Dr Laura Laslett is a musculoskeletal epidemiologist and NHMRC Early Career Fellow at the Menzies Institute for Medical Research. Her research focuses on osteoarthritic pain.

Biography

Dr Laslett moved to Hobart in 2009 to complete a PhD on determinants, correlates, modifiers of musculoskeletal pain at the Menzies Institute for Medical Research, University of Tasmania.  She commenced postdoctoral work in 2013 as an Arthritis Australia Heald Fellow, and then as an NHMRC Early Career Fellow in 2014.

Prior to moving to Hobart, Laura worked as a research officer at the Discipline of Medicine at The University of Adelaide (Modbury Hospital) for 6 years, and as an epidemiologist for SA Health for 2 years.  She also completed a 3 month placement at the Division of Rheumatic and Musculoskeletal Disease at the University of Leeds (Leeds UK) in 2012, funded by an Osteoarthritis Research Society International Young Investigator Scholarship. She has worked with several research groups (Adelaide, Hobart and Leeds (UK)) and has expertise in the conduct and analysis of intervention studies, including 4 clinical trials and an educational intervention for osteoporosis.

Research Themes

Despite the large impact of osteoarthritis on the community there are currently no treatments that are approved to change the disease course, and current treatments focus on reducing pain and improving function – the major symptoms of osteoarthritis. However, there is a significant gap between the level of pain relief patients want, and what they actually get from treatments available today.

Dr Laslett's research has focussed on targeting OA treatments to people with OA with different sources of pain. One study from Dr Laslett's PhD demonstrated the first proof of principle for using an osteoporosis drug and demonstrating that it had effects in a key structural feature of OA (bone marrow lesions), and reduced knee pain in people with knee OA (ZAP1 trial). A second study from her PhD demonstrated that a natural therapy was effective in reducing pain in people with knee OA.

She is currently running the Modic trial, in people with a similar structural feature to bone marrow lesions (Modic changes), and back pain. This study is testing two different osteoporosis treatments to assess whether they are effective in reducing back pain and Modic changes. She is currently involved with several other clinical trials for treating pain in the knees and hands.

Dr Laslett's research aligns to the University's research theme of Better Health. Her research interests include understanding sources of pain in osteoarthritis, and testing interventions that have the potential to treat musculoskeletal pain, and to change disease course of osteoarthritis and other musculoskeletal conditions.

Dr Laslett is also involved in the analysis of observational data to answer important clinical questions on pain and how it might be treated. She is working with Sarah Mattap (Honours student) to determine whether pathological changes at the tibial enthesis is associated with pain and other structural factors normally associated with knee pain. She collaborates with colleagues at the University of Leeds on analysis of data from a large US-based cohort to assess links between lower limb and upper limb pain, and collaborates with researchers at La Trobe University on non-structural factors associated with existing and new foot pain.

Collaboration

Laura currently collaborates with researchers at the University of Leeds and imorphics (Manchester). This work involves assessing predictors of newly painful joints, and assessing the effect of treatment with bone active medication changes the shape of knee joints.

She also collaborates with researchers at Monash University, Sydney University, the Queen Elizabeth Hospital, and the University of Western Australia on clinical trials in knee pain; and with researchers at La Trobe University on factors associated with foot pain.

Providing high-value care for osteoarthritis patients at the Royal Hobart Hospital (RHH) (2020)$12,940
Description
This prospective, observational study will recruit patients undergoing primary elective hip or knee joint replacement at study sites in Tasmania. Participants will complete patient reported outcome measure (PROMs) surveys and have accelerometry data collected preoperatively and at 6 weeks, 12 weeks, 6 months and 12 months post-operatively. The trajectory of their recovery is measured both by their subjective PROMs responses and objective movement data captured by accelerometry to provide insights into expected performance changes and evaluate PROMs characteristics.The project aims to quantify physical performance changes that occur in patients following hip or knee arthroplasty and compare the relationship between this objective data and subjective PROMs.

A randomised trial of krill oil for osteoarthritis of the knee (2019)$100,000
Description
This project is additional funding to NHMRC Grant J23281Knee osteoarthritis (OA) is a significant and increasing cause of pain and disability in the aging Australian population, but interventions aimed at structural modification have been disappointing. Pilot data shows that krill oil reduces knee pain and systemic inflammation after 30 days, suggesting that an approach targeted at specific mechanisms of disease is effective in reducing painful OA. However, this pilot study was short, did not collect any data on structural factors, and included patients with a variety of disorders.We aim to compare, using a randomised, placebo-controlled double-blind design over six months, the efficacy of krill oil vs placebo in the treatment of osteoarthritis (OA) of the knee in 260 patients with clinical knee OA, significant knee pain and effusions, both with regard to control of symptoms and localised inflammation.Key hypothesis: Krill oil decreases pain (assessed by 100mm VAS) by 10mm more than identical placebo over 24 weeks in patients with clinical knee OA, significant knee pain and effusions.Secondary hypotheses: Krill oil improves knee pain over 4, 8, 12, 16, and 20 weeks in patients with clinical knee OA.Krill oil decreases effusion size by 20% over 24 weeks in patients with symptomatic osteoarthritis of the knee and knee effusion.If krill oil can both improve symptoms and decrease effusion in OA it has the potential to slow progression to joint replacement. The proposed study represents an innovative approach to this and lends itself to easy implementation as krill oil is already available over-the-counter in Australian pharmacies.

Assessing the Australian osteoarthritis landscape - GPs' surgeons' and patients attitudes towards and understandings of osteoarthritis (2018)$24,824
Description
Osteoarthritis is a leading cause of pain and disability among older people and there is currently no cure. In recent years, there has been increasing concern about the discrepancies between care provided by health professionals and recommendations in osteoarthritis clinical practice guideline. For instance, in Australia, referral to a surgeon is five times more common than referral to allied health professionals that could help with conservative management. Furthermore, patients are ambivalent towards conservative management such as exercise, thinking it will be ineffective and may even cause harm to their already worn joints. The disconnect between practice guideline and care for patients with osteoarthritis limits optimal care. Therefore, this study aims to explore the attitudes towards and understanding of osteoarthritis held by Australian GPs, surgeons and patients with knee osteoarthritis. The overarching goal is to gather information needed to develop Australian-specific education and behaviour change programs for GPs, surgeons and osteoarthritis patients

Is the use of thermocyclopad effective for knee pain? (2016)$50,336
Description
Clinical trial to assess the efficacy daily use of the Niagara Thermo Cyclo Pad unit on the heat and vibrate settings, or the heat and vibrate settings plus use of the hand held unit will reduce knee pain, stiffness and symptoms over four weeks compared to the sham pad, for knee OA pain, function and stiffness.

Does either zoledronic acid or denosumab change back pain and morphology of Modic changes: a randomised open label pilot study (2014)$30,000
Description
This research aims to determine whether two different bone active medications (zoledronic acid 5mg; denosumab 60mg) will modify back pain and structural abnormalities over six months in people with severe back pain and certain changes on magnetic resonance imaging (MRI). This will allow us to decide whether these medications have a large enough effect to warrant further clinical study.
