Arthritis, it seems, has been with us since the dawn of time. There are even reports that ice age skeletons have shown signs of osteoarthritis. Professor Chang Hai Ding's work at the Menzies Institute for Medical Research at the University of Tasmania is centred on researching risk factors of osteoarthritis and evaluating new therapies for this disease and other inflammatory arthritis.

Seeking new answers for an old problem

The sad statistic that we all face is that around half of people who are over 50 years of age will develop osteoarthritis. The disease brings aching and stiff joints as well as severe pain in some serious cases. For some people it can mean it becomes very difficult to walk. Many people who suffer from osteoarthritis can't go up and down stairs and will eventually need total knee replacements.

Osteoarthritis is linked to degradation of the cartilage in your bones, the material that cushions the area where bone meets bone. When you suffer cartilage damage you will notice that your joints may become inflamed or swollen and when the bones touch because the cartilage has been damaged, you will suffer pain, joint stiffness and restriction of movement. The disease can be crippling for some.

Professor Ding's work, with others has led to the identification of major risk factors in why we develop osteoarthritis such as:

* Age, particularly when over 50 years of age
* Obesity, the higher your levels of body fat, the higher the risk of getting osteoarthritis
* Overweight in childhood will lead to increased knee pain in adults
* Women are twice as likely to be affected as men
* Genetics, you may be more at risk because of family genetic links
* Smoking can interact with genetics leading to loss of cartilage
* Vitamin D deficiency leads to faster breakdown of cartilage and knee pain
* Inflammatory and metabolic factors can lead to cartilage loss and/or knee pain

'My ground breaking work has been in identifying vitamin D deficiency and how that affects osteoarthritis,' said Professor Ding. 'Vitamin D deficiency can lead to loss of cartilage volume and pain in the knee and when that happens the likelihood of developing osteoarthritis increases. One of the reasons why that is important in Tasmania is that here many people suffer from a deficiency in vitamin D, mostly from lack of exposure to sunlight. That means there would be a higher prevalence of osteoarthritis here than on mainland Australia.

'In 2009 we obtained $1 million NHMRC grant to conduct a clinical trial testing the effect of vitamin D treatment on osteoarthritis, and discovered that using this dietary supplement could lead to pain reduction, physical function improvement and joint effusion reduction. We would really like another round of funding to develop this line of research further as the disease affects so many people, not just here in Tasmania, for this is a world-wide problem.'

'I come from the eastern area of mainland, China, and have been in Australia since 2000, so I have been here in Tasmania for almost 15 years. I came here first to work in the Menzies Centre for Population Health Research, because that was my area of interest and expertise.

'When I was growing up in China I wanted to be a doctor, so I studied Medicine, and graduated in 1990. Then I got my Master's degree in 1993 and Doctor's degree in 1999 in Clinical Pharmacology and did a lot of clinical trials in China. I came to Tasmania working in the area of Clinical Epidemiology. I really like doing this kind of research where we use volunteers from the Tasmanian community to help us with our research. We enjoy a great relationship with the wider Tasmanian community and it makes the work very rewarding.

'I still have very strong links to Chinese universities and because of that there are many PhD students here at UTAS from those Chinese connections, as well as visiting scholars.' Professor Ding has helped set up close collaboration between Menzies and several universities in China. So far 30 PhD students have been recruited from China to study in Tasmania at UTAS.

Professor Ding has co-ordinated more than 10 clinical trials for the treatment of rheumatoid arthritis and osteoarthritis, undertaken a lot of national project grants to explore aetiology of osteoarthritis and osteoporosis, and published extensively in these areas internationally.

'Recently I reported in the International Journal of Obesity that body fat was associated with increased and lean mass with decreased knee cartilage loss in older adults, suggesting loss of body fat but gain of lean mass are important strategies to prevent osteoarthritis progression. This is still a work in progress.'

Prof Changhai Ding is an ARC Future Fellow at the Menzies Institute for Medical Research. He is a member of Global Engagement Committee, UTAS; Associate Director, Menzies Institute for Medical Research; and Director, Arthritis Research Institute of Anhui Medical University. He has received >10 awards including a Future Fellowship by Australian Research Council (Level 2, 2012-2015), Career Development Award by NHMRC (Level 2, 2008-2011), 'Ten of the Best Research Projects 2013' by NHMRC, Servier Young Investigator Prize by Servier Research Institute (France) and Rising Star by UTAS. He is interested in epidemiological and clinical investigation of osteoarthritis and osteoporosis.

Biography

Changhai Ding is a Professor at University of Tasmanian, Honorary Professor at the University of Sydney, and an Adjunct Associate Professor at Monash University. He is currently an Adjunct Professor at Anhui Medical University, a Guest Professor at Xi'an Jiaotong University and a Visiting Professor at Shanghai University of TCM, China. He joined the Menzies Institute for Medical Research in 2000, had been a Senior Research Fellow during 2006 - 2009, and an Associate Professor during 2010-2013, and has been a Professor since 2014. Before joining UTAS, Changhai was a Professor (1999-2000), an Associate Professor (1995-1998) and an Associate Lecturer (1993-1995) at Anhui Medical University, China.

He is a Deputy Section Editor for 'BMC Musculoskeletal Disorders', an Associate Editor for 'Reviews on Recent Clinical Trials', an Editorial Board Member for other 7 international journals.

He is a Vice-President of Australia Chinese Association for Biomedical Sciences (ACABS), Chairman of ACABS Tasmania Branch and Vice President of Tasmania Chinese Professionals Association. He is the President of Australia Anhui Association (Melbourne), which has 300 members originally from Anhui Province, China.

Administrative expertise

Changhai is a member of Global Engagement Committee, UTAS, and Associate Director, Menzies Institute for Medical Research. He has helped to set up collaborations between Menzies Institute and Anhui Medical University, Southern Medical University and Xi'an Jiaotong University in China. In the last 3 years, 32 PhD students have been recruited from China, and around 50 visiting fellows from China have studied in University of Tasmania.

Changhai was the Chairman of 3rd Australia China Biomedical Research Conference Hobart Satellite Meeting and around 40 Chinese academics attended this meeting in 2011. Now he is organising 5th Australia China Biomedical Research Conference Hobart Satellite Meeting.

Research Themes

Changhai's research aligns to the University's research theme of Better Health. His research interests include understanding the roles of inflammatory biomarkers, adipokines and vitamin D in knee osteoarthritis and osteoporosis. His findings suggest that inflammatory and metabolic mechanisms are very important to maintain knee cartilage health suggesting aiming at these factors may prevent and/or treat knee osteoarthritis.

He has been responsible for the Magnetic Resonance Imaging (MRI) work on knee conducted at the Institute that has been acknowledged internationally. Using image processing, he developed the classification system of knee cartilage defects and scoring methodology for subchondral bone density, and initiated measurements of cartilage thickness, infrapatellar fat pad size and quality. He has created some new interpretations. For example, he found that smoking leads to knee cartilage loss and defect development primarily in those with a family history of knee osteoarthritis providing evidence for gene-environment interaction in the aetiology of knee osteoarthritis. He also found that infrapatellar fat pad size had a protective role on knee osteoarthritis, while abnormal infrapatellar fat pad quality would be detrimental.

Changhai has collaborated widely with Institutes in Australia (Monash University, Sydney University, University of Melbourne) and overseas (Anhui Medical University, Boston University, University of Nottingham, University of Montreal, Synarc). Many publications have been produced from these collaborations.

One of his PhD students was awarded Emerging Researchers in Ageing (ERA) 2013 Exchange Program ($10,000) by ARC Centre of Excellence in Population Ageing Research in 2013, Young Investigator Award from ASBMR 2014 ($2,000), and Arthritis Australia & State and Territory Affiliate Grant ($50,000); another one was awarded 'Chinese Government Award for Outstanding Self-financed Students Abroad' ($6000) in 2014. His 4 papers won 'Ten of the Best Awards' at the Menzies Research Institute Tasmania in 2014-5.

Current projects

  1. The Vitamin D Effect on Osteoarthritis (VIDEO) Study: a randomised controlled trial
    The VIDEO study is a randomised controlled trial to explore the effects of the vitamin D supplementation on knee structural changes (cartilage and bone) utilizing pioneering MRI techniques and limb muscle strength assessment in osteoarthritis patients. It is being supported by National Health & Medical Research Council (NHMRC ID 605501) and is in conjunction with Monash University in Melbourne.
  2. Inflammatory and Metabolic Mechanisms in Osteoarthritis Study
    This longitudinal study, based on established studies, is to determine the associations between serum levels of adipokines and cytokines and knee structural changes in older adults or OA patients over time.
  3. Osteoarthritis of the Knee Statin (OAKS) Study
    The OAKS study is a multicentre randomised controlled trial examining statin, a new treatment, for knee osteoarthritis. It is supported by the National Health and Medical Research Council (NHMRC ID 1048581).
  4. ZAP 2 trial - A randomised trial of zoledronic acid for osteoarthritis of the knee
    The ZAP 2 trial is a multicentre randomised controlled trial examining zoledronic acid, a new treatment, for knee osteoarthritis. It is supported by the National Health and Medical Research Council (NHMRC ID 1045415).

Changhai has successfully published many papers in peer-reviewed journals. In his career to date, he has published over 200 peer-reviewed manuscripts and 14 book chapters (1992-), including 130 manuscripts written in English. Of those 130 manuscripts, 76 were first-or senior- authored; 15 in the top rheumatology journal 'Annals of the Rheumatic Diseases', ten in the premier rheumatology journal 'Arthritis & Rheumatism', two in the top bone journal 'Journal of Bone & Mineral Research', and three in premier medicine journals (Archives of Internal Medicine, Journal of Internal Medicine, American Journal of Medicine). His publications have had 2854 citations within a short period (>300 each year from 2010, Scopus data, 1/7/2015). He has 17 papers cited > 50 times and 32 cited >30 times (Scopus data).

Changhai has been the recipient of over 20 grants in excess of $4.1 million during his career; with the most recent totalling $1 million to fund a double blinded randomised control trial examining the Vitamin D Effects on Osteoarthritis (VIDEO Study). He has been the CIA on 10 projects including 1 ARC project (FT110100276), 3 NHMRC projects (ID 490006, 490049, 605501) (2006-), been a CI or an AI in 3 NHMRC projects (ID 605512, 1048581, 1045415), and has participated in 2 other NHMRC projects (ID 106910, 302204) on knee osteoarthritis and cartilage research. These projects have paved the way for new and emerging ideas including the development of a method for early osteoarthritis assessment, and the generation of further knowledge about serum 25-(OH)D and its association with knee cartilage health and pain, implying that achieving vitamin D sufficiency may slow cartilage loss and reduce knee pain in knee osteoarthritis.

Osteoarthritis is the most common joint disorder and the most common cause of pain and disability in adults. Vitamin D deficiency affects up to 80% of older adults and is associated with knee pain and knee cartilage loss. The aim of this study is to compare, over a 2- year period, the effects of vitamin D supplementation versus placebo on knee pain and knee structural change (assessed by Magnetic Resonance Imaging, MRI) in patients with symptomatic knee osteoarthritis. Identifying the importance of vitamin D is an important step towards disease prevention and it may be possible to slow disease worsening and delay or prevent subsequent joint replacement.

Description
Osteoarthritis (OA) is one of the most common chronic conditions and shows a progressively increasing incidence in Western countries due to both population ageing and rising rate of obesity. Currently the aetiology of OA is unknown; genetic, environmental and mechanical factors may play important roles. Mi(cro)RNAs are a class of non-coding endogenous RNA molecules, only 18-25 nucleotides in length. Since their discovery in 1993, these molecules have been shown to play critical regulatory roles in a wide range of biological and pathological processes. Investigation of these molecules may improve our understanding of some chronic disorders such as OA. Some studies suggest MicroRNAs may play roles in regulating cartilage development. The aims of this cohort study are to examine whether obesity-related microRNAs are associated with knee cartilage loss in patients with knee OA.

Translational Research in Osteoarthritis: From Epidemiological Studies to Clinical Interventions (2011 - 2015)$765,216
Description
The research will focus on determining mechanisms (eg, inflammatory and metabolic), modifiable risk factors (e.g. obesity, physical inactivity), and potential therapies (e.g. vitamin D supplementation) for knee osteoarthritis (OA), one of the most common and debilitating conditions in the aged population. In Australia, OA affects ~26% of those aged >65 years, and costs the nation at least $2.32 billion in total health expenditure. Currently the aetiology of OA is unknown, and there is no disease-modifying therapy for OA. This research has potential to provide innovative and cost-effective approaches to prevent and slow the progression of OA, and thus reduce the huge burden of this disease from both an individual and societal perspective.

Research Supervision

Changhai is the primary supervisor of 7 PhD students and a co-supervisor of 1 PhD students. He is the group leader of his team at the Menzies Research Institute Tasmania, which consists of 8 CIs, 1 AI, 4 research assistants, 5 PhD students and 6 visiting fellows. He co-supervised 1 Honours, 3 Masters and 3 PhD Students to completion, and tutored Research Methods (MPH, Monash University, 2009-2011). Additionally, he has trained various research students and assistants to become competent in specialised skills such as reading MRI knee images, measuring knee cartilage volume and bone area, and the scoring of cartilage defects in MRI scans.
