Introduction
Leadership in Medication Management in Multidisciplinary Teams expands on the pharmacy practice content learned throughout the degree to explore the roles pharmacists have within wider health care. In this unit, students will undertake an elective placement. Elective options include extended community practice, aged care, general practice and a range of other health or industry-related placements. The University teaching-intensive weeks will focus on the development of knowledge and skills in medication management, vaccination, and extending knowledge in developing areas of pharmacy practice within multidisciplinary teams.

Learning Outcomes
- Communicate with the patient and collaborate with health care professionals to facilitate patient-centred care within the management of acute and chronic disease.
- Identify critical issues, conceptualise problems, use relevant information and formulate a range of solutions in the provision of medicines or health advice.
- Analyse activities and settings where a pharmacist may have an impact on patient outcomes.
- Design a preventative health care service.
