What Supporting Documentation is Required?
If you have a Learning Assistance Plan, it may include an accommodation to request an extension without a medical certificate. Contact your Unit Coordinator to let them know about your plan. They will then be able to advise you. You may be asked to provide a copy of your Learning Assistance Plan as supporting documentation.
Where possible, provide a copy of supporting documentation when requesting an extension. If this is not possible, you may be able to provide it within a reasonable time after your initial request. Contact your Unit Coordinator for advice on this.
Copies can be a digital scan or photo saved in a common file format (for example, PDF or JPEG).
Medical Reasons
For medical reasons, provide a copy of a Medical Certificate from a health care professional or a Statutory Declaration that supports your request.
Other Reasons
You may require an extension for reasons such as bereavement, family crisis, an accident, religious observance, jury duty or Defence Force Reserve duties. Provide a copy of a Statutory Declaration or other documentation (such as a jury summons) that supports your request.
Making a Statutory Declaration
A Statutory Declaration is a statement that you declare to be true. An authorised witness watches you sign this statement and then also signs it.
