcomes for quality of care.

ADEA has requested Dr Kathryn Ogden and Jenny Barr to conduct x 2 full day workshops with diabetes educators and consumers based on our Patient-Centred Care Requirements research conducted in 2015/2016. This will be follow up professional development education after the national webinar we conducted earlier in 2016 which will involve feedback of educator and consumer ratings about patient-centred care statements. One workshop is expected to be held in Melbourne and one in Sydney from November 2016 - Feb/March 2017.

The aims of this project are to: establish the nature of the link between blood calcium levels and parathyroid hormone (PTH) levels in patients with acute pancreatitis, including those who are hypocalcaemia and normocalcaemic; determine whether PTH can identify patients with incipient hypocalcaemia and hence poor prognosis; better understand if and how PTH can be used as a prognostic indicator in acute pancreatitis.

Building Capacity for Undergraduate Inter-Professional Learning: Development and Piloting of an 'Inter-Professional Activities Tool Kit' (2014)$38,640
Description
This project will provide a framework to facilitate students from a variety of healthcare disciplines to learn and practice together in a clinical environment. The project will utilise the integrated clinical healthcare setting of the Northern Integrated Care Service (NICS), drawing on its multidisciplinary environment, to pilot and evaluate a suite of inter-professional Learning (IPL) activities aimed at bringing together students from a variety of disciplines in planned learning activities while on clinical placement. Where appropriate, it will also look to other clinical settings to pilot and evaluate activities to ensure the objectives of the project are achieved within the timeframe. The project will also investigate the opportunities, barriers and facilitators, both cultural and structural, to IPL in clinical settings. This project provides a novel way of achieving IPL by providing clinical supervisors and educators with activities that can be implemented in any setting where there are students from different disciplines on placement at the same time. The intention is that the activities will value add to existing placement by pulling students from differing disciplines together in the course of current placements, avoiding the need to coordinate specific IPL placements.

Food for Thought - A novel clinical pathway for obesity management. (2014)$62,307
Description
Being obese is a significant risk factor to multiple medical conditions. Medicine tends to see obesity as just the 'weight' as opposed to interlinked physiological, mental and environmental issues. This research will be in the Northern Integrated Care Centre using best evidence regarding the psychology of eating and its contribution to obesity. Trial models of care/service delivery in the area of obesity management will be conducted. The project will provide a novel service for Northern residents most at risk of obesity and related conditions. It will develop a functional network between the Northern ICC, LGH, UTAS and Active Launceston. It will identify key issues impacting on referral pathways for obesity management across community, primary care and tertiary health care in Tasmania and contribute to greater understanding of referral pathways that support ongoing sustainability of behavioural management interventions. It will develop and evaluate a model that can be applied in other health organisations.
