To cancel your Fixed Term or salary sacrifice membership, please contact your relevant Unigym centre.

Direct Debit Cancellation

Terms and conditions * Unigym must receive this form by Monday of the week of payment *Access to the Unigym will strictly cease on the date up to which you have paid.*I acknowledge that if it is after the Monday of the week of payment, I will still be liable for the next membership payment.*I acknowledge that I will be liable to pay a cancellation fee of $35 (at the time of cancellation) if my membership period has been less than 12 months. *I acknowledge that I will be liable to pay any outstanding amounts owing to Unigym or the amount for any Unigym Activities supplied but not yet paid for.
Payment agreement * I understand that Unigym will deduct a $35.00 fee from my account. $35.00 will not be deducted if you have been a member for longer than 12 months or a relevant medical certificate has been supplied.
