Diseases of the bones and joints – like osteoporosis and osteoarthritis – are normally thought of as being issues of ageing and elderly people. Professor Tania Winzenberg’s research focuses, however, on what can be done in early life to maintain bone health and provide protection against these musculoskeletal conditions later on.

‘It is increasingly apparent that how we look after our bones and joints, and in fact our health in general, in childhood and young adult life has a profound impact on diseases that appear in later life,’ Professor Winzenberg says.

For example, research shows that people with poor bone health in early life are at a higher risk of fractures later on. Among women, having less strong lower limb muscles in early adult life predicts poorer balance in middle age, which is a risk factor for falls. Similarly, by using MRI scans, researchers are able to detect changes associated with osteoarthritis at younger and younger ages.

‘The key message is that staying healthy in older life means looking after yourself throughout your life.’

One of Professor Winzenberg’s areas of expertise is conducting systematic reviews and data synthesis. This is critically important to maximise the quality of evidence used to prepare clinical guidelines, as well as determining where there are evidence gaps that need to be targeted by future research.

Professor Winzenberg has developed clinical guidelines, particularly in the areas of bone health and disease prevention, with the Royal Australian College of General Practitioners for the RACGP’s Guidelines for preventive activities in general practice (the so-called ‘Red Book’), which is the main guide used by Australian GPs to provide preventive care at every life stage.

In her research, she avoids thinking about single diseases in isolation. ‘The key risk factors for a range of diseases are often the same, particularly lack of physical activity, smoking, poor nutrition, alcohol and other lifestyle factors. Therefore, lessons learned from one disease can often be applied in another setting.’

With her background in general practice, Professor Winzenberg is also aware that older patients in particular often have multiple chronic conditions, known as multimorbidity. A focus of her work is to take research and clinical guidelines on single diseases and adapt them to reflect multimorbidity.

With her expertise and wide experience in conducting systematic reviews, Professor Winzenberg is highly sought after as a consultant. She has undertaken a range of projects for organisations such as St Luke’s Health, the Heart Foundation, the National Health and Medical Research Council, and Primary Health Tasmania. These projects include the development of clinical guidelines, developing and evaluating delivery of health services, and literature reviews on topics as varied as the better coordination of health care and the ethical use of animals in research.

In her work, Professor Winzenberg collaborates extensively with other researchers at the Menzies Institute.

‘There are a lot of commonalities between my work and, for example, work here on health economics, nutrition, and primary care. The ease of working with a range of other groups is one of the real strengths of working at Menzies.’

Prof Winzenberg is a Senior Research Fellow-General Practice. She was recently awarded a prestigious and highly competitive NHMRC/PHCRED Career Development Fellowship ($397,000 2012-15). In 2011, she was one of only 8 primary care researchers globally selected to undertake the International Primary Care Research Leadership Programme at Oxford University, 2011-13. Her competitive research funding totals nearly $3 million. She has published in top international journals including the BMJ, the British Journal of General Practice and a chapter 'Calcium and Other Nutrients During Growth' for the international textbook 'Primer on the Metabolic Bone Diseases and Disorders of Mineral Metabolism', the major textbook of the American Society of Bone and Mineral Research (ASBMR).

She is a member of the Australian and New Zealand Bone and Mineral Society (ANZBMS), the ASBMR and the Australian Association of Academic Primary Care and is a fellow of the RACGP. She is an invited member of the Therapeutics, the Professional Affairs and the Research Committees of the ANZBMS. Her contributions to the RACGP are numerous, including as Deputy Chair, National Standing Committee Research, chairing the RACGP's Australian Family Physician Award Selection Committee and serving on the RACGP National Research Awards Committee.

She is osteoporosis editor, Cochrane Musculoskeletal Group (Cochrane Database of Systematic Reviews), is on the editorial board of the Australian Family Physician, and is an associate editor of BMC Family Practice. She is founder and co-convenor of the Tasmanian Physiotherapy Research Group.

Prof Winzenberg's consistent invited roles in conference chairing, organising and judging reflect her strong profile.

Targeted care for chronic plantar heel pain: Shockwave therapy for bone marrow lesions (the BALSA trial) (2024)$25,000
Description
A novel finding from our previous research is that 50% of people with chronic plantar heel pain (CPHP) have a bone marrow lesion (BML)- high signal on imaging within the heel bone that is associated with having CPHP in a dose-responsive manner, and when they shrink over time, with improved pain and function outcomes. Our research proposes to target these lesions with a type of therapy known as shockwave (radial extracorporeal shockwave therapy/ ESWT). This therapy has been shown to shrink BMLs, however it has never been tested specifically in a subgroup of BML-positive CPHP participants. In this pilot phase of the full trial, 44 CPHP participants with a BML will be randomised into receiving 3 sessions of shockwave one week apart, or a sham shockwave treatment. Our primary outcome of interest will be foot pain at 3 months, with secondary outcomes of function, quality of life and BML size at 3 months. This research is important as it will help inform the value of targeted care in a previously unrecognised CPHP subgroup, contributing to our understanding of the role of bone in CPHP.

Putting people first: using stories of lived experience to improve health care for Tasmanians who frequently use healthcare services. (2024 - 2025)$61,911
Description
A small proportion of people impose a disproportionately high burden on the healthcare system, called high healthcare service utilisation (HSU). These people have complex care needs, often having multiple chronic conditions. Interventions aimed at improving outcomes for people with HSU have had limited effectiveness. A major reason for this is a lack of understanding of how people with different forms of HSU differ in their characteristics and potential needs. For example, people who frequently use hospital emergency departments (EDs) are likely to have different health and/or social problems than those who require frequent hospital admissions and so require different care delivered in different ways.This project aims to understand the issues driving HSU across Tasmania. We will undertake semi-structured interviews with a purposive sample of Tasmanias with lived experience of HSU across the three regions (north, north-west, and south) to identify the issues specific to each region and / or type of HSU.

In late 2020 the Australian Government agreed to provide funding of $2M to initiate an innovative service model of care for Tasmanians with complex chronic conditions. This service, called Healthcare Connect North (HCN) will commence in Launceston, Tasmania in mid-2022 and is funded for 2 years from commencement. It will provide highly responsive and effective care to a cohort of people who consume a major portion of hospital and emergency department (ED) resources. This project will evaluate HCN, aiming to generate a high level of evidence addressing its effectiveness, acceptability, feasibility, and sustainability. It will comprehensively evaluate the model according to the established RE-AIM and Quadruple Aim evaluation frameworks, including its effectiveness and cost-effectiveness for the primary outcomes of reducing hospital admissions and emergency department visits.

A small proportion of people impose a disproportionately high burden on the healthcare system, known as having high healthcare service utilisation (HSU). Contributing to this is a combination of medical and social factors that makes it difficult for them to access the type of health care they need. To date, interventions aimed at improving outcomes for people with HSU have had limited effectiveness. A major reason for this is a lack of understanding of how people with different forms of HSU differ in their characteristics and potential needs. For example, people who frequently use hospital emergency departments are likely to have different health and/or social problems than those who require frequent hospital admissions and so require different care delivered in different ways. To deliver the right health care at the right time in the right place, we must first understand the needs of the people who need that care, but unfortunately, there is little research addressing this important evidence gap. In this study, we will use information from routinely collected public hospital admissions and emergency department presentations to characterise people with different forms of HSU, and the patterns of how they use healthcare services over time. Our goal is to generate information to guide the selection and/or development of future primary care/general practice interventions that will improve health outcomes for this vulnerable group of patients.

Knee osteoarthritis (OA) is common, but the current treatments are poor. Almost 60% of people with knee OA have joint inflammation. Patients with inflammation are more likely to experience joint pain and rapid joint destruction. We propose that treating patients with inflammatory knee OA with the anti-inflammatory drug, diacerein, will reduce pain and joint damage.

Unlocking the potential of a novel setting to promote physical activity (PA) among patients with Knee Osteoarthritis (KOA) - parkrun (2018 - 2019)$31,313
Description
Participating in PA reduces pain and improves function in people with KOA. Despite this, people with KOA are alarmingly inactive. It is, therefore, of high priority to identify effective, low-cost and sustainable PA promotion strategies for KOA. parkrun is an international movement of weekly 5 km walk/run events held in public spaces. Parkrun has a broad appeal to various population groups including non-athletes, however no previous reports had evaluated parkrun as a promising PA promotion setting for KOA. Our study will determine the feasibility, acceptability and safety of parkrun as a novel strategy to promote PA for people with KOA.

Assessing the Australian osteoarthritis landscape - GPs' surgeons' and patients attitudes towards and understandings of osteoarthritis (2018)$24,824
Description
Osteoarthritis is a leading cause of pain and disability among older people and there is currently no cure. In recent years, there has been increasing concern about the discrepancies between care provided by health professionals and recommendations in osteoarthritis clinical practice guideline. For instance, in Australia, referral to a surgeon is five times more common than referral to allied health professionals that could help with conservative management. Furthermore, patients are ambivalent towards conservative management such as exercise, thinking it will be ineffective and may even cause harm to their already worn joints. The disconnect between practice guideline and care for patients with osteoarthritis limits optimal care. Therefore, this study aims to explore the attitudes towards and understanding of osteoarthritis held by Australian GPs, surgeons and patients with knee osteoarthritis. The overarching goal is to gather information needed to develop Australian-specific education and behaviour change programs for GPs, surgeons and osteoarthritis patients

Improving outcomes in patients with knee osteoarthritis: identifying pain phenotypes with relationships to long-term health outcomes (2018)$9,669
Description
Knee osteoarthritis is a highly heterogeneous condition characterised by a complex and multifactorial nature. The pain experience in knee osteoarthritis is similarly individualized and complex; some patients may present with pain that appears attributable to classic signs of joint damage, while others may present with pain due to psychological distress or central mechanisms. However, whether these variables (or interactions between variables) are representative of different pain phenotypes in knee osteoarthritis and whether patients with distinct pain phenotypes have different risk for long-term health outcomes are unknown. This project aims to identify the pain phenotypes in OA patients and compare the risk of long-term health outcomes between different pain phenotypes. This study will help clinicians better prioritize and target interventions to individual patients and will lead to better outcomes and fewer treatment complications currently associates with pharmaceutical and surgical interventions that are widely used to treat knee osteoarthritis.

Understanding the perspectives of general practitioners regarding sedentary behaviour management in primary care: a qualitative study (2018 - 2019)$54,977

Despite the growing population-based evidence that demonstrated the strong linear association between sedentary behaviour and increased risk of chronic disease including type 2 diabetes and premature mortality, it is currently unknown how general practitioners (GPs) address this important risk factor in primary care. As GPs have the potential to implement health-related behavioural change as their advice is often respected and they are the 'front line' of the Australian health care system, we aim to understand the perspectives of GPs regarding the importance of sedentary behaviour assessment and management in primary care. To achieve this, we will explore the following research questions using an inductive qualitative research design and open-ended interview questions: (1) How do GPs perceive sedentary behaviour assessment and management in primary care? (2) Are GPs aware of sedentary behaviour management recommendations and/or of any strategies to implement sedentary behaviour management? (3) What are the GPs perceived barriers and facilitators to addressing sedentary behaviour assessment and management in primary care?

Description
This project is a case-control investigation into the clinical and metabolic factors and imaging abnormalities that are associated with chronic plantar heel pain.AimsThe primary aims of this study are:1. in a case-control comparison, to determine which of the following factors are associated with CPHP: a.imaging abnormalities on ultrasound (US), magnetic resonance imaging (MRI) & high resolution peripheral quantitative computed tomography (HRp-QCT);b.clinical factors; c.blood levels of inflammatory biomarkers and cytokines; andd.blood lipid levels, fasting glucose and glycosylated haemoglobin (HbA1C).e.psychological factors2.in cases, to determine which clinical and psychological factors, inflammatory biomarkers and cytokines, metabolic indicators and imaging abnormalities predict change in foot pain and/or function over 1 year.The specific clinical measures we will take at baseline include body mass index, waist girth, body (fat) composition, ankle strength and range of motion, range of motion of the big toe joint, foot alignment, and modified straight leg raise (a measure of nerve sensitivity to stretch). We will also monitor physical activity over 7 days with small waist mounted 'accelerometers', and take a comprehensive survey of medical, social and treatment history to date. The specific imaging assessments include ultrasound to measure thickness, quality and blood supply of the plantar fascia, high resolution peripheral quantitative CT to measure calcaneal bone density and structure, and MRI to assess for bone, nerve and soft tissue pathology in the heel. We will also use standardized questionnaires to assess for psychological drivers of pain such as depression and anxiety. We will take bloods and store these for later processing (contingent on us acquiring funding) to assess for novel systemic markers of inflammation and metabolic function (eg cytokines, lipids, glucose). The number of participants has been determined by formal statistical sample size calculations to ensure the study is large enough to detect any clinically important associations. We also intend, subject to funding, to undertake a 12-month longitudinal follow up of cases to assess for predictors of change in foot pain, function and quality of life. We will re-assess all clinical measures (but not foot posture) and the ultrasound, but not the HR-CT, MRI or blood-based measures.

Does vitamin D supplementation improve bone density in vitamin D deficient children? Establishing an international dataset to allow an individual patient data meta-analysis (2017)$7,000

Our previous study-level (aggregate data) meta-analysis suggested that vitamin D supplements may be beneficial for bone density specifically in children with vitamin D deficiency. However, the misclassification of vitamin D status inherent in study-level data means that the results are not definitive and cannot provide an accurate assessment of the size of any effect. Therefore, we propose to undertake an individual patient data (IPD) meta-analysis to determine whether the effect of vitamin D supplementation on bone density in children differs according to baseline vitamin D status, and to specifically estimate the effect of vitamin D in children who are vitamin D deficient.

Description
Prevalence of osteoarthritis (OA) spikes after a certain age and interestingly, femoroacetabular impingement (FAI) is a potentially modifiable risk factor of OA, as it can be treated surgically. However, there are no large longitudinal community based study combining structural, radiographic and clinical parameters of FAI to study the risk of the development of osteoarthritis. In this study of 1099 (2198 hips) people from the community we will be using a computerized method to estimate the proportion of FAI. Furthermore we also would explore its link with pain, cartilage damage and other bone structure changes relevant to OA. Assuming there is a link between FAI and all these factors, this study would be the largest and the first to define the proportion of FAI in Southern Australia and also help in identifying people who are at who are at higher risk of hip OA.

Making guidelines meet the challenge of multimorbidity in general practice: development of an evidence-based approach to integration of disease specific guidelines to better manage multimorbidity (2015)$80,453
Description
The aim of this study is to design an evidence-based approach to developing and implementing clinical guidelines for GPs that incorporate the complexities of managing multimorbidity. The project will do this by using answers from the following research questions: 1) is multimorbidity addressed within major Australian chronic disease guidelines? If so, how?; 2) is multimorbidity addressed specifically in prevention and treatment recommendations for physical activity promotion/exercise prescription?; 3) how do physical activity promotion/exercise prescription recommendations compare across different disease guidelines and what are the identifiable areas of commonality and/or contradiction across guidelines?; 4) how might physical activity promotion/exercise prescription recommendations in different guidelines be integrated in a way which will better support GP's management of multimorbidity? This study will focus on common chronic diseases which account for around 60% of the total chronic problems managed in general practice and which are also commonly seen as co-morbidities, namely: cardiovascular disease and its risk factors (hypertension and lipid abnormalities); musculoskeletal diseases (osteoarthritis, osteoporosis and back pain); diabetes mellitus; asthma and chronic obstructive airways disease; and depression. For research questions 1 to 3, the project will conduct content analysis of Australian evidence-based guidelines for management of chronic disease in primary care. Steps in this will include: 1) a systematic search of both the scientific and grey literature to identify existing Australian guidelines; 2) assessment of guideline quality against the Scottish Intercollegiate Guideline Network (SIGN) methodology; 3) data extraction and content analysis. For research question 4, the data from content analysis will be used to develop potential models of integrating recommendations from multiple guidelines into a form that is readily usable by GPs. The project will explore how to present information on multimorbidity in guidelines for primary care, with reference to physical activity/exercise qualitatively using key stakeholder interviews and focus groups with general practitioners to test alternative methods of presentation of guidelines information on managing physical activity recommendations in the presence of multimorbidity. Feedback will be sought on clinical relevance, usability, applicability to different multimorbidity scenarios, and applicability of existing tools to help practitioners discuss management of multiple diseases with their patients. The project will develop a guideline model for physical activity which as a next step will be ready for formal testing by randomised controlled trial.
