Improving the safety and efficacy of anticoagulant drug therapy in clinical practice

This project implemented systems to improve the management of anticoagulants in community and hospital care settings. This involved the use of quality improvement methodology and innovative patient-focussed IT strategies.

Incorporated in this project was an intervention to improve adherence to the Royal Hobart Hospital (RHH) anticoagulation initiation protocol and enhance patient safety for patients initiated on warfarin at the RHH.

This project was funded by the National Institute of Clinical Studies Scholarship.
