James is Professor of Medical Research with expertise in cardiovascular disease and blood pressure. He is Deputy Director and heads the Blood Pressure Research Group at the Menzies Institute for Medical Research.

James’s research program is dedicated to reducing blood pressure related premature death and disability. His research and translational activity to improve the accuracy of blood pressure devices and blood pressure measurement has had global reach with impact on awareness, policy and practice guidelines. He leads an initiative of the Lancet Commission on Hypertension Group to increase the global availability of blood pressure devices that have been clinically validated for accuracy. This work includes translation of research knowledge to influence regulatory pathways in the Americas as an advisor to the Pan American Health Organization.

He has published more than 300 papers, including as lead author on national and international consensus statements on blood pressure accuracy and measurement. James has extensive research collaborators in Australia and internationally, and is an investigator on research grant funding over $15 million.

Saving lives with better blood pressure management (2023 - 2027)$655,150
Description
Cuff measured blood pressure (BP) is one of the most important tests in all clinical medicine. However, cuff BP is not accuratein many individuals, and this can increase the likelihood of avoidable cardiovascular events (heart attack, stroke, kidneyfailure) through incorrect diagnosis and inappropriate medical care. My research to date has identified problems with theaccuracy of standard cuff BP devices used every day in clinical practice. Problems with accuracy are more apparent in olderpeople and those with greater arterial stiffness. My clinical BP physiology work has also identified potential solutions to helpachieve better, more accurate cuff BP measurements.Informed by my previous work, the proposed research program will include a series of studies using various data sources toachieve my vision of better BP measurement to minimise the burden of preventable cardiovascular disease, including:1. BP TYPE study will develop, validate and assess the clinical effectiveness of a method for accurate measurement of cuff BPusing machine learning analysis of BP wave shapes.2. INSPECT consortium studies will examine BP measurement differences between cuff BP and invasive BP and factors thatinfluence the accurate measurement of BP.3. TAVI-BP study will generate foundational knowledge on the accuracy of cuff BP and BP wave shapes in patients with aorticstenosis who are undergoing trans-aortic valve implantation.4. AIM-BP collaborative. I am co-lead of this international group and we will perform translationally focused research thataims to promote the use of best practice BP measurement methods and devices that are currently available.This program of research will comprehensively address the problem of inaccurate BP measurement and contribute to betterblood pressure measurement that will minimise the burden of preventable cardiovascular disease in Australia and globally.

Description
The study aims to identify cardiac complications of cancer treatment (chemotherapy) and find exercise and medical treatment that will improve these complications. Some cancer treatments lead to a risk of heart muscle damage. If this process is left unchecked, it can sometimes cause heart failure, especially as patients grow older and develop other risks for heart failure. In this project an ultrasound of the heart (echocardiogram) will be taken in order to measure cardiac status. We are trying to establish whether information from simple tests of the heart could guide treatment to protect patients from developing heart failure.

Assessing the clinical value of exercise stress echocardiography: an EXERcise stress Test collaboratION (EXERTION) study (2022)$24,951

The primary objective of this study is to determine the clinical value of ESE in the prediction of CAD compared to an exercise stress ECG test. Specifically, we aim to determine the association of ESE test result with 1) probability of subsequent coronary angiogram procedure, and 2) the risk of CAD (as confirmed by coronary angiography) in comparison to that of an age- and sex-matched group of participants who completed a simple exercise stress ECG test.

Risk-guided strategy for reducing readmission for Acute Decompensated Heart Failure (RiskHELP) (2019)$107,400
Description
A prospective RCT with 320 HF randomised patients admitted with Acute Decompensated Heart Failure (ADHF) confirmed by diagnosis from RHH. Eligible patients (based on study inclusion exclusion criteria), identified by applying the TAS-Help (Study based at Menzies) algorithm as being at high risk for hospital readmission and/or death, will be randomised into two groups of 160 patients each arm. The control arm will receive usual care while the intervention arm will receive DMP-Plus. Patients with ADHF will be screened for a period of 24 months or until targeted sample size is met.

The Royal Hobart Hospital's innovative Rapid Access Chest Pain Clinic (RACPC): A health economic evaluation of patient, RHH and community benefits (2019)$24,952
Description
The overall aim of this project is to conduct a comprehensive health economic evaluation of the RHH's cutting-edge Rapid Access Chest Pain Clinic (RACPC) patient care model, compared to the usual care General Cardiology (GCC) clinic model for RHH patients with newly presented chest pain. The RACPC commenced in June 2014 and has realised positive clinical outcomes for its patients. Our comprehensive health economic investigation will build on the clinical work to systematically evaluate the health economic benefits of this innovative model's patient pathways for people who present to the RHH's Emergency Department (approximately 70%) or by referral from a primary care physician (approximately 30%) with new onset chest pain.

Prevalence and cardiovascular risk associated with exaggerated exercise blood pressure among people with type 2 diabetes (2019)$59,958
Description
People with type 2 diabetes are at higher risk of death and disability from cardiovascular disease such as heart attack and stroke, for which the main contributing factor is high blood pressure. Since high blood pressure can be treated to lower the risk of cardiovascular disease, it is important that people with type 2 diabetes have their blood pressure correctly diagnosed. Pilot data from the investigator team have shown that people with type 2 diabetes have much greater propensity to have an exaggerated (high) blood pressure response to exercise, and this is a cardinal sign of abnormal (high) blood pressure that is missed using standard measurements under resting conditions. This has major implications for the potential of exercise stress test data as a means to identify people at high risk otherwise overlooked. These pilot observations will be confirmed in this current proposal using a nationally representative dataset of 125,000 people, with linkage to cardiovascular hospitalisations and death, in the largest analysis of its kind. Information from this study will inform clinical exercise stress test guidelines to improve care and reduce cardiovascular events among people with type 2 diabetes.

Improved cardiovascular disease health service delivery in Australian (IDEAL study) (2019 - 2022)$1,071,230
Description
Cardiovascular disease (CVD) has the highest burden of global disease and highest level of health care expenditure in Australia. Most of the management of CVD is performed in general practice, where the recommended strategy is to determine absolute CVD risk based on multiple factors (e.g. cholesterol, blood pressure, diabetes, smoking). Australian primary care doctors are not yet adopting guideline recommendations, with as few as 30% using risk calculators, and this leads to people remaining at high risk for otherwise avoidable CVD events. All partners in this grant are working together to rectify this evidence-practice gap, using a new, yet simple and widely implementable, digital health service that has been developed by researchers, industry, health service providers and policy makers.The concept involves absolute CVD risk being measured at pathology services in patients referred for blood lipid assessment. Patients have blood pressure measured and CVD risk factors collated electronically by pathology services using an automated, high throughput approach, with absolute CVD risk results conveyed to general practitioners via an established pathology reporting system. Abnormal results are highlighted and advice provided regarding appropriate, guideline-directed action. Significant development work that confirms proof-of-concept has been completed. This program will refine and test the clinical value of the new health service method upon establishment statewide across Tasmanian pathology services. In an Australian first, the system is designed for research infusion, with all information data-based (through UTAS) and serving as a platform for rapid assessment of clinical value by linkage to health outcomes.Ultimately, the new service is expected to improve CVD outcomes, reduce health care costs and improve doctor and patient experiences. Importantly, the model is transferable to other national and international settings.

Clinical importance of neural baroreflex sensitivity in the Paris Prospective Study 3 (2016)$7,500
Description
Appropriate regulation of blood pressure via baroreflex alterations to heart rate and peripheral arteries is critical for cardiovascular health, yet little is known about the neural components controlling the baroreflex (neural baroreflex sensitivity) and its relation to blood pressure. The Paris Prospective Study 3 is an INSERM supported study among 10,000 French subjects, initiated to identify novel heart rate and carotid artery stiffness predictors of sudden death from cardiovascular disease. All subjects have had extensive testing, including continuously recorded echotracking of the carotid artery, neural baroreflex sensitivity, central and brachial blood pressure. All subjects have also undergone exercise stress testing and there is a 10 year follow up period currently underway. A series of cross sectional analyses will be undertaken to address several novel scientific questions related to neural baroreceptor sensitivity and blood pressure under resting and exercise conditions. Principal research questions are to determine:1. The independent relationship between neural baroreceptor sensitivity and central blood pressure (systolic and central pulse pressure)2. The relationship of the neural baroreceptor sensitivity -central blood pressure interaction with blood pressure variability.3. The influence of neural baroreceptor sensitivity on abnormal blood pressure responses to exercise.

Detection and management of subclinical pulmonary vascular disease (2015 - 2017)$110,000
Description
Pulmonary hypertension is a debilitation condition, with a 3 year survival rate of only 50%. Tasmania currently has a higher prevalance and incidence of pulmonary hypertension related to connective tissue disease than other regions in Australian. There is a large disease registry that for over 10 years has been aggressively screening and treating patients at only mild onset of this condition. This involves treating patients with vasodilator therapies. This is believed to improve exercise capacity, mobility, vascular spasm, right heart function and overall quality of life. It is not yet clear if the strategy of early treatment improves hard clinical outcomes and whether this is cost effective. This is important as the cost of maintaining these patients on treatments over a long term period need to be justified. We aim to look at a wide range of aspects of this disease process. Initial, at the microvascular level to determine how this affects functional decline in patients, to the function of the right ventricle and how new research methods are able to detect changes earlier. We will also asses whether timing of intervention impacts upon health status, and what the cost efficiency of early intervention regarding disease outcomes and quality of life.

Cognition and Type 2 Diabetes - A pilot randomised controlled trial (RCT) exercise (2014)$60,000
Description
The aim of this pilot RCT is to examine the feasibility and optimal sample size for a definitive RCT of the efficacy of a structured exercise program in improving or preserving brain health in people aged 50-75 years with Type 2 Diabetes mellitus (T2DM). Participants (n=50) will be randomised into either a 6 months light stretching/balance group (control) or a 6 months aerobic/resistance training group (intervention). The primary outcomes will be brain imaging biomarkers - MRI brain volumes and white matter integrity and measures of cognitive function. Secondary outcomes will include ambulatory central haemodynamics , microvascular health (assessed by central haemodynamic measurements, retinal photography and albuminuria, fitness and strength.

Microvascular dysfunction in adult offspring of type 2 diabetics (2014)$20,294
Description
Offspring from type 2 diabetics (T2D) are at much greater risk of developing diabetes than a person with no family history. The mechanism for this increase in risk is not known. This project will determine whether microvascular dysfunction in skeletal muscle and skin of first degree relatives with T2D occurs before development of insulin resistance. We will recruit people with and without family history of T2D. It will measure microvascular actions (using ultrasound) in these people before and after a mixed meal challenge. The outcomes of this study will determine whether monitoring microvasculature responses in muscle is an important 'early' strategy for assessing risk that might inform patient treatment to prevent or delay progression to insulin resistance or T2D.

Assessing the validity on non-invasive methods to estimate central blood pressure (BP) (2014)$6,983
Description
The prognostic and clinical importance of central BP beyond conventional brachial blood pressure (BP) measurement has recently been demonstrated. However, assessment of central BP is largely confined to a research setting, because current, non-invasive methods to assess central BP are impractical for routine clinical use. The objective of this research program is to determine (and refine where necessary) the accuracy (validity) of two relatively new upper-arm 'cuff' devices to estimate central BP. In doing so, it is envisioned that our results may enable a wider clinical application of central BP, of which measurement may result in a more accurate assessment of CV risk related to high BP.

The role of central haemodynamics in Type 2 Diabetes Mellitus-related brain disease (2014 - 2018)$863,538
Description
Type-2 Diabetes Mellitus (T2DM) is associated with a 2-3 fold risk of incident dementia. The underlying mechanisms for this increased risk are yet to be clearly define. T2DM is a strong risk factor for central arterial stiffening which eads to abnormal central aortic haemodynamics. This may lead to the transmission of harmful pulsatile pressures to the microcirculation of vulnerable organs such as the brain, potentially leading to tissue damage and dysfunction. Clarifying whether this is the case opens the door to interventions targeted at improving central haemodynamics to reduce the impact of T2DM on the brain.

Tasmanian Study of Echocardiographic Detection of Left Ventricular Dysfunction (2014)$53,679

HF is increasing in Australia but treatment in the early stages is able to prevent or delay its progression. The problem is how to recognise the early phase. This project will: implement a screening tool for heart failure; deliver an effective heart failure screening outreach service; and implement an effective communication process to Primary Care and referral to Cardiology. A database will be established to support follow up and personalised care of at risk subjects. This study expects to prevent 24 people developing heart failure.

Cardiometabolic benefits of exercise training in type 2 diabetes independent of weight loss (2014)$50,735
Description
This project will recruit an overweight type 2 diabetes cohort, medically assessed for an exercise program. It aims to recruit some with family history of type 2 diabetes and some without. The cohort with exercise test before and after 6 weeks of the exercise program, with type 2 medications discontinued before test. The aim is to show that heart health will not be as good with family history of type 2 and that exercise will improve heart health but will be of more benefit to those with a family history than those without.

For over 100 years doctors have measured blood pressure (BP) using a cuff at the upper arm. However, this method has many drawbacks that can result in inappropriate treatment decisions. New technology has emerged to measure central BP, which allows a more accurate BP assessment and may improve patient care. This study will test the clinical value of this new measure of central BP. Findings are expected to lead to a change in the way that people with high BP are diagnosed and managed by doctors.
