What happens when someone becomes addicted to a prescription medicine? Is teen drinking behaviour influenced by the age parents introduce their kids to alcohol? Is drug addiction a life sentence, or can it simply be a phase in a person's life? These are questions that psychology researcher Dr Raimondo Bruno is asking.

Minimising the effects of addiction

One project Dr Bruno and his team are undertaking looks at the use of common medications such as sleeping tablets, painkillers and wake-promoting drugs and how they affect a person's ability to drive safely. They're also testing how such drugs interact with alcohol.

'A person taking a prescription medication, such as sleeping tablets, might have a drink and think they're OK to drive. While they'd be under the legal blood alcohol limit, our studies show their driving skills can actually be significantly impaired by just a small amount of alcohol when they're also taking certain medications. We use a driving simulator and cognitive assessment so we can better understand the extent to which some common medications can affect our driving abilities,' said Dr Bruno.

Dr Bruno's work on medications and driving is being undertaken in collaboration with the Research on Alcohol Drugs and Driving group based at the University of Sydney.

'I was very surprised when I started researching drug markets: I didn't realise the extent to which pharmaceutical drugs were a thing that people were becoming addicted to. At the time, in the early 2000s, this was a bigger issue in regional areas in Tasmania, New Zealand and Western Australia that were isolated from the main drug markets than in larger population centres; now this is an issue nationally and internationally.'

'We're currently following people with chronic pain over five years to better understand factors that lead to good or poor outcomes. We want to see what things lead to escalating doses or a person becoming dependent on their prescription medication or other drugs.'

An ongoing study of young people recruited as 12 or 13-year-olds is pitched at determining whether the ways and times in which parents introduce alcohol affect a young person's future use.

'As a result of this study, I'm hoping we'll be able to make clear recommendations about what parents can do to increase the likelihood that their children will go on to make healthy choices about alcohol through their teens and later in life,' said Dr Bruno.

'I see addiction as a phase in a person's life.'

Reducing harm is the recurring thread through Dr Bruno's many areas of interest.

'I see addiction as a phase in a person's life. I hope my research can be used to reduce any harm that people might experience during their time or times of addiction, so that they don't have ongoing health problems later in life.'

'I enjoy the balance I have in my position, teaching and doing research. I get to teach people in the clinical psychology program about diagnosis and thinking carefully about the implications of giving someone a mental health diagnosis, as well as understanding the pros and cons of psychiatric medications and substance use.'

'The thing I really like about the alcohol and drug research sector in Australia is it's relatively small and strongly collaborative. Much of the work I do is as part of research teams based across the country,' he said.

Dr Raimondo Bruno is an Associate Professor in the School of Medicine at the University of Tasmania. His main research interests include the cognitive consequences of use of medications and illicit drugs; illicit drug market trends; and approaches to reduce the harms associated with substance use. He has active research collaborations with key national research centres in the substance use field, and holds conjoint positions at Turning Point Alcohol and Drug Centre, and the National Drug and Alcohol Research Centre, University of New South Wales.

Biography

Raimondo has been involved in the drug and alcohol sector for over a decade, in numerous research, evaluation, and consultancy roles. He has a strong track record in the area of substance use, particularly in relation to the epidemiology of illicit drug use and cognitive effects of psychoactive substances. He completed his PhD in 2005, and obtained a lectureship at the University of Tasmania in 2006. Raimondo is a registered psychologist, and the state representative to the Australasian Professional Society on Alcohol and other Drugs.

Professional practice

Raimondo is a Council member of the Australasian Professional Society on Alcohol and Drugs (APSAD), the leading multidisciplinary organisation in the alcohol and drug field in the Asia-Pacific. He was convenor for the 31st APSAD (2011), the principal Australasian drug research meeting, attracting >500 delegates. He is also a member of the College on Problems on Drug Dependence based in the USA.

Committee associations

Between 2008 and 2013 Raimondo was co-chair of the Pharmaceutical Use Working Group of the Alcohol and Drug Council of Australia (ADCA). This group actively contributed to drug policy nationally through development of a National Pharmaceuticals Policy Position Paper, and submissions to the National Drug Strategy, National Pain Strategy and related policies. In 2014 he was invited to contribute to the Australian National Council on Drugs reference group to provide advice into subsequent phases of the National Illicit Drug Campaign.

Since 2006, Raimondo has supervised PhD students to completion, as well as clinical psychology masters and honours students, including a Rhodes Scholar. He is currently supervising PhD candidates either as primary supervisor or in collaboration with other groups at UTAS (e.g. Menzies) and other institutes (National Drug and Alcohol Research Centre, University of New South Wales). He currently leads a research group of 15.

Raimondo has contributed to the boards of multiple local agencies, including the Alcohol, Tobacco and other Drugs Council of Tasmania, which represents non-government drug treatment services. He makes active contributions to Tasmanian drug policy, both through direct service on key advisory groups to the state Government (e.g. the Tasmanian Illicit Drug Action Plan Steering Group and the Tasmanian Inter-Agency Working Group on Drugs) and through his work being heavily cited within key drug policy documents (e.g. 'Tasmanian Opioid Pharmacotherapy Program Policy' (2012); the 'Review of Opioid Prescribing in Tasmania' (2012); the 'Tasmanian Alcohol Action Framework: 2010-2015' (2010); and 'Tasmanian Illicit Drug Action Plan 2015-17' (2015)).

Research Themes

Raimondo's research aligns with the University's research theme of Better Health and is centred around 5 main areas:

Monitoring drug markets
Raimondo's research group is part of two national studies that monitor trends in the extent of use and availability of drugs as well as the health of those that use them. The Illicit Drug Reporting System focuses on substances such as methamphetamine, heroin, cocaine, cannabis and pharmaceutical opioids, and is co-ordinated in Tasmania by Barbara de Graaff and Dr Amy Peacock. The Ecstasy and related Drug Reporting System examines ecstasy, mephedrone, methamphetamine and GHB, and is co-ordinated in Tasmania by Dr Allison Matthews. These studies are co-ordinated nationally by the National Drug and Alcohol Research Centre, and help contribute to local and national drug and health policies, for example, guiding the types of equipment that are available at needle and syringe programs and training required for drug treatment service or event staff. They are also collaborating on projects with the University of Queensland examining novel approaches to substance use epidemiology through examination of wastewater. In addition, he has a particular interest in novel psychoactive substances (such as synthetic cannabinoids and emerging psychostimulants and hallucinogens) and their use, and is collaborating with researchers internationally in this area.

How do medications affect driving safety?
Safe driving is an extremely complex process, and may be impacted by medications and other substances that affect the brain. Raimondo's research group are interested in how commonly used medications, such as sleeping aids and anti-anxiety medications, painkillers, and wake-promoting drugs affect a person's ability to drive safely, as well as how these effects may change if the drugs are combined with alcohol. Studies are conducted locally using a driving simulator and cognitive assessment, as well as in collaboration with the Research on Alcohol Drugs and Driving group based at the University of Sydney.

Are there lasting effects of illicit drug use on cognition, and how do drug combinations affect cognition acutely?
Use of many different drugs may produce residual effects on how the brain functions. Use of ecstasy (3,4-methylenedioxymethamphetamine), for example, may produce medium-term changes in the levels of serotonin, a key chemical messenger in the brain. Raimondo's research group are conducting numerous studies to better understand how ecstasy affects memory, impulsiveness, and visual functioning. They are also interested in the effects of other drugs such as benzodiazepines, opioids and cannabis; and in medications used in drug treatment such as Sativex and lisdexamphetamine. In addition, the group have conducted numerous studies examining the impact of combined consumption of alcohol with energy drinks on cognition, intoxication and behaviour.

Reducing harm from non-prescribed pharmaceutical use
Pharmaceutical drugs may not always be used in the way they were prescribed. Raimondo's research group are conducting a number of studies to understand how non-medical use of medications impacts on health, safety, and drug treatment. For example, in collaboration with researchers in the University of Tasmania's Division of Pharmacy (Dr Rahul Patel and Professor Stuart McLean) they are working to develop techniques to reduce harm that may arise if people inject pharmaceutical tablets. They are also collaborating on national projects to develop better assessments of people in opioid treatment to improve patient safety and to determine factors that predict good and sub-optimal outcomes in opioid treatment.

Reducing problem substance use
Another research theme is work that aims to reduce the likelihood that people will develop problems with substance use. For example, we are contributing to national studies aiming to understand how parents can positively affect their teenage children's alcohol use. We are also conducting work examining factors that might help or hinder people seeking treatment for substance problems, and factors that might contribute to people engaging in health-risk or health-protective behaviours.

Collaboration

Raimondo has significant experience with co-ordination of large scale multi-site studies. This includes a decade of work contributing to the national Illicit Drug Reporting System and Ecstasy and Related Drug Reporting System studies, which involve recruitment of large numbers (>800) of substance consumers per annum. These projects are key components of Australia's monitoring systems for substance use and make strong contributions to Australian drug policy at local and national levels. Raimondo's collaborative research has demonstrated for the first time the extent and characteristics of consumers of Emerging Psychoactive Substances in Australia and has led to two invited presentations (one national, one international) and ongoing international collaborations (US, NZ, Canada, UK) to better understand the health and social impacts of these drugs.

Awards

In recognition of a decade of contributions to preventing harms associated with substance use in the Tasmanian community, CI Bruno received the 'Outstanding Contributions' award in the 2013 Tasmanian Alcohol, Tobacco and Other Drug Awards.

Efficacy and safety of two versus three high dose psilocybin-assisted therapy sessions in comparison to a placebo for treatment resistant major depressive disorder (TR-MDD): A double blind, 3-arm Phas (2023)$736,386
Description
The aim of the study is to examine the efficacy and safety of two or three high dose (25 mg) psilocybinassisted psychotherapy (PAP) versus inactive control with psychotherapy in treatment resistant major depressive disorder(TR-MDD). The design is a 57-week, 3-arm, double-blind, multi-centre randomised controlled trial (RCT) will be conducted.The three arms being: 1) Three sessions of PAP, 2) Two sessions of PAP and one session of inactive control, and 3) Threesessions of inactive control. The number and length of the psychotherapy sessions will be equivalent across the arms.Methods: outcome measures will be obtained at baseline, immediately after two dosing sessions, and 3-months, 6-monthsand 12-months follow-ups. Participants: 160 will be recruited and randomly and equally allocated to the 3 treatment arms.This will include English-speaking aged 18-65 years who have MDD, are currently symptomatic scoring ≥20 on theMontgomeryAsberg Depression Rating Scale (MADRS), and have not responded to two or more evidence-basedtreatments. Primary outcome: to determine if two dosing sessions of PAP reduces depressive symptoms on the MADRS: i)immediately after the second dosing session, and ii) at short-term follow-up at 3-months.

Randomised double-blind placebo-controlled study of lisdexamfetamine for the treatment of methamphetamine dependence (2016 - 2019)$894,253
Description
AIMS: To examine the safety and efficacy of lisdexamfetamine in the treatment of adults with severe methamphetamine (MA) dependence.BACKGROUND: Australia has one of the highest rates of MA use in the world, with evidence of increasing harms such as ED presentations and hospitaladmissions. Existing treatments for MA users rely on psychosocial treatments, however these are of only modest effectiveness, particularly in heavy users ofMA. The need to develop safe and effective medications is well recognised, and research to date suggests substitution agonist therapies (as for nicotine andopioid dependence) are most promising for those with severe addiction. Lisdexamfetamine is a prodrug of dexamphetamine (converted to dexamphetamine inthe body after oral dosing) with lower abuse potential (misuse and diversion) than other stimulants. It is an exciting novel medication for treating this patient group, building upon favourable findings of previous pilot studies of dexamphetamine substitution treatment. STUDY DESIGN: A 4 year multisite (Sydney,Newcastle, Adelaide) double blind placebo controlled randomised controlled trial to be conducted in Australia's only specialist stimulant clinics. Eligibleconsenting adults (severe MA dependent users who have not previously responded to conventional treatment) will be randomised to 14 weeks oflisdexamfetamine or placebo in addition to counselling and case management. Participants will be followed up 8 weeks after the medication phase. Primaryoutcome is illicit MA use during the medication phase (self-report and objective urine drug screens). Secondary outcomes include adverse events, abuseliability, physical and mental health, other substance use, cognitive performance, psychosocial functioning, treatment retention and satisfaction.SIGNIFICANCE: The study is the first controlled trial of maintenance lisdexamfetamine for the treatment of severe methamphetamine dependence, withmajor clinical and public health implications.

Pharmaceutical opioids for chronic non-cancer pain: Evaluating health outcomes and economic impact over five years (2016 - 2019)$775,920

Chronic non-cancer pain (CNCP) is a major contributor to disability. Increased opioid prescribing for CNCP has produced concern aboutdependence and overdose in the absence on data on its long-term effectiveness. Novel statistical methods will test causal relationships over 5years between treatment, outcomes, and costs on 1,514 CNCP patients prescribed opioids. We will answer critical questions on 5-year outcomes,and whether, and for whom, opioids and other clinical interventions reduce costs.

An analysis of the experiences and motivations of clients accessing Sydney's Medically Supervised Injecting Centre and the factors facilitating or inhibiting service uptake and retention (2016)$7,000
Description
Sydney's Medically Supervised Injecting Centre (MSIC) has operated since 2001. Like other supervised injection facilities (SIFs) or drug consumption rooms (DCR), it provides a place where people who inject drugs (PWIDs) can safely inject pre-obtained drugs. Such harm reduction services aim to lower fatal overdose and HIV and HCVtransmission rates, improve PWIDs' health and wellbeing through referral into treatment programs, and increase public amenity and safety by reducing street-based drug use (Hedrich et al. 2010; IWG 2006; NCHECR 2007; Salmon et al. 2007, 2010; Thein et al. 2005). An evaluation of the MSIC (the only service of this type in Australia)confirmed that it 'positively impacts on clients, has a high level of support from local residents and businesses, has not been shown to cause an increase in local crime or drug use and saves at least $658,000 per annum over providing similar health outcomes through other means in the health system' (KPMG 2010, p. ix). Still, challenges exist for SIFs such as the MSIC in delivering harm reduction to the most marginalised and vulnerable of PWIDs.

Recovery Buddy (2015)$27,424
Description
The aim of this project is to develop, implement, and evaluate a mobile phone application (Recovery Buddy), an interactive tool to support clients during the period following substance use treatment, when the risk for relapse is high. The Bridge Program (Salvation Army) is a not-for-profit organisation offering an intensive 8-week substance use treatment program. Post-program 'aftercare' lasts up to 12-months and includes weekly/fortnightly counselling sessions. During the 8-week program, clients complete problem solving exercises and develop personalised coping plans and relapse prevention skills which they are encouraged to use during aftercare. However, client feedback suggests they may not access their coping plans when needed and may struggle to maintain their relapse prevention skills. It is a significant challenge to engage clients during the aftercare period and despite best efforts, around 60% of clients will relapse within the first 12 months of treatment. Recovery Buddy will allow clients to access their personally tailored exercises directly from their mobile phone whenever they need. Clients may choose to access exercises directly or may be directed to exercises based on how they are feeling or based on answers to regular surveys on substance use, mental health, and motivation. For example, a client indicating they are 'having a craving' would be guided through an exercise aimed at tolerating cravings.

The production and misuse of new psychoactive substances (NPS) whose licit or illicit status is unclear is of concern to health and law enforcement agencies around the world. Until now, the key methods used to identify unknown new psychoactive substances (NPS) have been traditional forensic science (forensic chemistry) analyses of seizure samples, test purchases, and pill testing. However, the dynamic nature of the NPS market described in the literature means that forensic chemistry and traditional surveys will not be able to provide a full picture of the situation of NPS use in any population. There is a real need for a cost effective method that could provide information about the actual use of NPS in the general population (or sub-population) and wastewater analysis can arguably be a strong candidate for such methodThe aim of this study is to use wastewater analysis to monitor changes in consumption of emerging illicit drugs that have been seized by Queensland Police Service and analysed by QHFSS in the last three years.

Description
A randomised, blinded, experimental study (n=30) will be undertaken to determine the dose-dependent, independent and combined impact of alcohol and energy drinks on the following outcomes: physiology (e.g. heart rate), coordination, cognitive performance, and risk-taking behaviour. Fifteen healthy males and fifteen females aged between 18 and 35 years will be assigned to one of three groups that receive either: (i) alcohol placebo, (ii) 0.5g/kg alcohol or (iii) 0.65g/kg alcohol. Participants will complete four experimental sessions in random order: allocated alcohol dose + 3 standard cans of 250ml Red Bull energy drink (combined condition: strong); allocated alcohol dose + 2 standard cans of 250ml Red Bull energy drink (combined condition: medium); allocated alcohol dose + 1 standard can of 250ml Red Bull energy drink (combined condition: weak); allocated alcohol dose + energy drink placebo (alcohol condition). Sessions will be approximately 3 to 4 hours in duration (depending on allocated alcohol dose) and separated by a minimum of two and a maximum of 10 days apart. Forty minutes after commencing beverage consumption, participants will complete several measures of coordination, cognitive performance, and risk-taking behaviour. Participants will have the opportunity to be reimbursed up to $20 per session for task performance in order to simulate real-life risky decision-making where there are tangible rewards and consequences of behaviour. Objective measurement of physiology outcomes (i.e., heart rate, blood pressure) will be taken throughout the session, as will measures of perceived intoxication. Following completion of the test battery participants will remain in the laboratory until they have recorded two consecutive BAC of 0.020% within a 15 minute period (0.00% BAC for provisional/learner licensed participants intending to drive). Participants will be debriefed regarding treatment order at the conclusion of participation and reimbursed $150 (including time and travel reimbursement).
