Prof Andrew Palmer is a New Star Professor of Medical Research and the Head of the Health Economics Research Unit at Menzies, working in the field of health economics since 1994. He is a leader in the subject of health economics in chronic diseases, has conducted research and published extensively in many disease areas including aged care/dementia, diabetes, multiple sclerosis, kidney disease, hypertension, dyslipidaemia, osteoporosis, alcoholism, growth hormone deficiency, rheumatoid arthritis, cardiovascular disease and oncology.

As well as being the Associate Editor (Health Economics) of Diabetic Medicine, he is the 2013-15 International Diabetes Federation (IDF) Chairman of the Task Force on Health Economics, the Co-founder/current Steering Committee member of the Mount Hood Modelling Diabetes Group, a Member of the 2013-15 Heart Foundation Future Fellows Selection Committee, member of the 2013-15 National Diabetes Strategy Group reporting to the Federal Minister for Health, Member of the Diabetes Australia Health Economics Expert Reference Group, and was an American Diabetes Association panel member for the development of diabetes health economics modelling guidelines.

Biography

Prof Palmer studied medicine at the University of Tasmania, after which he worked at the Launceston General Hospital, then as a general practitioner in Sydney. In 1994 he moved to Switzerland, where he was researcher and Medical Director of the Institute for Medical Informatics and Biostatistics (IMIB) from 1994 until 2000. CI Palmer co-founded CORE, Center for Outcomes Research, in Basel, Switzerland in July 2000 and was Medical Director and CEO until 2005. He was named by the International Society for Pharmacoeconomics and Outcomes Research as one of the top 10 most influential people in health economics in 2005.

Research Themes

Professor Palmer's research aligns with the university's research theme of Better Health. He has conducted research and published extensively in the area of health economics and the modelling of diabetes and diabetes prevention.

In addition to his work in the area of diabetes, CI Palmer has conducted research and published extensively in other disease areas including multiple sclerosis, nephropathy and renal failure, hypertension, dyslipidaemia, deep vein thrombosis and pulmonary embolism prevention, osteoporosis, alcoholism, growth hormone deficiency, rheumatoid arthritis, cardiovascular disease and oncology.

CI Palmer led the development of the CORE Diabetes Model, which has been used to inform reimbursement and policy decisions regarding diabetes interventions on an international scale. The CORE Diabetes Model is the most widely published diabetes health economics model, and has been used in over 30 reimbursement submissions for pharmaceutical agents and devices worldwide. The methodology applied by CI Palmer's team has been used as the basis for international recommendations for guidelines in diabetes modelling.

CI Palmer co-founded, has chaired and been an active member of the Steering Committee of the Mount Hood Modelling Group, committed to the increased accuracy, validity and usefulness of complex diabetes models throughout the world. He was part of the American Diabetes Association Panel for developing guidelines/recommendations on the development of models of diabetes. The data generated from the MS prevalence and cost of illness study was used by Australia's leading MS advocacy group, MS Research Australia, to lobby the federal government for implementation of the Disability Insurance Scheme (in the meantime this has been passed with bipartisan support in the Federal Parliament).

CRE for Pulmonary Fibrosis - towards individualised care (2023 - 2027)$2,500,000
Description
Interstitial lung disease (ILD) encompasses a group of lung disorders characterised by fibrosis of the lung tissue. Typically ILD patients experience significant impacts on quality of life, and effective treatments are lacking. The most common form idiopathic pulmonary fibrosis is also the most serious with a life expectancy of 2-5 years. This team is at the forefront of international developments prompting a new and individualised approach to care of patients with ILD. Recent advances show there are common underlying genetic and biological pathways, and new anti-fibrotic medications are effective across the whole ILD spectrum. In this CRE-ILD, we bring together a new multidisciplinary, diverse team from across Australia, to address patient-identified research priorities.

A randomised clinical trial of a digital self-management package for people with Interstitial Lung Disease (the REBUILD-SM trial) (2023 - 2024)$1,999,997
Description
Interstitial lung disease (ILD) encompasses a group of lung disorders characterised by fibrosis of the lung tissue. Whilst twonew antifibrotic drugs slow th";􀁿.ration of progressive ILD, there is unmet need for treatments to improve health-related quality of life (HRQoL).Research priorities identified by people living with ILD include treatments to improve HRQoL, and ILD-specific interventions to support selfmanagement.Recent data show the important role of self-management to improve outcomes for patients with other lung diseases, with resultant improved HRQoL,but its role in ILD is yet unstudied.Our team has developed a world-first, evidence based, ILD-specific self-management package in partnership with patients and clinicians. We have alsodeveloped and tested the RE-BUILD (Registry for Better Understanding of ILD) smartphone application, linked to the Australasian IL,D registry. We willnow integrate the ILD-specific self-management package with our RE-BUILD app to form REBUILD-SM, and intervention with improved reach anddelivery.We hypothesise that REBUILD-SM will improve HRQoL for people living with ILD, through better self-efficacy and reduced symptom burden andanxiety, leading to reduced health care utilisation.Our expert multi-disciplinary team will evaluate the efficacy and cost-effectiveness of the technology-delivered intervention in a randomised clinicaltrial.Aim 1. To compare the clinical efficacy of REBUILD-SM versus standard care in people with ILD.Aim 2. To compare the cost-effectiveness of REBUILD-SM versus standard care in people with ILD.Aim 3. To understand barriers and facilitators for adoption of REBUILD-SM by people with ILD and health professionals to underpin futureimplementation and drive uptake for clinical practice. This world-first study will provide the high-quality evidence needed to drive uptake of this digitalself-management intervention, and ultimately improve best practice care for people with ILD across Australia.

Eliminating hepatitis B in remote Indigenous settings: generating health economic evidence (2022 - 2025)$750,738
Description
Chronic hepatitis B (CHB) is endemic across Indigenous communities in the Northern Territory (NT), with prevalence between 2-5%. Of this group, up to 25% will die from decompensated cirrhosis or liver cancer without care and treatment. In turn, liver disease is the third leading factor contributing to the gap in life expectancy between Indigenous and non-Indigenous Australians.In 2017 the NHMRC funded the Hep B PAST Partnership Project (GNT1151837), which aims to eliminate CHB from NT Indigenous Australians. This is being achieved through two mechanisms: improvement of CHB health literacy amongst participating Indigenous communities and healthcare providers; and transition of CHB care to a primary healthcare based, co-ordinated chronic disease model supported by the establishment of a CHB clinical register.The preliminary results of the Hep B PAST Project have demonstrated a highly effective model of care to increase rates of CHB diagnosis, treatment and community education. The Hep B PAST Project will be completed in mid-2023. To successfully eliminate hepatitis B, it is now critically important to transition this care model Territory-wide. Health economic evidence is now required by government to support this. We have developed five studies to generate this evidence:1: Quantify costs of the Hep B PAST model of care and the status quo approach to CHB care;2: Quantify quality of life/health state utility values for hepatitis B health states;3. Develop and validate a health economic disease simulation model for hepatitis B forIndigenous communities;4: Project clinical outcomes of the Hep B PAST model of care and the status quo up to 2035;5: Assess the cost-effectiveness of introducing the Hep B PAST model of care as standardpractice across the NT.We will develop the first Indigenous-specific health economic model that will be made available to support rapid, robust evaluations to introduce Hep B PAST into other states/territories.

Look before you leap: how transthoracic echocardiography before fractured neck of femur saves lives in frail older people. The proposed pragmatic ECHONOF III Trial (2022 - 2026)$3,686,075
Description
For our frail and elderly citizens who fall each year, hip fracture is the frequent result. Urgent surgery offers the best hope of restoring independence and mobility, but it is one of medicine's highest risk procedures. One in 10 patients dies within 30 days, and three in 10 dies within the year. Three in 10 are so frail they must depend on long term nursing home care. Many of these consequences result from underlying cardiac disease, often unknown at the time of surgery. However, pre-surgical focused cardiac ultrasound (FCU) is effective, accurate, and quick in diagnosing heart disease, and does not delay surgery. In our randomised pilot of 100 patients, FCU led to a 39% reduction in a composite outcome of death, acute kidney injury, stroke, myocardial infarction, pulmonary embolism or cardiac arrest at 30 days after surgery. Although FCU is rapidly replacing the stethoscope, there is significant costs of equipment and training required for its widespread use and there is no definitive evidence of improved patient outcome, hence a large pragmatic randomised trial is required. Our group are world leaders in teaching and research in FCU and are the best team to conduct this trial. Ultrasound empowers healthcare professionals to make the best management decisions, potentially transforming patient outcomes and alleviating the vast financial burden of sub-optimal results. The proposed clinical trial of 2000 participants and up to 15 national and international sites, following the same protocol as the pilot. The pilot demonstrated excellent group separation of the primary outcome favouring FCU, good recruitment rate, multicentre collaboration, ease of delivery of the intervention and trial management. This trial is supported by the Australian and New Zealand College of Anaesthetists Clinical Trial Network, and we have sufficient trained staff and equipment at the hospitals, assuring successful completion of this trial.

Understanding the economic burden of climate-related extreme events: A framework to support future planning and decision making in the health care sector (2020 - 2021)$57,528

This project blends climate change projection data with health economics to estimate the future health and economic impact of climate-related extreme events in Tasmania. By building on our team's previous research examining the historical impact of heatwaves on Tasmania's health system, this research provides a foundation for policy development and health system planning. Importantly, in addition to specific information on heatwave impacts, this research will provide a generalisable framework to allow the health economic impact of climate change to be estimated for other climate-related natural disasters and compound extreme events. This information can be used by health care services both in Tasmania and across other jurisdictions.

The Menzies Institute for Medical Research Multiple Sclerosis Flagship Program (MS Flagship) leverages the unique critical mass of MS research in Tasmania connected with Australian MS researchers, collaborators, MS societies and people with MS to address the gaps in the assessment, diagnosis and treatment of MS. Our work builds on our well-developed collaborations and laboratory, epidemiological, health economics and clinical research platforms. Our aim is to improve the clinical management of MS by slowing, halting and even reversing the rate of MS disease progression and to improve the quality of life and reduce the impact that MS has on individuals, their families and society. InforMS - an electronic patient-driven health care model with digital biomarker monitoring that improves the clinical care of people with MS (2020 - 2025)$1,167,815
Description
Multiple Sclerosis (MS) is a high cost and high burden neurological disease.Problem: there are many disease-modifying therapies available, so clinicians can theoretically switch treatments rapidly based on disease activityand side-effects. Yet this proves difficult when data on these is infrequently collected (currently 1-2 times a year) and is insensitive tofluctuations between visits.Solution: modern computing technologies (mobile apps, wearables) are changing the way health is monitored throughout medicine. We areenabled to collect more data, more accurately, and more frequently, and to share it more easily. If we could effectively harness such newmonitoring technologies for MS, clinicians could be supported with objective, prospectively collected, frequently measured, change-sensitivedata.Proposed research: we will create an electronic patient-centred health care system, InforMS, that becomes a one-stop-shop managementsystem for people with MS. It visualises patient-reported data, allows daily tracking and uploading of data from smartphones and wearablesensors, and allows sharing with clinicians via print, email, third-party access and via MSBase: the most widely used clinician registryworldwide. For clinical care, it would (1) enable timelier changes to treatments, and (2) provide greater confidence that treatments work. Forpatient self care, it would (3) empower people with MS to take a central role in managing their disease, and (4) activate them to collect noveltime-sensitive data. For research, it would (5) deliver powerful annual trend data to enhance MS research. This is a paradigm shift in MS careand research.We already have a prototype of InforMS, developed together with end-users. In this proposal, we will add several key functionalities, roll out thefull InforMS system within a registry of ~3,000 people with MS, and follow participants over time to test and optimise the system in practice.

What is the economic impact of allergic respiratory disease in Tasmania? (2019)$10,000
Description
This project will provide the first economic assessment of the burden of allergic respiratory disease (allergic rhinitis and allergic asthma) in Tasmania. Together, allergic rhinitis and allergic asthma affect over 20% of the Tasmanian population, yet there has been no rigorousanalysis of the health and economic impacts. This project will cost the direct and indirect costs of these conditions across Tasmania. In doing so, we will provide a rigorous evidence base to build a case for allergic respiratory research, and directly inform government and non-government investment in interventions. Our research will be co-designed with expert input from government and non-government stakeholders (Asthma Australia) to ensure alignment with end-user needs.

The Royal Hobart Hospital's innovative Rapid Access Chest Pain Clinic (RACPC): A health economic evaluation of patient, RHH and community benefits (2019)$24,952
Description
The overall aim of this project is to conduct a comprehensive health economic evaluation of the RHH's cutting-edge Rapid Access Chest Pain Clinic (RACPC) patient care model, compared to the usual care General Cardiology (GCC) clinic model for RHH patients with newly presented chest pain. The RACPC commenced in June 2014 and has realised positive clinical outcomes for its patients. Our comprehensive health economic investigation will build on the clinical work to systematically evaluate the health economic benefits of this innovative model's patient pathways for people who present to the RHH's Emergency Department (approximately 70%) or by referral from a primary care physician (approximately 30%) with new onset chest pain.

The LSCS trials a new model of coordinated care for clients with complex needs . Three community service organisations, Australian Red Cross, Mission Australia and Baptcare will employ 12 Lead service. Coordinators to provide the service to a maximum of 30 Tasmanians with multiple service needs. Clients will work with the coordinator to identify needs and match them to available services. The evaluation aims to test if this model of coordination leads to better health and wellbeing outcomes for the clients and more efficient and cost effective service delivery.

Reducing the Burden of Liver Cancer for Tasmanians (2018)$350,000
Description
Hepatocellular carcinoma (HCC) is the most common form of primary liver cancer and is the third leading cause of cancer deaths internationally. It occurs most commonly amongst patients with cirrhosis of the liver, which is often caused by chronic infections with the hepatitis B and/or hepatitis C viruses. The clinical outcomes for patients diagnosed with HCC are highly dependent of the stage at which it is detected. Late detection is associated with significant morbidity and mortality. If detected early, HCC is potentially curable.Screening programs have been suggested to increase the rate of early diagnosis for at-risk groups. In Australia, current guidelines recommend HCC screening for all patients with cirrhosis, regardless of liver disease severity or aetiology. Screening consists of 6-monthly liver ultrasound and alpha-fetoprotein tests. Uptake of screening for HCC is very low in Australia, evidenced by the 5-year survival rate of just 17%. The aims of this project are to a) evaluate current uptake and adherence of HCC screening across Tasmania, b) develop strategies aimed at supporting at-risk patients to participate in regular HCC screening, c) evaluate the cost-effectiveness of screening interventions, and d) assess the effectiveness of new treatments for hepatitis C in reducing the rate of HCC in Tasmania.

Health by Stealth: Increasing Physical Activity through Active Travel in Tasmania (2018 - 2020)$282,362
Description
We are facing a global inactivity crisis. Participation in recommended amounts of physical activity (PA) has remained stagnant since the 1980s,indicating limited population-level impact of strategies to promote PA. A shift in focus from leisure-time to transport-related PA holds promiseas an attractive but under-exploited opportunity to support individuals to incorporate more activity into daily life. Healthy transport options -walking, cycling and public transport - are critical social determinants of health, by reducing traffic accidents, increasing social contact,reducing air pollution, and increasing exercise. Improving our understanding of travel behaviours and identifying strategies to increase use ofhealthy transport options have been identified by our three highly engaged Tasmanian Partner Organisations as important priorities. This projectseeks to generate evidence with service providers, state government and local government authorities to support decision-making and resourceallocation in relation to increasing active and public transport use. Working closely with our Partners, we will analyse data from existing datasources to better understand Tasmanians' travel behaviours and the factors that influence these, by evaluating the effectiveness of two 'realworld'fiscal strategies aiming to increase active and public transport use, and by developing and implementing a knowledge transfer strategy toensure findings are effectively communicated. This project will generate some of the first evidence internationally about the effectiveness offiscal strategies to increase active and public transport use, and will provide crucial information to directly impact the work of service providers,decision makers, policy-makers and practitioners. By identifying effective strategies to increase PA through active transport, the implications of this project have the potential to make major direct health, social, environmental and economic impacts globally.

Ambulatory oxygen for interstitial lung disease (2018 - 2022)$1,503,717
Description
Exertional desaturation (low blood oxygen during exercise) is a hallmark of fILD, occurring in over 50% of patients. It is sometimes treated withambulatory oxygen therapy (AOT), which involves breathing supplemental oxygen during physical activity. However the absence of clinicaltrials has given rise to marked variations in policy and practice globally. Even where AOT is available, treatment adherence using the traditionaldelivery method of cylinder gas is extremely poor, as patient burden frequently outweighs any benefit. We have shown that (1) oxygen therapyimproves exercise capacity and markers of skeletal muscle metabolism, suggesting that AOT may impact on physical function, (2) AOT can bedelivered effectively in fILD using a portable oxygen concentrator (POC), which is lighter and more manoeuverable than a cylinder; and (3)POCs are acceptable to patients. We have recently modified a POC to deliver air, providing the effective sham treatment needed for a robustclinical trial.This multi-site randomised controlled trial will determine the clinical benefits and societal costs of AOT for people with fILD and exertionaldesaturation. Our hypotheses are that (1) AOT delivered using a POC will deliver clinically significant improvements in physical activity,symptoms and HRQoL; and (2) AOT delivered using a POC will be cost-effective from a societal perspective. We will conduct a randomisedcontrolled trial with blinding of participants, assessors and clinicians, and an embedded economic evaluation. 260 participants with fILD andexertional desaturation will be randomly assigned to use either AOT or air delivered using a POC for 6 months. If this trial demonstrates clinicaland economic benefits of AOT then the findings can be rapidly translated into practice, with global impact on ILD outcomes and costs.

Based on the published report 'Economic Impact of Multiple Sclerosis in 2010 - Australian MS Longitudinal Study (AMSLS)' Roche is interested in cost and utility data collected from patients with primary progressive multiple sclerosis (PPMS) only.Despite the type of MS not specifically being captured in the 2007 Cost Diary, 39 people with PPMS could be identified by matching participants with a more recent data collection.Roche is seeking Australian, PPMS-specific cost and utility data to inform a PBAC submission lodged in March 2017. A sub-analysis of the data presented in the 2010 report for PPMS patients only was identified as a viable data source to inform the PBAC submission. Sustained Effective Oral Care to Significantly Reduce Aspiration Pneumonia experienced by Adults with Dementia in Residential Care (2016 - 2017)$99,572
Description
Oral care is critical to keeping the mouth clean and the body and brain as healthy as possible. This is especially the case for adults with dementia who rely on others for care. With poor oral health, harmful bacteria throughout the mouth cause tooth decay, gum inflammation, gum disease, and tooth loss. Frequently adults with dementia take multiple medications and the side-effects of these medications include reducing saliva and drying the mouth which in turn increase the harmful oral bacteria. When these harmful bacteria are breathed (or aspirated) into the lungs, the lungs become infected and aspiration pneumonia results. Adults with dementia, particularly those who live in residential communities, are vulnerable to aspiration pneumonia as they are often frail, not well nourished, have complicating medical conditions such as diabetes, heart disease, lung disease, and swallowing problems, as well as needing help from others for their care. We know, from published studies, that vulnerable adults who have their own teeth need to brush their teeth for 2 minutes after each meal. Adults who wear dentures need to clean their dentures each day and take them out when they are planning to sleep. Whether they have their own teeth or wear dentures, all adults with dementia also need to rinse their mouth after each meal. Unfortunately, oral care for adults with dementia in residential communities remains inconsistent and fragmentary. Our studies have documented the barriers, real and perceived, that prevent systematic oral care and we have developed a protocol to overcome these barriers. This protocol has five components: (1) establishment of an innovative Community of Practice (CoP) including health care professionals in the community-at-large, policy-makers, and consumers, together with visionary residential care leaders and nurse-carer champion partners to support staff and drive practice change; (2) education of nurse-carer champions by CoP members; (3) ongoing education and training of all point-of-care staff by nurse-carer champions as they work with RACF-based dental/oral health therapy students, and are supported by CoP members and online resources; (4) implementation of daily evidence-based oral care by staff; and (5) regular audits of daily oral care by nurse-carer champions and the students to ensure its sustained implementation.To measure the effectiveness of our protocol, we will follow 60 residents in each of two residential care communities in Tasmania. We will document the number of cases of aspiration pneumonia these residents experience before and after a 3-month period of daily evidence-based oral care (component 4 of the protocol). We also will document the number of times these residents have been transferred to hospital or admitted to hospital before and after the 3-month period of daily oral care. To document improvements in the residents' oral, nutritional and general health and their quality-of-life (QOL), we will use the following measures that are appropriate for adults with dementia: (i) Oral Health Assessment Tool (OHAT), (ii) Mini-Nutritional Assessment (MNA), (iii) 3-day food record, (iv) Yale Swallow Protocol that identifies any signs of swallowing difficulty, (v) Sit-to-Stand Manoeuvre that identifies any difficulties with strength and movement, and (vi) the EuroQol-5 dimension tool that documents residents' perceptions of their ability to move, care for themselves, and participate in daily activities along with any pain, discomfort or anxiety they are experiencing. We will administer these measures to the residents before and after the 3-month period of evidence-based daily oral care. We will analyse the data and write reports about the findings. We will publish these reports in academic journals and magazines that are widely-read by the public. Comparison of learning focused cardiac ultrasound between self-directed/simulator and instructor/live model teaching (2016)$32,000
Description
Our proposed project aims to test whether our new self-directed learning simulator course is as effective as the current standard teaching model that uses lots of instructors and human volunteer models. The advantages of the simulator course include reduced reliance on instructors, increased availability of the course (ability to train a lot more doctors than the current standard course) to and the ability to also teach learning of diagnosis of heart disease (not possible with the current human model). A health economic analysis will also be performed to determine if the new simulator course is as cost-effective (economical) as versus the traditional course. If the new course is as effective and economical as the current course then the simulator course may become the new method for teaching bedside ultrasound as it has important advantages over the traditional method such as increased accessibility and capacity to train doctors, and the ability to teach how to diagnose heart conditions. This would result in significant health benefits to a very large number of patients worldwide.

The number of people with multiple sclerosis (MS) is increasing dramatically in Australia, with rapidly escalating associated costs, both direct due to medical expenses and indirect due to lost or reduced employment. Existing and upcoming treatments for MS are expensive, with current prices of up to $25,000 per patient per year and rising. Using the best available data, this study will identify the treatments that are greatest value for money for the most common form of MS (relapsing-remitting MS), with a goal to reduce wastage of limited healthcare resources.

Bariatric surgery in Tasmania: investigating health service use, costs, patient outcomes and policy options (2014 - 2019)$839,027

About 1 in 20 Tasmanians are severely obese. Bariatric surgery can improve health for individuals with severe obesity but access to surgery is poor for most public patients. This study will investigate the pathways of patients referred for bariatric surgery in the Tasmanian public and private hospital systems to better understand the demand for surgery, the health outcomes, and costs associated with these different pathways. The findings will inform public policy and resource allocation.

Guidance of Heart Failure Management Programs by Risk Assessment (2013 - 2018)$1,422,739
Description
The aims of this project are to reduce Heart Failure (HF) admissions and costs in Tasmania by: i) using data linkage to identify locations where community service delivery should be targeted, and to evaluate current performance in HF readmission across the State; ii) developing a risk algorithm for HF readmission, including psychosocial determinants; iii) using i) and ii) devising a novel means of providing a disease management program (DMP), titrated to risk assessment, with physiologic assessment used to guide the management of the highest risk patients; iv) developing a clinical trial to define the merits of this approach; v) evaluating the cost-effectiveness of this intervention; vi) presentation of educational activities with community-based providers. A successful intervention is likely to be sustainable. As HF readmissions in Tasmania cost ~4550 bed days/y, the investment in this grant (~$1/2m/y) could be reproduced by a 50% reduction of readmissions.
