Facilitating Quality Use of Medicines (QUM) between hospital and community (Med eSupport)

This project investigated and evaluated the potential of community pharmacy to contribute to the quality use of medicines by consumers as they move from one health setting to another, in particular between hospital and home.

This project was funded by the Department of Health and Ageing via the Third Community Pharmacy Agreement.
