Introduction
This unit will enhance your ability to care for people in their last year of their lives. You will develop skills in recognising; responding, renegotiating and reinforcing appropriate care and assessing the ‘domains of distress’ experienced by the person. You will also extend your knowledge and skills in communication, collaboration and decision making that is centred around the person, their family and care givers. The unit will enhance your ability to provide consultative and compassionate care, and implement shared decision making.

Learning Outcomes
- Discuss the contemporary and sociocultural experience of death and dying and recognise when a person may be in the last year of their life.
- Respond with meaningful dialogue and evidence-based assessment skills to establish current and potential unmet needs
- Establish person centred goals of care, renegotiate and realign goals and desired outcomes.
- Explain the supports that exist to enable the person to achieve their goals in the last year of their life.
