Nearly one in ten Australians suffers from osteoarthritis, a chronic condition that affects the joints (e.g. hips, knees, hands, back or ankles).

Associate Professor Dawn Aitken leads a program of research which centres on improving our understanding and management of osteoarthritis. Osteoarthritis is a painful, disabling, and costly condition that affects the joints in our body including the knees, hips and hands.

Dawn is particularly interested in the role of exercise in the prevention and management of osteoarthritis. She works closely with clinicians to help improve the conservative (non-surgical) management of osteoarthritis and has published numerous papers on the relationship between physical activity and osteoarthritis. Her work has been used to develop new physical activity guidelines for people with chronic diseases by the US Department of Health and Human Services and he she has contributed to Australian policy white papers.

Dawn also leads or collaborates on a number of major clinical drug trials aimed at slowing down – or halting – the progression of osteoarthritis. Her team conducted the largest study in the world to examine whether the commonly used osteoporosis medication – zoledronic acid, can be used to treat knee osteoarthritis. The study found that zoledronic acid did not slow cartilage loss or improve knee pain. It provides definitive, high-quality evidence that zoledronic acid should not be used to treat knee osteoarthritis. This work was published in JAMA (The Journal of the American Medical Association) in April 2020. She has also worked on turmeric and fish oil for knee osteoarthritis, adalimumab for hand osteoarthritis, and zoledronic acid and denosumab for back pain.

Associate Professor Dawn Aitken is Head of the Musculoskeletal Health and Disease Research Group at the Menzies Institute for Medical Research. She is currently supported by a NHMRC/MRFF Career Development Fellowship. She completed her PhD in 2011 at the University of Tasmania examining early changes that occur in knee osteoarthritis. Before this she completed a Bachelor of Science, with Honours in Human Kinetics, at the University of Ottawa, Canada.

The primary aim of this study is to work with GPs to co-design interventions to increase referral rates of people with osteoarthritis to GLA:D / exercise therapy. The co-designed intervention will be piloted and implemented in a future cluster-randomised controlled trial (RCT) to increase GP referral rates to exercise therapy. ACTIVE KNEECAPS! tArgeted effeCTIVE treatments for adolescent KNEECAP pain (2023)$2,043,046
Description
More than one-quarter of adolescents live with regular, debilitating kneecap pain each year. This leads to disability during daily activities, lower quality of life, and high rates of withdrawal from sport, physical activity and social interactions. Kneecap pain in adolescence is not a self-limiting disease. 75% of adolescents have persistent PFP for up to 20 years, setting the scene for a lifetime of knee pain and associated health implications, such as arthritis. Adolescents who present to primary care for their kneecap pain are met by clinicians who do not have the evidence to make informed treatment decisions. Best-practice exercise treatments for adults with kneecap pain are only half as effective for adolescents, and do not address their unique needs. Our vision is to address this knowledge gap by providing stakeholders with co-designed, evidence-based treatments that are acceptable, safe and effective for adolescents with kneecap pain. ACTIVE KNEECAPs will determine if (i) shoe inserts or (ii) targeted physiotherapy are more effective and cost-effective over 1 year than (iii) providing simple advice. Key outcomes are pain severity, function in daily and sporting activities, quality of life, and physical activity. Leveraging our multidisciplinary team's existing knowledge translation platforms and strong relationships with key stakeholders, we will develop tools to facilitate uptake of evidence-based treatments in primary care. Through the combination of a high-quality randomised controlled trial, embedded knowledge translation activities, and an expert multidisciplinary team of investigators and clinical partners, our project will result in tangible beneficial changes to health practice in primary care, better health outcomes for adolescents with kneecap pain, and increased health efficiency.

Walk with Ease Australia (2023)$591,280
Description
Osteoarthritis (OA) is a serious and debilitating condition that affects over 3 million Australians and yet there is still no cure. In 2018, the National Osteoarthritis Strategy identified seven priority areas throughout a person's journey with OA, from prevention, early treatment to end-stage management and joint replacement. A key goal of the Strategy was to improve the uptake of physical activity and exercise by Australians diagnosed with, or at risk of developing OA. Currently, 90% of people with OA do not meet physical activity guidelines. This grant proposal will test an innovative public health approach to address risk factors associated with the prevalence and persistence of OA in Australia and implement these approaches to improve the quality and cost-effectiveness of preventative healthcare interventions. We propose to develop, evaluate, and implement a sustainable, community-driven walking program for people with OA. The Walk with Ease program is a 6-week walking program developed by the Arthritis Foundation in the USA that is proven to reduce pain and improve function in people with moderate to severe levels of pain. This program will be adapted to the Australian setting by partnering with state-based arthritis consumer groups who will co-design the delivery of the program that will have a focus on engagement, adherence, and long-term sustainability.

Providing high-value care for osteoarthritis patients at the Royal Hobart Hospital (RHH) (2020)$12,940
Description
This prospective, observational study will recruit patients undergoing primary elective hip or knee joint replacement at study sites in Tasmania. Participants will complete patient reported outcome measure (PROMs) surveys and have accelerometry data collected preoperatively and at 6 weeks, 12 weeks, 6 months and 12 months post-operatively. The trajectory of their recovery is measured both by their subjective PROMs responses and objective movement data captured by accelerometry to provide insights into expected performance changes and evaluate PROMs characteristics.The project aims to quantify physical performance changes that occur in patients following hip or knee arthroplasty and compare the relationship between this objective data and subjective PROMs.

Knee osteoarthritis (OA) is common, but the current treatments are poor. Almost 60% of people with knee OA have joint inflammation. Patients with inflammation are more likely to experience joint pain and rapid joint destruction. We propose that treating patients with inflammatory knee OA with the anti-inflammatory drug, diacerein, will reduce pain and joint damage.

A randomised controlled trial evaluating community walking for knee osteoarthritis (2018)$30,000
Description
This project will implement a community walking group for patients with knee osteoarthritis. This pilot study aims to evaluate the feasibility of recruitment, retention, randomisation and the implementation of a RCT aimed to examine the effect walking has on knee structure (assessed using advanced magnetic resonance imaging (MRI) technology). We will recruit 48 participants with knee osteoarthritis who will be randomised to either a 6-month community walking group or a usual care (control) group. Participants in the walking group will train 3 days per week for 1 hour each day. Walking will be performed at a moderate intensity (50-70% of each participant's heart rate reserve). This frequency/intensity is recommended for Australian adults (Australian National Physical Activity Guidelines) and for adults with knee osteoarthritis (Arthritis Australia). The structure of our program is based on a group walking protocol which showed improvement in pain in knee osteoarthritis patients. The walking program will be supervised by accredited physiotherapists with experience in prescribing exercise for knee osteoarthritis patients. Each group walking session will consist of:*10 min warm up consisting of slow walking and calisthenics: arm circles, trunk rotation, shoulder and chest stretches, and side stretch.*40 min group walking phase where participants are encouraged to walk at 50-70% of their heart rate reserve as determined from an exercise treadmill test at the start of the study. *10 min cool down consisting of slow walking and flexibility exercises. Participants randomised to usual care will receive usual standard care from their medical practitioner, as well as generic information about knee osteoarthritis. This will the control group, and this is required in order to examine the true effect walking has on knee health.

Improving musculoskeletal pain by matching the right treatment with the right patient - Medical Research Future Fund (MRFF) Next Generation Clinical Researchers Program - Career Development Fellowship (2018)$82,000
Description
During the course of my CDF Fellowship I am leading five innovative research projects that aim to provide clinically translatable evidence offering new effective treatments that improve outcomes for patients with musculoskeletal diseases. Four out of the five research projects are funded either by NHMRC or industry. I am applying for the ASBMR Rising Star award to fund part of the fifth project in my research program: Do osteoporosis therapies work for back pain? We will recruit 125 patients with chronic low back pain patients. Men and women will be recruited if they are >34 years of age, have at least moderate back pain (≥40/100 on 100mm VAS) for at least 3 months, and have type 1, type 2 or mixed type Modic changes on MRI (same inclusion criteria in MODIC1). Exclusion criteria include: Neuropathic pain component is unclear/definite (painDETECT questionnaire13 score ≥12, based on MODIC1 findings); Diagnosis of a centrally mediated pain phenotype (e.g. fibromyalgia); Prior use of denosumab or bisphosphonates; Serum 25-hydroxyvitamin D concentrations < 40 nmol/L; Pregnancy or breastfeeding; Prior cancer diagnosis; Poor dental fitness; Use of high doses of opiate pain medication (equivalent to oxycontin ≥80mg/day); Leg pain is of greater severity than back pain; Back surgery within the last 5 years.Eligible participants will receive one subcutaneous injection of denosumab (60mg in 1ml saline) or 1ml saline every 6 months for 24 months (0, 6, 12, 18 months).

Unlocking the potential of a novel setting to promote physical activity (PA) among patients with Knee Osteoarthritis (KOA) - parkrun (2018 - 2019)$31,313
Description
Participating in PA reduces pain and improves function in people with KOA. Despite this, people with KOA are alarmingly inactive. It is, therefore, of high priority to identify effective, low-cost and sustainable PA promotion strategies for KOA. parkrun is an international movement of weekly 5 km walk/run events held in public spaces. Parkrun has a broad appeal to various population groups including non-athletes, however no previous reports had evaluated parkrun as a promising PA promotion setting for KOA. Our study will determine the feasibility, acceptability and safety of parkrun as a novel strategy to promote PA for people with KOA.

Assessing the Australian osteoarthritis landscape - GPs' surgeons' and patients attitudes towards and understandings of osteoarthritis (2018)$24,824
Description
Osteoarthritis is a leading cause of pain and disability among older people and there is currently no cure. In recent years, there has been increasing concern about the discrepancies between care provided by health professionals and recommendations in osteoarthritis clinical practice guideline. For instance, in Australia, referral to a surgeon is five times more common than referral to allied health professionals that could help with conservative management. Furthermore, patients are ambivalent towards conservative management such as exercise, thinking it will be ineffective and may even cause harm to their already worn joints. The disconnect between practice guideline and care for patients with osteoarthritis limits optimal care. Therefore, this study aims to explore the attitudes towards and understanding of osteoarthritis held by Australian GPs, surgeons and patients with knee osteoarthritis. The overarching goal is to gather information needed to develop Australian-specific education and behaviour change programs for GPs, surgeons and osteoarthritis patients

Improving outcomes in patients with knee osteoarthritis: identifying pain phenotypes with relationships to long-term health outcomes (2018)$9,669
Description
Knee osteoarthritis is a highly heterogeneous condition characterised by a complex and multifactorial nature. The pain experience in knee osteoarthritis is similarly individualized and complex; some patients may present with pain that appears attributable to classic signs of joint damage, while others may present with pain due to psychological distress or central mechanisms. However, whether these variables (or interactions between variables) are representative of different pain phenotypes in knee osteoarthritis and whether patients with distinct pain phenotypes have different risk for long-term health outcomes are unknown. This project aims to identify the pain phenotypes in OA patients and compare the risk of long-term health outcomes between different pain phenotypes. This study will help clinicians better prioritize and target interventions to individual patients and will lead to better outcomes and fewer treatment complications currently associates with pharmaceutical and surgical interventions that are widely used to treat knee osteoarthritis.

A randomised trial of zoledronic acid for osteoarthritis of the knee (2013 - 2015)$954,596

Osteoarthritis (OA) is the most common form of arthritis and is increasing markedly due to an ageing population. Despite its large disease burden, there are currently no approved disease-modifying drugs available which modify structural progression of OA. The aim of this study is to compare zoledronic acid treatment to placebo on knee structural change and knee pain over two years. It is hypothesised that zoledronic acid will reduce cartilage loss, knee pain, and bone marrow lesion size.
